Dear Sir/Madam,

I, ANGIE F. CARO, hereby authorize GERILYN MAE C. HIPONIA to claim my last pay cheque
from PHINMA UNIVERSITY OF ILOILO on my behalf. I am unable to claim it personally due to my
current work here in Roxas, City.

Please find attached a copy of my ID and the necessary documents to claim the cheque. If you

RELEASE WAIVER AND QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

That I, CARO, ANGIE., Filipino, of legal age, and formerly employed with UNIVERSITY OF ILOILO, INC. (hereinafter, the
University) do by these presents acknowledge receipt of the sum of Php 6,811.66 Philippine Currency, from the
University, in full payment and final settlement of any and all my claims relating to adjustment for differential pay due to
me or which may be due to me from the University and all other claims which I may have arising from, related to, or
connected with my employment with University of Iloilo including 13th month pay, unpaid premiums, unremitted
deductions, tax refund, separation pay and the like (hereinafter referred to as “claims”).

In consideration of said payment, I do hereby quitclaim, discharge and waive any and all actions of whatever nature,
expected real or apparent, which I may have against the University, its directors, officers, employees, agents and clients
relating in my adjustment for my differential pay and Claims. I will institute no action, whether civil, criminal, labor or
administrative against the University, its directors, officers, employees, agents, and clients relating to my differential pay
and Claims. Any and all actions which I may commence either solely in my name or jointly with others before any office,
board, bureau, court or tribunal against the University is hereby deemed and considered voluntarily withdrawn by me
and I will no longer testify or continue to prosecute said action(s).

I declare that I have read this document and have fully understood its contents. I further declare that I voluntarily and
willingly executed this Release, Waiver and Quitclaim with full knowledge of my rights under the law.
