Science LOR Questionnaire

(NOTE: TO BE FILLED FROM THE RECOMMENDER’S PERSPECTIVE, NOT STUDENT’S)

This LOR should reflect:

 Student’s academic capabilities from the point of view of your teacher


 Student’s strengths, weaknesses, academic interest areas, and future goals
 Student’s uniqueness, problem solver, proactive, enthusiastic
 Student’s emphatic, passionate, reliable and self disciplined skills
 That the student is an articulate and confident speaker

If there is more than one incident, or project related to any of the questions, please feel free
to mention them all. Keep in mind that every activity, fact or quality has to be justified with
an instance of learning from student’s life, or classroom endeavors.

The LOR can has to 1 page long at the least and can include any of the following points:

Start with mentioning in what capacity is the recommender writing and for how long did they
know the student? (Mandatory)
Mention the name and exact designation of the recommender, their association with student.
Regarding the duration, please mention the month and year of the beginning and end of
association; for continued associations mention the number of years and the present nature of
engagement.

1. Specify which topics the student is strong in within this particular subject. Explain their
process of learning. What kind of guidance did the recommender give to the student?
Please refer to the syllabus of the given subject and pick out topics, areas of interest, and
chapters which mark your student’s passion for the subject. Mention any independent online
researches that student has done in the same, or any reference book that they have studied on
their own. Also, mention how many hours and what methods to tackle learning deficiency are
used, and how recommender has helped student with notes, extra classes, doubt-clearing, and
motivational instances. Write the details with justification.

2. In which topics has student scored extremely well?


Mention the topics and specific test scores that attest to this fact of scoring well. Additionally,
mention the class and the name of the particular examination where student scored high marks;
Adding percentile would be great, for instance, the student came in the top 5% of the class.

3. Describe any class participation, for example, any presentation or project for which
student has received positive comments from the recommender or fellow students.
Describe the questions student asks in class, their inputs in the class discussions, and their
assignment submissions on time for which they may have received praise, in full detail . Also,
describe the class presentations, viva, or any other group activities and field trips taken for
academic enrichment of the subject, mention student’s contribution in the same. Mention all
the summer assignments and projects in full detail of topic.
4. Were any of your student’s projects, assignments or research work (individual or group)
particularly challenging yet they achieved top rank in them? Explain any poor performance
on quizzes or tests. What difficulties did student face and how did they work around them
to improve their score?
Mention the chapter and topics that student finds difficult specifically from your subject books,
and narrate how he/she overcame the difficulty. Mention all assistance sought by student, and
efforts recommender has taken to clear doubts. Also, add any test results that were poor, with
justifiable reason and your growth after it. Elaborate on the role of the teacher to help student
in the same.

5. Elaborate on tests or any other specific work that student is particularly proud of, with
specific names or examples of the work.
Mention excellent scores with specific test’s name and year or month. Mention any
distinguished project, research paper, internship, or field work that he/she is proud of if it is
related to the subject of LOR. Please describe the student’s process of achieving this distinction
alongside the references and study sources that he/she used. Mention how the recommender
helped student in this regard too.

6. Describe any challenges that the student had to overcome in and outside the classroom,
any major achievement that makes him/her stand out from the rest of the crowd. How
would you say they’ve grown academically?
This could include anything that is directly related to the institution/school, where the student
took initiative. It could also include student winning exceptional completions, helping solve a
fight, suggesting some major change in the school like formulating a school union, or winning a
national Olympiad. Feel free to elaborate on any family issues, time management issues, or
personal learning difficulties.

7. Rate your student’s performance in practical tests, lab work, etc. Comment on their
curiosity to learn, initiatives taken and overall intellectual prowess. How have they availed
of the opportunities to enhance their learning or skills?
If there is any lab work describe their endeavors and learnings from the same and how it is
different from the theoretical knowledge structure. Elaborate on any successful experiments or
group activity of school level, zonal level, or national level.)

8. Indicate an instance when student was a strong collaborator in a group project in this
class. What role do he/she play in the context of the larger school community? How have
they functioned as a leader in this class? Give evidence of overall intellectual vitality.
Here, you can mention your student’s activities and role in the relevant school clubs and student
organizations. Moreover, if they have taken any classes for their friends and helped them
understand the subject, mention that too.

9. What is the student’s relation with his/her teachers and peers? Cite an instance when
student was an effective communicator in this class.
Here, explore student’s relationship with the teacher and peers from the point of view of the
teacher. Mention his/her demeanor, manners, politeness, and inquisitive nature. Also, mention
how they help thier class and friends academically. Any other personal associations of academic
relevance, informal study groups, and exceptional contributions to the peer group should be
brought to notice.
10. Describe the kind of experiments that the student found challenging in the lab. How did
they overcome the difficulty?
Mention any specific experiment that challenged student’s intellect and how they sought help to
bring out results from the failed experiment; Also describe the support that recommender
extended to the student.

11. How do the student coordinate with their team in the laboratory? Any significant initiative
that they have taken to make the team more efficient with the testing results and
presentation?
Describe in detail the nature of your student’s engagement with his/her team members in the
laboratory along with relevant examples. Specify their role in the activities.

12. Conclude and sign off with the recommender's name, designation and his/her
institute/company’s name.
