PEDIA- Instruments

1) AMBU- Artificial Manual Breathing Unit

250 neonate/ 500 pedia/ 2000 adults

Self inflated/ Non-rebreathing

2 valves- pop-off (LEAK Test)🡪 To prevent high pressure formation, which is mostly not necessary
(>40mmh req in asthma, pulm edema, partial obs)/ Fishmouth (sucker) valve🡪 Prevents back
flow of air

2) NG Tube/ Ryles-

PVC🡪 Change after 15 days/ Polyurethane🡪 6 wks

Blunt tip with holes surrounding it, so that even if tip gets blocked.

With markings

Newborn is 5-7fr, children 8-12 fr, adults 18fr (1f r= 1/3rd mm diameter)

Measured from TIP OF NOSE🡪 Tragus🡪 Umbilicus

After inserting, inject air, hear with steth over abdomen (WHOOSH TEST)/ pH of contents/ put
end in water, check for air bubbles/ Xray

Indications- Therapeutic and Diagnostic

- Gastric lavage
- Gastric aspirate test for neonatal sepsis
- Poisoning
- TEF
- Enteral feeding and administration of drugs

3) ICD Tube

Bag should be at a lower level than chest (Principles: Gravity, Pos pressure, Suction)
Identify 4th ICS and using Kelly clamp after incision, insert and drive it in for 2-3cm, towards apex.

4) Insulin Syrimge

1ml (100 units of insulin) and 2ml sizes

Needle is covered with silicone to pass easily through the skin

🡪0.25ml best for children

Uses: Inject insulin/ Mantoux / BCG/ Allergy testing/ Vit K 1ml/ Collect ABG analysis

5) Venflon

More the gauge, lesser the diameter (outer) and lesser the flow rate

Wide Bore Cannulas- Orange= 14G (max. flow rate)

Grey- 16G

White- 17G

Green- 18G

Pink- 20G (length decreases)

Blue- 22G

Yellow- 24G (PEDIA)

Purple- 26G (Pedia)


6) Bone Marrow

Parts- Trocar/ Stillete / Cannula/ Adjustable Guard

Dry Tap- Myelofibrosis, Aplastic Anemia (do fine aspiration instead), Hairy Cell Leukemia

Complication- Vasovagal Attack

7) ET Tube

Adapter connects to the AMBU bag or ventilator

Black line- at the level of the vocal cord so that the tip is at the bifurcation of trachea

Radioopaque marker- for visualisation in Xray

Cuff- Positive ventilation and preventing leak 🡪 (used in age >8yrs)/ Use syringe to inflate the cuff

[Less than 8yrs subglottic area is narrow acts like a cuff]
