Research Proposal: Development of AI-powered Triage App in
Collaboration between IIT and AIIMS Guwahati

Background :
There are many accidents and disaster that happen all the time. Many people
lose their lives because they do not get proper medical treatment at time of
accident.
Accidents and disasters encompass a wide range of events that can have
significant impacts on communities, infrastructure, and the environment.
In mass casualty incidents (MCIs), the burden on doctors and other healthcare
professionals can be overwhelming, leading to significant challenges in
effectively managing and triaging patients.

The role of medical professionals and services in disasters and accidents is


crucial in several key aspects:

1. Emergency Response: Medical personnel, including doctors, nurses,


paramedics, and emergency medical technicians (EMTs), are at the
forefront of providing immediate medical assistance to victims. They
stabilize injuries, administer first aid, and prioritize treatment based on
the severity of injuries.
2. Triage: During mass casualty incidents, triage becomes essential.
Medical teams assess and categorize patients based on the urgency of
their medical needs. This helps in efficiently allocating resources and
providing timely care to those who need it most.

3. Medical Care and Treatment: Hospitals and medical facilities play a
vital role in receiving and treating patients following accidents or
disasters. They manage a wide range of injuries, from minor to critical,
and coordinate specialized care as needed, such as surgery, intensive care,
and rehabilitation.

Role of AI in Triage

1. Speed and Efficiency: AI can assist in rapidly assessing and categorizing


patients based on symptoms and severity, reducing the triage time.
2. Data Analysis: AI can analyze large volumes of data quickly, providing
insights that can help in decision-making and resource allocation.
3. Support for Doctors: AI tools can support doctors by handling routine
assessments, allowing them to focus on more complex cases.
4. Consistency: AI can ensure consistency in triage decisions, reducing the
variability that can occur with human judgment under stress

Importance of Efficient Triage

1. Reducing Mortality: Timely and accurate triage can significantly reduce


mortality rates by ensuring that critically injured patients receive
immediate care.
2. Optimizing Resource Use: Efficient triage helps in optimizing the use of
available resources, ensuring that they are directed where they are most
needed.
3. Managing Chaos: In chaotic situations, a systematic triage process can
bring order and facilitate better coordination among healthcare
providers.
4. Preparation and Training: Regular training and drills for healthcare
professionals on triage protocols can enhance their readiness and
effectiveness during actual MCIs.

Impact of AI based Triage

1. Time Sensitivity: Effective triage is crucial in MCIs to ensure that


patients receive timely care according to the severity of their injuries.
Delays in triage can lead to increased mortality and morbidity.
2. Accuracy: Accurate triage is essential to prioritize treatment for those
most in need and to make the best use of available resources. Mistakes
in triage can result in critical patients not receiving timely intervention.
3. Systematic Approach: Standardized triage systems (e.g., START - Simple
Triage and Rapid Treatment) help in quickly categorizing patients based
on the urgency of their conditions.

Mumbai: One in 10 patients in India dies on the way to hospital, simply
because motorists are clueless about what to do when they see an ambulance
on the road, according to the Radhee Disaster and Education Foundation.

"When I met traffic officials, they told me motorists want to help but cannot
due to lack of awareness,” said Dr. Rita Savla, founder director of the
foundation.

TRIAGE

Medical triage is a critical process used in emergency and disaster situations to


prioritize patient care based on the severity of their condition and the available
resources.

Triage Process: The triage process involves systematically evaluating each
patient, assigning a triage category, and marking them for appropriate care:

• Triage tags or color-coded identifiers are often used to clearly mark each
patient's category.
• Continuous reassessment is crucial to ensure that patients receive timely
treatment as their conditions may change.

Primary Assessment: Medical triage begins with a rapid assessment of each


patient's condition to determine the severity of injuries or illnesses. This initial
evaluation focuses on identifying life-threatening conditions that require
immediate intervention.

Triage Categories: Patients are categorized into different triage levels based on
the severity of their condition:

• Immediate (Red): These patients have life-threatening injuries or


conditions that require immediate medical attention to survive.
• Delayed (Yellow): Patients with serious injuries that are not immediately
life-threatening and can wait for treatment.
• Minor (Green): Patients with minor injuries or illnesses that do not
require urgent care and can typically wait the longest for treatment.
• Expectant (Black): Patients who are so severely injured or ill that they
are not expected to survive even with medical intervention. This category
is particularly challenging emotionally and ethically for triage personnel.

Project Overview:
The proposed project aims to develop a mobile application that
utilizes advanced AI algorithms to assist healthcare providers in the
triage and initial management of trauma patients. This application will
leverage AIIMS's expertise in clinical practice and medical knowledge,
alongside IIT's proficiency in AI research and application development, to
create a robust tool for real-time decision making for INDIAN settings.

Integration of Machine Learning Algorithms: Employ cutting-edge machine
learning techniques, including natural language processing (NLP), deep
learning, and predictive analytics, to enhance the diagnostic accuracy and
efficiency of the triage app.

Creation of a user-friendly and Adaptable Platform: Develop a application


platform that is easy to use and can be customized and integrated into diverse
healthcare settings, accommodating variations in patient demographics, medical
conditions, and healthcare infrastructures.

Methodology :
Data Collection and Analysis

Collect and analyse extensive datasets of anonymized patient records,


encompassing diverse medical conditions, symptoms, diagnostic tests, treatment
outcomes, and demographic information. These datasets will serve as the
foundational training data for developing and validating AI algorithms within
the triage app.

Algorithm Development

Utilize state-of-the-art AI and machine learning methodologies to develop


robust algorithms capable of:

Clinical Decision Support: Integrate deep learning models to predict disease


severity, recommend appropriate diagnostic tests, and triage patients based on
clinical urgency, leveraging historical patient data and real-time inputs.

This app will be provided to nearest medical centre, police force, traffic police ,
army personals, railway police force, industrial workers, ambulance drivers and
general citizens. This will help medical centre to be prepared and help the
people at accident site during transportation and in hospitals more efficiently

Prototype Development : Build and iteratively refine a functional prototype of


the AI-based triage app, incorporating developed algorithms, user interface (UI)
design principles, and backend infrastructure.

It will act as platform between medical centre and accident site, to guide and
make standard/professional level triage decision without Bias and Error.

Objective of Collaboration :
The collaboration between the IIT Guwahati and the AIIMS Guwahati for
developing an AI-based triage app aims to leverage the complementary
strengths of both institutions to achieve the following specific objectives:

Enhance Clinical Decision-Making:

Develop an AI-powered triage application that aids healthcare professionals in


making more accurate and timely clinical decisions. By integrating AI, we aim
to minimize human errors, reduce subjectivity, and ensure consistent and
evidence-based triage assessments.

Optimize Resource Allocation:

Develop a triage system that improves the efficiency of resource utilization in


healthcare settings, including emergency departments and outpatient clinics.
Efficient triage helps in managing hospital resources such as staff, equipment,
and beds more effectively, especially in high-demand scenarios like pandemics
or mass casualty incidents.

Advance AI Research in Healthcare:

Contribute to the field of AI by developing innovative algorithms and models


specifically tailored for healthcare applications. This collaboration provides an
opportunity to advance academic research in AI, generating new knowledge and
methodologies that can be applied across various medical domains.

Foster Interdisciplinary Collaboration:

Promote a synergistic partnership between AI researchers at IIT Guwahati and


clinical experts at AIIMS Guwahati , fostering interdisciplinary research and
development. Combining technological insights with clinical expertise ensures
the developed solution is both technologically robust and clinically relevant,
addressing real-world healthcare challenges.

Outcome of this project :

Improved Patient Outcomes

A reduction in morbidity and mortality rates due to timely and appropriate


triage and treatment prioritization. Patients will benefit from quicker access to
necessary medical interventions, particularly in emergency situations, resulting
in better overall health outcomes.

Optimized Resource Allocation

More efficient use of healthcare resources, including staff, equipment, and


hospital beds. Hospitals and clinics will be able to manage their resources more
effectively, reducing wait times and ensuring that critical resources are allocated
where they are most needed.

Advancements in AI Research

Significant contributions to the field of AI, particularly in the development of


algorithms and models tailored for healthcare applications. The project will
generate new knowledge and methodologies that can be applied across various
medical domains, driving further innovation in AI and healthcare.

Successful Interdisciplinary Collaboration

A successful partnership between AI researchers and clinical experts, fostering


interdisciplinary research and development. This collaboration will serve as a
model for future interdisciplinary projects, demonstrating the benefits of
combining technological and clinical expertise.

Resource Requirements:

We anticipate the need for access to clinical data, computing resources, and
collaborative space for meetings and discussions. Additionally, funding support
for research assistants, software development, and project management would
be beneficial for the successful execution of this project.
We would like to make a team of 5-6 members from Computer science, Graphic
design and data science background from IIT GUWAHATI.
We also require financial support from IIT and AIIMS for research and
development.

Conclusion:
The collaboration between IIT and AIIMS in developing an AI-based triage
app represents a significant advancement in healthcare technology. By
combining expertise in AI research and clinical practice, this project aims to set
a new standard for triage efficiency, ultimately improving patient care and
healthcare delivery.

This proposal outlines a comprehensive roadmap for the development and


implementation of an innovative AI-driven triage solution, leveraging the
strengths of both academic and clinical institutions to address critical healthcare
challenges effectively.
