1) Safety & Health Committee Meeting was carried out on 26 April 2025 .
All issues raised therein have
been addressed. Review of minutes of previous meeting of 29 March 2025 carried out and there are No
pending issues from last safety and health committee meeting.
2) Master Informed all crew members about the itinerary of the vessel that she will remain at anchorage
during coming months as there is no itenary from the charteres. Recent crew change will Occur on 28
APRIL 2025 , followed by same day Provisions and Stores.
3) Master Reminded all crew members that "Health, Safety, Security, Environmental and Quality
excellence (HSSEQ) is an acronym that refers to organizations’ standards and practices to ensure the
safety and well-being of their employees, the protection of the environment, and the quality of their
products or services and advised all crew that in order to comply HSSEQ all crew members should aim to
meet companies set KPI targets e.g. zero fatal accidents, no negative media coverage, no PSC
detentions, zero oil spills into the sea and overall safe and flawless operation of vessels, in compliance
with all applicable Flag state and other regulations as per applicable ISO and ISM requirements.
4) Information regarding accidents /major NCR's/ new rules and regulations within the fleet received as
BCST discussed. Master Appritiated the hard work of all the crew members and all the departments. He
congratulated all crew members for the accident / Incident free month.
5) Master quoted that "Just because you always did it that way doesn’t make it right"advised all crew
members to follow safety culture and "STOP and THINK MORE" in day to day working and any safety
issues to be dealt immediately. Master reminded “Safety is Gainful; the accident is Painful” so all crew
members to maintain safety as utmost priorty at the work place. Due to work pressure safety should not
be compromised in any given circumstances. Crew to be self motivated at work place and always aim for
Zero accident so as reach back home safely.
6) Master reminded all crew member that “ near miss is an unplanned event that did not result in injury ,
illness or damage – but had potential to do so”advised and requested all crew members to "Alert today
and be Alive tomorrow", He said Near miss observation and reporting helps in identifying possible future
accidents.
All crew members were encouraged to report near misses, so that onboard organizations can identify
potential hazards and take proactive measures to prevent future accidents. This not only reduces the risk
of injuries and property damage but also fosters a culture of transparency and accountability within the
workplace. When near miss incidents are reported and investigated, valuable insights can be gained,
leading to continuous improvement in safety protocols and procedures.
It was reminded to all crew members that Company follows a No-Blame culture and eliminates mistakes
through training, guidance and continual improvement of its systems. Near miss from Deck & Engine
department were discussed, with root cause & proposed counter measures.
7) Safety and Health Inspection of Galley, E/R, accommodation and Deck were carried out monthly using
the Company form CHEK-009. All LSA and FFA equipment's are in continuous readiness. Work place
was noted treated with non-slip material, clear of any obstruction, free of any tripping and stumbling
Hazards. All ship side hand rails and safety chains found intact. Galley staff carried out the daily check for
provision food and maintaining good housekeeping, under senior officers' supervision. All the area were
well lit, free of garbage. Healthy and hygienic conditions were maintained in accommodation and galley.
All the safety signs and posters were intact and in place.

9)Discussed 2025-46-BCST-MMN ENSMS-OTH-25_019 MRN-admin-25003 Message from the
President & CEO on MOL Foundation Day (1st April 2025). Explained to all crew members that 2025 has
marked the 141st anniversary of MOL [Link] congratulated on successful FY2024 and looked
ahead as the stage was set for long-term success and the achievement of our Group Vision . All were
briefed about MOL Group Management Plan “BLUE ACTION 2035”.
10) Discussed 2025-47-BCST-MEN ENSMS-OTH-25_016 MRE-SA-25003 Sharing Biofuel Information,
all were highlighted that Biofuels are emerging as a crucial alternative to conventional fossil fuels in
maritime transport, offering a more sustainable energy source for the shipping industry.
Biofuels significantly reduce greenhouse gas emissions compared to conventional fossil fuels. This helps
the maritime industry meet global sustainability targets and reduce its carbon footprint.
11) Discussed 2025-49-BCST-NMN ENSMS-MRN-25_011 MRN-SA-25009 Contact with berthed
vessel while unberthing, all were briefed about the lessons learnt from the accident , Share maneuvering
plan sufficiently among the Master, OOW and pilot including tug assistance, use of anchor, swinging
point, and etc. before maneuvering. Fully understand the effects of external forces such as wind, currents,
and pay necessary attention to their changes. If the pilot's intention is questionable and deemed unsafe,
the master should override the maneuvering and ensure the vessel's safety.
12) Discussed 2025-50-BCST-SMN ENSMS-OTH-25_02 MRN-SA-25010 CAUTION! - Lifeboat
accidents are frequently occurring , all were briefed about various lifeboat incidents , reminded about
various points to be considered before drill/ maintenance with regard to lifeboats and davits in good
condition, appropriate PPEs for the planned work / Drill, understanding of structure of lifeboats and davits
and communication between crew members, especially between supervisors and
operators, well maintained.

16) It was reminded to all crew members that CBT training is important tool to enhance their on job skills.
All crew members were advised to strictly complete their CBT training timely as per Rank training matrix.
Crew memberswere advised that they should not continue CBT training for more than 45 minutes , after
that they should take a break so as to maintain the concentration of learning.
17) All crew complied with work and rest hours as per MLC. Crew were reminded that ISF records to be
updated on daily basis and any NC's to be reported immediately to respective head of departments.
Master instructed to fill ISF accurately as per actual working hours. Dept head (C/O & 1AE), to make work
planning prior arr. port and/or for any other important/busy work schedule to avoid any NC's and to
ensure crew can get proper rest.
Seagull CBT training to be conducted during working hours of crew members, hence to be also included
in ISF records as and when CBT training is undertaken.
18) All staff were encouraged to maintain a healthy lifestyle by exercising and having nutritional meals. All
Smokers were advised to smoke within the designated smoking corners provided in the smoking rooms,
ensuring that the air filter switched on and working. Also all crew members to eat meals during meal
timing and every crew member must wash their own dishes for late night snacks.
19) Smoking regulation discussed, master reminded all smokers to comply with safe smoking practices,
Do not smoke in cabins, use of safety matches, No lighters allowed, Smoking in designated areas only
and keeping the smoking rooms doors closed at all times.
20) Master quoted “ Choose health not hangover , he further said “Getting drunk makes your insides
junk” and reminded that all crew members to strict adhere to companies' drug and alcohol policy. He
stressed that defaulters will not be spared under any curtumstance and will be punished as per
companies law enforcement.
21) Welfare/MLC-All crew members were satisfied with Welfare and recreational arrangements on board.
Work and rest hours were complied with as per MLC convention. All crew were satisfied with the quality of
food provided to them. Health and hygiene standards were maintained as per company standards. No
any concerns were received regarding the quality of food.
22) PPE - All PPE reported to be in good condition. All crew advised to comply with PPE matrix. No work
on board to be carried out without compliance of PPE matrix. Personal PPE (Immersion Suits, Lifejacket
etc.) shall be kept clean and be maintained ready for use anytime.

1.) All crew members to have a right to a safe and secure workplace, where safety standards are complied
with, where you have fair terms of employment, decent living and working conditions, including social
protection such as access to medical care, health protection and welfare.
2.) Crew members are entitled to a fair employment agreement or contract setting out the terms and conditions
of employment. It must be signed by crew member and employer, be easy to understand and legally
enforceable. The shipowner is responsible for ensuring that your rights under the employment agreement or
contract are respected even if you are also working for another company on the ship.
3.) Shipowner has liability for contractual payments for death or long-term disability due to an occupational
injury, illness or hazard to be set out in the employment agreement or collective bargaining agreement.
4.) Shipowners must have a form of financial security, which may be insurance, directly accessible to crews,
with sufficient funds to provide timely financial assistance in the event of abandonment. It means no more
seafarers left for months without food, wages and unable to get home.
5.) Employer must ensure that crew member is able to send all or part of your earnings home. This could be by
regular bank transfer of remittances agreed when crew member sign’s the contract. Remittances should be
sent on time directly to the person or account nominated by crew member. Crew member shouldn’t be
expected to pay unreasonable charges for such services. The exchange rate must be at the current market
rate or at an official published rate.
6.) Ships must carry sufficient quantities of good quality food and drinking water and supply it free of charge
during period of engagement. Food provided has to be nutritious and varied and prepared and served in
hygienic conditions. Religious and cultural differences also have to be considered.
7.) Shipowners are responsible for any costs resulting from sickness, injury or death connected to crew
member employment, from the date you start a contract until you have been repatriated, or you can claim
medical benefits under an insurance /compensation scheme. This could be a State, or private
sickness/accident insurance, or a workers’ compensation scheme.
8.) Employment agreement should clearly set out any aspects of social security protection provided by the
shipowner, any statutory deductions that are to come out of your wages (to be paid into a State scheme) and
any contributions that the shipowner has to pay on your behalf.
1. Master explained the scenario that While bosun was trying to tighten the bolts of the flange, he was exposed to
fumes of crude oil and was intoxicated and became unconscious momentarily and lost balance and fall down.
While falling he hit his helmet on of the stringer, and due to chin strap being elastic type which resulted in helmet
falling away, and Bosun hit his head on a metal structure.
2. Bosun was immediately transferred to Hospital using Neil-Robertson stretcher. His Blood pressure,
heart rate, and temperature were checked and found normal.
The wound on Bosun head were cleaned and bandaged. Ice was applied to the wound to prevent
swelling. He was feeling fine. On the following day and Bosun has disembarked to complete further checks ashore.
3. Master explained that the cause of Incident is he might be intoxicated by cargo vapor and could not keep control
and Lost balance and fall down. Also, the chin strap of the Bosun Helmet was elastic type, securing with some
plastic over the chin- which was slipped when Bosun fell down & exposed head for injury.
4. Master reminded all crew members that While working, please always pay attention to your surroundings,
potential hazards and have a situational Awareness.
5. Master advised all crew members to follow safety culture in day to day working and any safety issues to be dealt
immediately. He stressed upon dressing appropriate PPE as per PPE matrix for all the job order carried onboard.
