MEMORANDUM

FOR: The Chief, Investigation Section

SUBJECT: Initial Investigation Report, re - Theft

DATE: May 5, 2025

I. AUTHORITY:

1. Standard Operating Procedures of this Police Unit with reference


to the incident recorded in the Patrol Section Incident Logbook,
Entry No. 0131200-31121, dated May 5, 2025.

II. DETAIL:

2. Upon receipt of the report of the Chief of the Patrol Section, the
undersigned investigator was dispatched by the Duty Desk Officer to conduct
an initial investigation regarding a Theft incident reported by Ms. Ana Santos,
45 years old, a resident of Purok Seriguelas, Barangay Pasihagon, Siquijor.

3. At the scene, the investigator observed that the front gate of the
complainant’s property appeared to have been forcibly opened. The
complainant stated that at around 07:50 AM of May 5, 2025, while she was
inside her house, an unidentified male suspect entered the yard and stole a
black Honda scooter bearing plate number NBP 4567, which was parked in
the front yard.

4. CCTV footage from a neighboring residence confirmed the presence of a


male individual committing the act. The suspect fled heading south. Efforts
to identify the suspect are ongoing, including requests for enhancement of
the video and coordination with nearby barangays for possible identification
leads.

5. Scene was documented and initial interviews were conducted with the
complainant and nearby witnesses. Photos were taken of the gate and
surrounding area.

III. RECOMMENDATION:

6. It is respectfully recommended that:

a. A follow-up investigation be conducted to identify and locate the suspect.

b. A request for forensic enhancement of the CCTV footage be forwarded to


the appropriate technical unit.

c. Coordination with barangay officials and nearby PNP units be strengthened


to track the possible route and identity of the suspect.

D. The complainant be advised to remain vigilant and report any suspicious


activity or updates.
