"University" under Choose your educational institute type.
2. Fill in the Student’s details (Use your IIM Calcutta email address only).
3. Fill in the Parent’s details.
4. For University and Course Details, enter the following:
o State: West Bengal

o University Director's Name: Saibal Chattopadhyay
o University Phone Number: 33 2467 8310
o Details of the Course (UG/PG/Diploma): Post Graduate Diploma
o Name of the Course: PGDBA
o Current Year of Study: 1
o Nearest Police Station: Joka
5. Confidential Survey:

6. Select all checkboxes and submit the form.
7. After submitting the form, take a screenshot of the confirmation details shown on
the next window.
