2. METHODOLOGICAL PART
2.1 Aim of the work and research problems
The aim of this study was to show selected needs and nursing problems observed in
the patient diagnosed with multiple sclerosis. The main problem of this study was to answer
the question: What nursing problems are there in caring for a patient with MS? However, in
order to solve the main research problem, the following specific research problems were also
established:
● What is the patient's bio-psycho-social status?
● Does the patient have sexual problems related to the disease?
● What is the patient's self-sufficiency?
● What is the patient's and her immediate family's understanding of the disease?
● Which roles should a nurse play in educating the patient and the family about the disease?
● What are the needs of the patient and quality of life required?
2.2 Research methods, techniques, and tools
The study used the case study method. The following research techniques were applied:
analysis of medical records, interview, measurement, observation. Analysis of medical
records is a qualitative examination and assessment of the patient’s information which is in
either printed or electronic format. The interpretation of data follows reviewing of the records
for understanding and the development of comprehensive knowledge of the patient and their
condition [4]. The interview is the most frequently used tool in the gathering of the patient’s
information, regarding the most necessary information on their condition. During the course
of the interview, the nurse can interpret the patient’s posture and any other nonverbal signs
and symptoms relating to the patient’s condition [3]. Measurement refers to the use of detailed
methods, scales, tables, and instruments in order to ascertain the characteristics of certain
entities through assigning to them a specific numerical value. The value of the quantity is
determined by measurement, as are the distributions of quantities in spatiotemporal regard;
the relations between quantities, their distributions and breadth of the relations are also
weighted using measurement [6]. Observation is carried out under two modes, structured and
unstructured. In structured observation, data collection is conducted using specific variables
and according to a predetermined schedule, hence it is also called systematic observation.
Unstructured observation is conducted without predetermined variables and objectives.
Medical observation is systematic [2]. SF-36 Questionnaire SF-36 is a set of generic,
coherent, and easily administered quality-of-life measures. These measures rely upon patient
self-reporting and are now widely utilized by managed care organizations for routine
monitoring and assessment of care outcomes in adult patients. According to research, this
questionnaire consists of 36 items which are grouped into 8 categories which include
physically working, physical and emotional constraints, socially working, pain in the body,
general and psychological health. This scale can be used in all age groups. The Kurtzke
Functional Systems Scores (FSS) The Kurtzke Functional Systems Scores is one of the most
widely utilized assessment instruments in MS to gauge disease progression. The FSS
measures the level of functioning in the central nervous system and attributes scores to
disability. The respondent’s gait and use of assistive devices is assessed, in addition to the
level of functionality of trouble moving limbs, loss of coordination, speech difficulties, loss of
sensation, bowel/bladder function, visual function and mental function. The FSS has an
ordinal clinical rating scale, numbering from 0 to 5 or 6. It has been used in the majority of
clinical trials in MS for the past forty years. The FSS is an ordinal clinical rating scale that is
rated on the basis of the judgment of the examiner. The FSS is a single-item scale and there is
no composite or summed score. The FSS includes pyramidal, cerebellar, brainstem, sensory,
bowel and bladder, visual, cerebral (or mental), and other. In the FSS, the higher the score the
greater the severity of impairment.
2.3 Description of the area
The researcher presents a study held on patient X who was selected through
convenience sampling. It was conducted at 1 Kliniczny Szpital Wojskowy. The patient was
diagnosed with Diabetes Type 2 after being delivered to the hospital by an ambulance.
Research tools for this study consists of data that has been collected from the hospital and
given by the patient. Consent was given by the patient and the patient's family prior to the
beginning of the case study. The case study was conducted from the 18th of October after the
patient was admitted into hospital.
