a) Noticing patients having limited information about their disease condition
b) In my role as a bedside nurse in the CCU, I noticed that many patients were anxious and had limited
understanding of their conditions and treatment plans. To address this, l always made sure I take
sometime to educate patient about heart health, medication management, and lifestyle changes. I
used A4 size white papers to draw diagrams and explain the reason behind heart attacks. Focussing
also on effects of nicotine due to smoking on the blood vessels, giving good examples of diet plans
mainly what to avoid for people with obesity issues . I noticed that my initiative to increase their
knowledge about their condition actually was benefiting them and they were taking very good interest
to make a change in their lifestyle and they understood their aftercare before leaving the hospital.
c)This initiative not only empowered patients with knowledge but also improved their engagement in
their care, leading to better outcomes and higher patient satisfaction . I got a good feedback from a
patient who was readmitted who gave up smoking completely and lost good bit of weight during his
recent admission.

a) Dealing with an Extremely Anxious Patient
b) A few weeks ago, I encountered an 81-year-old man named Michael who arrived with crushing chest
pain and was diagnosed with a Code STEMI. He was airlifted to the Cath lab at GUH, where a coronary
angiogram revealed triple vessel disease, with a history of eight stents and some in-stent restenosis.
The results showed 100% plaque formation in the LAD, RCA, and LCx, making it a challenging procedure
to place any additional stents.
Upon arrival in the CCU, Michael was still experiencing chest pain and was started on an Isoket infusion.
He was extremely anxious about his condition, and his spouse and daughter were also very worried
about him. Recognizing that the situation was becoming tense, I knew I needed to take the lead and
provide empathy and support to Michael and his family.
I introduced myself to Michael and explained that he had experienced a heart attack but was in the
right place with a dedicated team working to help him. I made sure he understood that his needs were
being addressed and that I was there to help him feel better. I also took the time to communicate with
his family, explaining exactly what was happening with Michael's condition. Thankfully, his spouse was
very understanding of the seriousness of the situation and appreciated the information I provided.
c)I realized that being confident and reassuring while communicating with both the patient and his
family played a crucial role in diffusing the tense atmosphere and helping them feel more at ease
during this difficult time.
3. Commitment to Providing a Quality Service including evaluating information and judging
situations

a) Identifying a deteriorating patient and providing ongoing monitoring and high quality care
b) During my night shift in the coronary care unit, I was caring for an 82-year-old patient who had just
undergone a mitral clip procedure for severe mitral regurgitation. When I arrived, I noticed that his vital
signs were changing; his oxygen saturation and blood pressure were dropping. I sought help from my
senior staff, but I wasn't satisfied with their input, so I decided to call the on-call SHO (Senior House
Officer). However, I still felt uneasy about his decision. Knowing the situation was serious, I thought to
reach out to the registrar, but I hesitated because the vital signs were fluctuating.
Instead, I called for the ICU outreach team to assess the patient. They evaluated him and agreed that
we needed further input from the registrar. The patient was in heart failure, and after the procedure, he
had become dehydrated, which caused his lactate levels to rise. It was challenging to decide whether
to administer furosemide (a diuretic) or not. A chest X-ray was performed, which showed fluid
overload, and upon listening to his lungs, the ICU outreach found bilateral basal crackles present.
After careful discussion and analysis of the investigations , we decided to give a stat dose of
furosemide, which significantly improved the patient's condition, and his vital signs stabilized.
C) This experience strengthened my commitment to providing high-quality care and highlighted the
importance of careful monitoring, effective communication, and collaboration in ensuring patient
safety.
