1)An adult asthmatic female patient aged 38 years, weight 52kg developed severe breathlessness at night (2

a.m) and brought to hospital emergency. She was given Aminophylline 250 mg rapid i.v. injection as it was
emergency but patient breathlessness was not relieved. Instead he developed nausea, headache, tachycardia,
palpitations and tremors of fingers. What was the likely cause of this problem and how it could have been
prevented? Report the ADR by filling the ADR reporting form.(RN 1-50)

2)Mrs. Pushpa, a 72 year old female, wt 52kg was diagnosed with bronchial asthma and presented to OPD
on 12/5/2022. She was treated with metered dose inhaler Butamate-200 (Budesonide 200ug + Formoterol
12ug) inhalation twice daily and tablet Telecast-L(Monteleukat 10mg + Levocetrizine 5mg). She again
presented in OPD with oral thrush on 18/7/2022. On taking history it was known that patient never rinsed
her mouth after taking the puff. What will be your approach here? Report the ADR by filling the ADR
reporting form.(RN 51-100)

3)Mr. Awasthy, a 40 year old male, wt. 56kg presented with fever on 15/1/2019. He was prescribed tablet
Paracetamol 500mg SOS and Tablet Cotrimoxazole (Sulfamethoxazole800+Trimethoprim 160mg) BD
for 5 days. On 16/1/2019 he complained of itching and red rashes all over the body after taking tablet
Cotrimoxazole. What will be your approach here? Report the ADR by filling the ADR reporting form.(RN

4) Mr Ram Mohan a 48 year male patient diagnosed with moderate hypertension . He was prescribed Tab
prazosin 1 mg BD and Tab Hydrochlorothiazide 25 md BD on 1 Jan 2020 for one month. After 2 days of
treatment, the patient felt giddy and fell down and sustained superficial scalp injury. The dose of Prazosin
was reduced to 0.5 mg after checking the BP standing (Standing BP 100/70mmHg). Fill in the Adverse
Drug Reaction monitoring form based on the hospital record of a patient describing an adverse event. Also
perform the causality assessment.(RN 151 -last RN )
