Re: Jake Peterson, 18 years old

Dear Nurse,

I am referring to your care Mr. Peterson, who has complaints of alopecia and possible depression. He has
been a student at our boarding school for the past 8 years and is in need of continued care and
management as he will be moving in your university.

Mr. Peterson has been concerned with hair loss for 5 years now for which he was prescribed steroid
creams. He has also complained of having depression. It is also worth noting that his parents are living
and working abroad with the history of his father having alopecia in his 50s and his mother having
depression in her 40s.

On March 2014, Mr. Peterson arrived at the clinic complaining of hair loss in patches and had a school
bullying incident that may be the reason for his anxiety and depression. After 4 years, his anxiety
returned and pronounced he is having hair loss. Mr. Peterson declined seeing the doctor and opted for
homeopathy but has shown no results for 2 months and this increased his distress and dampens his
mood.

It would be greatly appreciated if you could organize special exam arrangements for Mr. Peterson as he
has extreme anxiety during exam time and needs a separate exam room. Kindly also monitor his mental
health as he has shown to have increasing anxiety for the past 8 months.
