Diabetic Foot and Hypertension Prevalence Study

This study assessed the prevalence of diabetic foot, hypertension, and peripheral neuropathy among adult diabetic patients at Dodoma Regional Referral Hospital, revealing a diabetic foot prevalence of 22.3%, hypertension at 58.8%, and peripheral neuropathy at 13.1%. Significant associations were found between diabetic foot and factors like type of diabetes and uncontrolled blood glucose, while hypertension was associated with age, type of diabetes, education level, and medication regime. The findings highlight the need for better blood sugar control and regular screenings to reduce diabetes-related complications.

ABSTRACT
Background: Diabetes mellitus is a disease of great concern because poorly controlled
hyperglycemia leads to development of diabetic complications which increase the number of
hospitalizations, increase patient management costs, morbidity and mortality among patients with
diabetes. A study which was done in Dodoma region showed that the prevalence of diabetes was
11.8%; this shows how alarming diabetes mellitus is in our community. Despite the fact that there
were studies which have been done on diabetes in Tanzania, there was no recent study done to
determine the prevalence of chronic diabetes complications.

Purpose: The purpose of this study was to assess the prevalence of diabetic foot, hypertension,
peripheral neuropathy and their associated factors among adult diabetic patients attending diabetes
clinic at Dodoma Regional Referral Hospital.

Methodology: A prospective cross-sectional study was conducted among 160 patients attending
diabetic clinic from October to November 2022. A questionnaire was used to collect information
from consented patients. SPSS version 20 was used to analyze the data and the p-value < 0.05 was
considered as statistically significant.

Results: The prevalence of diabetic foot was 22.3%, hypertension 58.8% and 72.3% of
hypertensive diabetic patients had uncontrolled BP(BP>140/90 mmHg) and prevalence of
peripheral neuropathy was 13.1%. The factors which have showed to have statistically significant
associations with diabetic foot were type of diabetes(p=0.02) in which type 2 has more patients,
duration of diabetes(p=0.01), uncontrolled blood glucose(p=0.01), and peripheral
neuropathy(p=0.00). The factors which showed statistically significant association with
hypertension were age of the patient (p=0.00) age >60 years showed higher proportion, type of
diabetes(p=0.00) type 2 has higher proportion, level of education(p=0.00), and medication
regime(p=0.00) the patient was using. Peripheral neuropathy didn’t have significant statistical
association with any factors.

Conclusion: The study has shown high prevalence of chronic diabetes complications which are
diabetic foot, hypertension and peripheral neuropathy. Controlling blood sugar, early diagnosis of
type 2 diabetes, and regular screening of peripheral neuropathy will help to reduce the
complications of diabetes which are leading cause of morbidity and mortality among patients with
diabetes.
