The Company is not wound up or subject to any existing winding up Petition.
The business of the company is still an ongoing concern.
There is no pending insolvency proceedings against any of its sole proprietors/partners/directors;
The company has the ability to generate income/Salary/Annual Income i.e. B/BE or e-Filling/Rental Income.
The Company’s loan/financing is presently not under rehabilitation with Credit Counselling and Debt Management
Agency (AKPK);
I am duly authorized to make this application and to submit this form on behalf of the Company and ;

ii. Our guarantor is Credit Guarantee Corporation Malaysia Berhad (CGC) / Syarikat Jaminan Pembiayaan Perniagaan Berhad

I/We understand that any approved repayment arrangement may be disclosed to Bank Negara Malaysia or such other authority
or body established by BNM or such other authority having jurisdiction over the Bank and that this repayment arrangement status
may reflected in the relevant reports.

I/We understand that the approval of this application and terms attached to the approval is subject to the Bank’s sole and absolute
discretion. I/We also understand that the deferment will take only effect upon the Bank’s approval and /or notification of the approval.
