Questionnaire Design for High School Research

The research instrument utilized for data collection was a questionnaire designed specifically for Senior High School Grade 12 students, enabling efficient data gathering. The researchers created and validated the questionnaire through feedback from students at Tomas Claudio Colleges to ensure clarity and relevance to the study's objectives. After finalizing the instrument, the questionnaires were distributed personally to the respondents for data collection.

Research Instrument

The instrument used to collect the data was gathering the questionnaires. This was used because it
gathers the data faster then any other methods. Besides the respondents of the study were Senior High
School students in the grade 12 department. They could read and analyze each question properly to
understand the situation with ease.

The researcher prepared questionnaires, the draft questions was drawn and made by their own
understanding, readings and observations, reviewed and published based on the relevance of the study.
The researchers saw that there were enough items and data’s to be collected to cover all the aspects of
the problem that is being faced in the current situation, and to answer the specific questions under the
statement of the main problem. Then, the researchers have submitted the questionnaires to its adviser
for it to be finalized.

For validation, the copies of the questionnaires were given to the students of Tomas Claudio Colleges,
Morong, Rizal. The students of the Senior High School Department from the Grade 12 Department are
required to answer the questionnaires to validate the self-made questionnaires. After gathering the data,
they had validated the copies of the questionnaires to be passed to their subject teacher Ms. Christine
Joy S.J Zafra, these persons were given papers to assess the students. They were asked if the questions
were clear and understandable; the number of items were collected to make sure that they had enough
respondents to validate their study; the researchers should also check if all the objective are matched
and not biased; and if all of it were relevant answers to the research problem.

The copies of the questionnaires were then distributed personally by the researchers to their
respondents.
