Step 1: Identification of Terms

are the principles of bioethics in medicine?

Step 3: Problem Analysis

1. In the medical profession there are 4 main moral principles in medical ethics, namely:
[Link] Beneficience, which is a moral principle that prioritizes actions aimed at
the patient's good
· Protecting and protecting the rights of others.
· Prevent harm that can befall others.
· Removes conditions that can harm others.
· Helping people with various disabilities (disabilities).
· Helping people who are in danger.
c. The principle of non-maleficence, which is a moral principle that prohibits
actions that worsen the patient's condition. This principle is known as "primum
non nocere" or "above all do no harm".
This principle is related to the Hippocratic expression which states "I will use
therapy to help the sick according to my abilities and opinions, but I will never
use it to harm or harm
them ”. The principle of non-maleficence is often discussed in the medical field,
especially in controversial cases related to cases of terminal illness, serious
illnesses and serious injuries.
This principle plays an important role in making decisions to maintain or end
life. Its application can be performed on both competent and incompetent
patients. Basically, the principle of non-maleficence
provides opportunities for patients, their guardians and health workers to accept
or reject an action or therapy after weighing the benefits and obstacles in certain
situations or conditions.
d. The principle of justice , namely the moral principle that emphasizes fairness and
justice in distributing resources (distributive justice).

· For each person based on the needs

It is important to implement the principles of autonomy is assessing the


patient's competence.

2. Fragment Surah Al Maidah: 2

‫وتعاونوا على ٱلبر وٱلتقوى ولا تعاونوا على ٱل إثم‬

means:

"And please-menolonglah you in the (working) virtue and piety, and do mutual
assistance in sin and transgression"

From the verse It is known that Islam encourages humans to be able to help others, especially
in things that lead to good and of course positive, and prohibits humans from helping each
other in negative things that only cause sin. When it is related to the medical profession, in
Islam, it does not consider differences based on nationality, race, culture, language, rich and
poor. All have the same right to do good or are needed well. So a doctor must not be selective
in helping and treating the community.

3. Development In the meantime, Professor Kaiser Ali (Canada) in his presentation at


the National Meeting of the Indonesian Network for Bioethics and Health Humanities
(JBHKI) in Surabaya 2006 stated that medical bioethics is a moral aspect of medical
science (Practice of Moral medicine). ). Nowadays it is also very common for us to
hear the term "Bioethics and Health Humanities" or Health bioethics and humanities.
Medical humanities contain the understanding of the human aspect of medical
science (Practice of Human medicine), because we know that the sciences of
medicine, morals and humanity cannot be separated from one another.
4. Islamic bioethics is based on the principle of equality (in addition to piety), the
brotherhood of man (in tawhid), the freedom to choose without coercion (principle of
selectivity and creativity) sehinggaadanya limit freedom of choice, and the ultimate
goal must be consistent way to truth, goodness only from God and fairness between
rights and obligations that respect individual contributions and efforts or choices.
Islamic bioethics is a combination of rights and obligations, freedom and
responsibility, reasoning (logic) and conscience, revelation and tradition, worship and
muamalah, goals and methods, principles and values, to lead to a balance of human
potential both worldly and ukhrowi potential, plant potential (physical), animal
potential (body and instinct), intellect potential (mental), angelic potential (spiritual).
Islamic bioethics have principles, among others; 1) Tabligh principles, 2) Amanah
principles, 3) Ukhuwah principles, 4) Fathonah principles, 5) Sincerity principles, 6)
Kaffah principles, 7) Shiddiq principles, 8) Uswatun hasanah principles, 9) Rahmatan
lil'alamin principles, 10 ) Yaqin principle, 11) Adil principle, 12) Daulah principle
and 13) Istiqomah principle, which means:
Tablighprinciple
Tablighmeans conveying or informative, doctors must convey what is the patient's
right, convey whatever information the patient needs, for example informed consent
This principle emphasizes the importance of providing information and
communicating effectively, as is and without being hidden for the benefit of the
patient. Hadith says Qulil Haqqu walaukana muuran. This includes information about
religion, health, medicine and social affairs. Islam teaches its people to always
convey the message of truth and justice. Silence when there is a violation of ethics is
not the right attitude. Silence when there is a violation of ethics is the same as a
perpetrator of a passive ethical violation.
The principle of Amanah
Amanah means maintaining trust or Veracity, doctors must be able to maintain trust
and trust, and always keep patient secrets even until the patient dies. Safe means
trustworthy. This principle emphasizes that doctors act based on contractual
agreements with patients. The patient entrusts his trust that the doctor will do his best
and will not harm him. With this principle, doctors are expected to be able to meet
the expectations of belief based on these mandates. Doctors have an obligation to act
according to the minimum standard of medical services and that is a professional
mandate. Awareness of this principle will require doctors to keep promises and
maintain confidentiality.
Ukhuwah Principles Ukhuwah
means brotherhood or cooperative, communicative, doctors must be able to cooperate
with anyone, must always maintain good relations with others, ukhuwah Islamiyah,
ukhuwah human. The doctor sees that the patient is a part of himself. As a human, the
patient has the same potential and basic characteristics as the doctor so that he wants
the same treatment as the doctor wants to be treated. Doctors feel what patients feel is
also felt by remembering that they are brothers and sisters who are like one body.
The principle of Fathonah
Fathonah means smart = Life long study / learning, a doctor must continue to learn
throughout his life, gain knowledge and hone his skills on an ongoing basis

according to their responsibilities and authority. Doctors are able to make decisions

Principle of Sincerity
In taking medical action, it is always based on the intention of dedication or altruism
solely because of Allah at the time before starting, at work and after work. angry
when reviled, not proud of themselves when praised and there is money or money is
not ready to serve.
Kaffah Principle
Doctors must do their best for patients, exert their ability and efforts for patient
safety.
Sidq /honesty
Sidqor honesty includes honest words, honesty in promise, honest deeds, honest in
relationship and honest in heart. Honest words means that they are in accordance with
reality. Being honest in your promises means trying to keep your promises and not
breaking promises. Honest in deeds means actions that are done according to his heart
so that the inside and outside are in accordance. Being honest in your relationships
means interacting with patients without any element of deception. Honest in heart
means sincere intention and motivation in the heart that will not be easily swayed by
various obstacles, obstacles and obstacles from anywhere. Honesty near truth and
truth leads to true happiness. Shiddiq = Truth telling, Truth doing, the doctor must
realize that what he is doing is right, what he knows is true and what he says is
nothing but truth.
Uswatun hasanah
Principles This principle is a combination of the principles of beneficence and non-
maleficence as in western bioethics. It's just that this principle is framed by the
principle of tawhid. The principle of tawhid means believing and relying on
everything from Allah and with the permission of Allah as causa prima. Uswatun
hasanah = Beneficence, doctors must realize that what they do is solely for the good,
satisfaction, healing and benefit of the patient.
Rahmatalill'alamin
Principle This principle emphasizes the importance of the value of pragmatism in
every doctor's action. An action will conform to this principle if it is able to benefit
the patient indiscriminately. From the origin of the word grace which means goodness
or benefit from Allah who is gracious for all his creatures. With this principle, doctors
will always prioritize goodness for the benefit of their patients. Doctors must always
have a firm intention in their hearts to give love, save the patient's soul, apply the
principle of aegroti salus lex suprema (patient safety is the main thing).
The Principle of Trust
This principle is a logical consequence of the principle of freedom. After the choice
has been made, the patient must be bound and already limited by the choice made on
the basis of awareness and the right to self-determination. Yaqin = Accountable, a
doctor must be sure about what he is doing because everything will be held
accountable before ALLAH SWT, and also fellow humans.

The Principle of Fair


Fair = Juctice, doctors must always treat everyone fairly, not discriminate, do not
stigmatize and classify patients. This principle emphasizes the importance of sharing
in matters of truth, maslahah and goodness proportionally, not taking sides with one
party, fulfilling the principle of balance between rights and obligations, and fulfilling
the principle of degree of need. In all actions or actions doctors will not discriminate.
Services provided to patients are due to humanitarian calls so that doctors will treat
them the same because of their humanity. Different treatment is justified if it is based
on accuracy and merit.

The principle of Sovereignty

= Autonomy, doctors must always respect the rights of patients, respect human rights
and dignity as a complete biopsychosocio-cultural being. Departing from the story of
Umar which reminded Amar bin Ash when he wanted to evict Jewish land to expand
the mosque. Umar cut his sword on the bone and ordered the Jews who asked for
justice to bring the bone to Amr bn Ash so that Amr bin Ash canceled to expand the
mosque. This principle emphasizes the importance of patients given the best choice
and given the freedom to choose without any pressure or without any persuasion.

The principle of Istiqomah

Istiqomah = Excellence, doctors must always do their job seriously and intend to
continuously improve themselves to perfection.
