ERYTHROCYTE SEDIMENTATION RATE (ESR) is a test that indirectly measures the degree of inflammation present in the body. ESR

Increase in: infections, Vasculities, Inflammatory arthritis, Renal disease, Anaemia, Malignancies, and plasma cell dyscrasias, Acute
allergy Tissue injury.
Finding a very accelerated ESR ( >100 mm/hour) in patients with ill-defined symptoms directs the physician to search for a
systemic disease (Paraproteinemias, Disseminated malignancies, Connective tissue disease, severe infection such as bacterial
endocarditis).
Decreased in: Polycythaemia vera, Sickle cell anaemia.

1) Vitamin B12 is a coenzyme involved in two very important metabolic functions vital to normal cell growth and DNA synthesis.
2) Deficiencies of Vitamin B12 occur due to a defect in the secretion of intrinsic factor, resulting in inadequate absorption of vitamin
B12 from foods.
3) Vit B12 levels are decreased in megaloblastic anaemia, partial or total gastrostomy, pernicious anaemia, peripheral neuropathies,
chronic alcoholism, senile dementia, and treating epilepsy.

• Interpretation:
[Link] antigen (CEA) is an oncofetal glycoprotein that is normally expressed by mucosal cells. It is overexpressed by a
variety of malignancies. Although it is most commonly associated with colorectal cancer.
[Link] can also be elevated in other malignancies such as breast, liver, stomach, and pancreas.
[Link] are a number of benign conditions that may lead to elevations in serum CEA level including cigarette smoking,
pancreatitis, biliary obstruction, peptic ulcer disease, and hypothyroidism, but the extent of elevation is substantially less, and it is
rare to see an elevation of >10 ng/ml in this context.
[Link] is not an effective marker for hidden (occult) malignancy, since early tumors do not cause significant blood elevation.
5.A single test is difficult to evaluate but the number of tests done weeks apart show the trend in disease progression or regression.
