Abstract
Healthcare excellence is a fundamental aspect of national development, and hospitals
play a critical role in delivering quality medical services. This paper compares the
healthcare standards of two major institutions: the Pakistan Institute of Medical
Sciences (PIMS), Islamabad, and the Dalian Medical University Hospital (DMUH),
China.
The comparison is based on infrastructure, patient care quality, technological
advancements, research contributions, and affordability. The study provides insights into
healthcare excellence in two different regions and offers recommendations for
improvement. It also discusses healthcare policies, government funding structures, and
the impact of medical education on hospital performance.
By analyzing patient care, accessibility, and institutional challenges, this paper provides

1 Introduction
Healthcare institutions serve as the backbone of medical advancements and patient care.
PIMS, a leading healthcare provider in Pakistan, and DMUH, a prominent Chinese
medical institution, offer unique insights into the healthcare systems of their respective
countries. This study examines their strengths, challenges, and contributions to global
healthcare excellence. The research aims to compare the healthcare infrastructure,
evaluate the quality of patient care and medical research, assess technological
advancements, and analyze affordability and accessibility in both hospitals. Moreover,
this study investigates government policies, investment in public health, and
collaborations with international healthcare organizations to provide a holistic view of
hospital effectiveness and service delivery. Founded in 1947 as Guandong Medical
College, Dalian Medical University (DLMEDU) has developed into a multi-disciplinary
medical university located in the coastal city of Dalian. As the biggest harbor city in the
Northeast China region, Dalian is an important international shipping center, logistics
center and financial center in Northeast Asia. Owning an excellent environment and
temperate climate, Dalian was firstly listed as the “Best Tourist Cities” of China. In 2009,
it was recognized by UNEP and awarded the highest prize of international garden city by
IFPRA. DLMEDU is the largest medical university in China, which enrolls the most
international students, as much as 1300 from 82 countries in degree programs. It
occupies an area of 373 acres with building area of 94 acres and contains 22 teaching and
scientific research institutions, 5 supplementary teaching organizations, 1 branch college,
1 affiliated medical school, 9 affiliated hospitals (2 are directly affiliated hospitals), over
100 teaching bases, 1 state level International SciTech Cooperation base, 1 ministerial
level research institution, 1 ministerial level lab, 10 provincial level key labs, 1
provincial level engineering technology research center and 6 provincial key labs for
universities.

2 Background Information

2.1 Pakistan Institute of Medical Sciences (PIMS)

The Pakistan Institute of Medical Sciences (PIMS) was established in 1985 and is
located in Islamabad, Pakistan. It is affiliated with the Ministry of National Health
Services, Regulations, and Coordination. With a bed capacity of 1,200, PIMS is one of
the largest public-sector hospitals in Pakistan. It offers a wide range of medical services,
including cardiology, oncology, pediatrics, emergency care, and surgery. PIMS also

serves as a teaching hospital, providing medical education and training to students and
residents. Its primary role is to deliver affordable healthcare to a large population,
particularly low-income groups. The Pakistan Institute of Medical Sciences (PIMS)
Islamabad is the constituent Hospital of Shaheed Zulfiqar Ali Bhutto Medical University.
PIMS is the country’s premier post-graduate medical Centre since 1988. It is one of the
region's leading tertiary level hospitals, which includes 22 medical and surgical specialist
centers. The institute includes three semi-autonomous hospitals, including the Islamabad
Hospital (IH), which is a 592-bed hospital, the 230-bed Children’s Hospital specializing
in pediatric care and the 125-bed Maternal & Child Health Care Center, which
specializes in obstetrics and gynecology.

The Pakistan Institute of Medical Sciences (PIMS) is a vital healthcare and medical
education hub in Pakistan. With its diverse medical services, advanced research, and
training programs, PIMS continues to play a critical role in improving public health and
medical standards in the country.

2.2 Dalian Medical University First Affiliated Hospital (DMU


Hospital)

The Dalian Medical University First Affiliated Hospital (DMU Hospital) was established
in 1930 and is located in Dalian, China. It is affiliated with Dalian Medical University
and has a bed capacity of 2,000. DMU Hospital is renowned for its advanced medical
technology and research output.

It specializes in oncology, cardiology, neurology, organ transplantation, and traditional


Chinese medicine. The hospital serves as a major referral center for complex medical
cases and is a leader in medical education and research in Northeast China. The hospital
is engaged in clinical teaching of 18 academic fields at 8 different levels, which are
composed of 23 teaching and research departments, 39 third-level disciplines, 82 clinical
departments, 13 examination departments, 25 administrative departments and 25
teaching bases. The hospital has two first-level doctoral programs, Integrated Traditional
Chinese and Western Medicine and Clinical Medicine. Additionally, Integrated
Traditional Chinese and Western Medicine is the national key program and national key
specialty. The key laboratory of critical cases of Integrated Traditional Chinese and
Western Medicine in Liaoning province is the provincial key laboratory. The hospital
has 6 national clinical key specialties accredited by the National Health and Family
Planning Commission, followed by Nursing, Cardiovascular, Nephrology, ICU, Acute
Abdomen Surgery of Integrated Traditional Chinese and Western Medicine and
Orthopedic Surgery. It also comprises 8 Liaoning provincial medical centers, 7 Liaoning
provincial key programs, as well as 15 Dalian municipal key programs. Sino-UK
Regenerative Medicine Center is the “International Cooperation Base in Science and
Technology” accredited by the Ministry of Science and Technology. There are a couple
of well-known medical specialists in this hospital. Among the 3900 staff members, more
than 600 of them have senior professional titles, one nurse was awarded the Florence
Nightingale Medal, and 40 specialists are enjoying special government allowances.

2.3 Brief Introduction

The first affiliated hospital Founded in 1930, Dalian Medical University's first affiliated
hospital Having developed over eighty years, it is presently a nationally well-known,
contemporary, comprehensive, first-ranked, state-owned hospital integrating clinical
services, medical education, and scientific research. Covering a 182,000 square meter
building area, the hospital comprises the First Campus, the Second Campus, the Third
Campus and Quanyong Campus. Comprising 3,700 approved beds, the hospital receives
over 2.16 million outpatients and over 90,000 inpatients and performs almost 30,000
inpatient operations yearly. It has recently become the strongest overall strength and the
most trusted medical research-oriented affiliated hospital among the population.
Comprising 23 teaching and research departments, 39 third-level disciplines, 82 clinical
departments, 13 examination departments, 25 administrative departments and 25 teaching
bases, the hospital is involved in clinical instruction of 18 academic subjects at 8 distinct
levels. Two first-level doctorates from the hospital are Clinical Medicine and Western
Medicine and Integrated Traditional Chinese Medicine. Furthermore, the national major
program and national key specialty is Integrated Traditional Chinese and Western Medicine.
Orthopedic surgery combined with traditional Chinese and Western medicine. Along with
15 Dalian municipal significant programs, it also consists of 7 Liaoning province key
projects and 8 Liaoning provincial medical centers. Certified by the Ministry of Science

and Technology, the Sino-UK Regenerative Medicine Centre is the "International


Cooperation Base in Science and Technology". This facility has a few well-known
medical experts. Of the 3900 employees, more than 600 hold senior professional titles;
one nurse received the Florence Nightingale Medal; 40 experts are benefiting from the
special government allowances.
The hospital has always been following the building of discipline and seeing medical
progress as the fundamental goal. The hospital has conducted 853 research projects since
2000 and received 121 national and provincial level awards for its scientific and
technological successes. SCI has referencing 418 publications. Clinically, hundreds of
novel approaches have been used these years. The hospital's medical technology now
stands advanced on a national scale.
As the hospital develops, several kinds of academic conferences are supported or planned
annually. Medical institutions in more than a dozen different nations and areas have
created academic ties. Hospital management has been much more improved in recent times.
The implementation of the ISO 9001 quality certification system has made hospital
administration more and more scientific and uniform. The hospital information system
has helped the institution to digitize its administrative records progressively. One hundred
hospitals in China, under the name "National Advanced Collective of Establishing
Civilized Enterprise." The hospital ranked sixth in the patient satisfaction assessment
conducted by the National Health and Family Planning Commission, with four metrics
leading the way. Attending the sixth National College Students Clinical Skills Contest on
behalf of Dalian Medical University, the hospital won the excellent honor in the finals in
May 2014. Furthermore, the hospital got the twelfth position in the top employers'
research of Chinese medical institutes. In the list of Chinese medical institutes with the
best reputation, our hospital came in sixth overall—first class in northeastern regions.

2.4 Healthcare Infrastructure

PIMS provides essential healthcare services to the Pakistani population with a relatively
older infrastructure. It has emergency services, ICUs, and specialized units but lacks
high-end robotic surgery facilities. In contrast, DMUH is a modern hospital with
state-of-the-art facilities, including robotic-assisted surgeries, AI-powered diagnostics,
and telemedicine. PIMS has limited integration of digital health records, whereas DMUH
is fully digitalized with AI-driven diagnostic tools and telemedicine services. Government
investment in healthcare infrastructure has played a pivotal role in DMUH’s advancement,
whereas PIMS faces resource constraints due to limited public healthcare spending.

The MBBS program for international students in DLMEDU was set in 2004, and in 2007,
the MOE declared it as the qualified university to offer an English-taught MBBS program.
The program is open each spring and autumn, with the same duration and tuition fee.
Besides, it can be taught in English and Japanese upon students’ choice. According to the
framework agreement on degree accreditation between the MOE of China and other
countries, the degree awarded by DLMEDU is recognized in 31 countries. In addition, the
university also signed agreements with medical associations in Thailand, Ghana, and Sri
Lanka.

The doctor-to-patient ratio at PIMS is approximately 1:1300, whereas at DMUH, it is
significantly lower at 1:600, indicating better-personalized care. Surveys show that patient
satisfaction at DMUH stands at 87%, compared to 65% at PIMS. This difference is
largely due to better facilities, shorter waiting times, and advanced medical procedures

3 Comparative Analysis

3.1 Infrastructure and Facilities


The infrastructure and facilities of a healthcare institution are critical determinants of its
ability to deliver quality care. PIMS, with its 1,200 beds, is one of the largest public
hospitals in Pakistan. It provides basic to moderately advanced medical equipment and
has specialized departments for cardiology, oncology, and pediatrics. However, the
hospital often faces challenges such as overcrowding and resource limitations, which
impact its efficiency.
In contrast, DMU Hospital boasts a larger bed capacity of 2,000 and is equipped with
state-of-the-art medical technology. The hospital has advanced specialized departments,
including oncology, cardiology, neurology, and organ transplantation. It also features
modern trauma centers and emergency care facilities. The infrastructure at DMU Hospital
reflects China's significant investment in healthcare modernization.

to PIMS. However, PIMS plays a critical role in providing affordable healthcare in a
resource-constrained setting.

PIMS has limited research output due to financial constraints and primarily contributes to
national-level health studies. In contrast, DMUH publishes hundreds of research papers
annually and collaborates with several international universities. While PIMS produces
around 50 research papers annually with minimal international collaborations, DMUH
contributes over 400 research papers and engages in extensive global research
partnerships. The Chinese government provides significant funding for research initiatives,
allowing DMUH to stay at the forefront of medical innovations.

3.3 Affordability and Accessibility

PIMS provides affordable healthcare, with treatment costs ranging between $50 and $500,
subsidized by the government, making it accessible to low-income patients. DMUH,
though offering world-class medical facilities, is significantly more expensive, with
treatment costs ranging from $500 to $5000, particularly for international patients. The
difference in cost reflects the disparity in healthcare funding and investment in medical
technology.

4 Impact on Public Health

4.1 PIMS, Islamabad

PIMS provides essential healthcare services to a large population, particularly low-income


groups. It plays a key role in managing infectious diseases and maternal health but is
limited by resource constraints and overcrowding. Despite its contributions, PIMS faces
challenges such as overcrowding, resource limitations, and infrastructure constraints. The
government and healthcare authorities are working on expansion plans, technological
upgrades, and staff capacity building to enhance its service delivery and maintain its
status as a leading medical institution.

4.2 DMU Hospital, Dalian

DMU Hospital serves as a major referral center for complex medical cases and
contributes significantly to medical research and education. Its focus on advanced
treatments and preventive care enhances public health outcomes. The hospital was
successively entitled the “National Advanced Collection in Health Care” three times. It
was enlisted in the “Top One Hundred Hospitals” in China and labeled as “National
Advanced Collective of Establishing Civilized Enterprise”. In the patient satisfaction
survey taken by the National Health and Family Planning Commission, the hospital took
fifth place, and 4 indicators of these data took the lead. The hospital, on behalf of Dalian
Medical University, attended the fifth National College Students Clinical Skills Contest
and gained the outstanding award in the finals in May 2014. Furthermore, the hospital
ranked the twelfth place in the best employers’ investigation of Chinese medical
institutions. In the list of Chinese medical institutions with the highest credibility, our
hospital ranked seventh place throughout the country, which is the first class in northeast
provinces. The university has carried out the exchange and cooperation in domestic and
abroad widely. Till now, it has established cooperative relationships with University of
Sao Paulo Medical School in Brazil, The University of Adelaide in Australia, the Ohio
State University in USA, the University of Groningen in Holland, University of Toyama
in Japan, University of Oslo in Norway, BaFuLuoFu State Medicine University in Russia,
Dnepropetrovsk National University in Ukraine etc 99 universities or scientific
institutions in 38 countries and regions.

5 Strengths and Weaknesses

5.1 PIMS, Islamabad

PIMS's strengths lie in its affordability and accessibility, making it a critical healthcare
provider for low-income populations. However, the hospital faces challenges such as
overcrowding, long waiting times, and limited research output.

5.2 DMU Hospital, Dalian

DMU Hospital's strengths include advanced medical technology, efficient patient


management, and a strong research culture. However, its higher costs and limited focus
on low-income populations are areas for improvement.
