The student authorizes and gives their consent to the Office of the University Registrar to collect, process, and store their personal information strictly for graduation purposes, in accordance with the Data Privacy Act .

Students with a board exam are required to submit 2 passport size photos with nametag and white background along with 2 documentary stamps. Non-board exam students must submit 1 passport size photo with nametag and 1 documentary stamp .
