Subject : Clarification of Surname for Visa Application - [Rajesh Kumar]

Dear Consulate General,

I am writing to provide clarification regarding the surname discrepancy in my child's and my


own documents, particularly in relation to our application for a visa to Germany.

Upon reviewing the documents required for the visa application, I have noted that my
surname, "Pathak," was not consistently recorded in my educational records during my
schooling years. This oversight inadvertently extended to subsequent official documents.
However, I wish to confirm that my correct surname is indeed "Pathak," which is consistently
reflected in other official documents such as my Voter ID card.

To substantiate this clarification, I am enclosing copies of my Voter ID card, clearly


indicating my surname as "Pathak." Additionally, I have obtained an Article 4 affidavit from
the Indian Court, affirming my legal surname as "Pathak," inherited from my parents.

And I do have submitted my Daughters birth Certificate for the Name Validation.

I kindly request your understanding and consideration of this matter in processing our visa
application, particularly for my child. Should you require any further documentation or
information to facilitate this process, please do not hesitate to contact me.
