This questionnaire is constructed to gather valuable insights into the operations,


efficiency, and challenges faced by the organization. To be specific, the goal of this Industrial
Engineering capstone project is to identify areas for improvement in the organization to
maximize efficiency.

Your responses will help us to analyze existing workflows, pinpoint operational bottlenecks, and
develop data-driven recommendations to enhance productivity and overall performance. All
information provided will be treated with utmost confidentiality and used solely for academic
purposes. With respect to this, we could indicate your company as anonymous if you prefer,
ensuring that the collected data remains confidential. If required, we are willing to sign a Non-
Disclosure Agreement (NDA) to ensure compliance with your confidentiality policies. Additionally, we
are open to discussing any conditions or restrictions you may have regarding data usage.

I. Production and Process Efficiency

1. What is the current production capacity per shift/day, and how frequently is this capacity met
or exceeded?

2. How do you forecast production targets, and what factors (e.g., raw material quality, labor
availability) most affect your throughput?

4. What specific stages in the crushing process are most prone to delays or interruptions?

6. What is the average loading time per truck, and what factors contribute to longer loading

area?

8. Are there designated lanes or areas for loading different types of aggregates (e.g., crushed
stone, sand, gravel)?

9. Have there been instances of misloaded or incorrect deliveries? If so, what measures are in
