Introduction
QUASAR™
The Quality Assurance System for Advanced Radiotherapy (QUASAR™) supports the testing of a
wide variety of dosimetric and nondosimetric functions of Radiation Therapy Planning Systems
and CT Simulators using a set of innovative quality assurance (QA) tools.
The QUASAR products should be used for regularly scheduled testing, as well as commissioning
new systems and upgrades, and for testing repairs.
The QUASAR system provides you with confidence that your radiation treatment planning
software and CT simulators are performing to their full potential.
This Guide
This document is a User’s Guide for the QUASAR Programmable Respiratory Motion Phantom
(model number 100-1011) and for the QUASAR Programmable Respiratory Motion Assembly
(model number 100-1012) in imaging and radiotherapy quality assurance applications. This
guide provides a description of the phantom and provides instructions for set up and operation.
Important Note
This User’s Guide is not a guide to Radiation Therapy Planning Systems, CT Simulators, linear
accelerators, CT Imagers, or PET/CT scanners. Appropriate QA of these systems depends
primarily on the skills and knowledge of the operators of these systems. Consequently,
technical competence in the operation of this product must be supplemented by a thorough
understanding of the systems being tested.

any kind. The use of lubricants may interfere with normal operation of the phantom.
Do not immerse
The QUASAR Programmable Respiratory Motion Phantom is not protected against the
ingress of fluids. Fluids may interfere with its operation. The phantom should not be
immersed in liquids of any kind.
Maintenance
The QUASAR Programmable Respiratory Motion Phantom is manufactured to very close
tolerances in order to ensure accuracy and good performance for many years. The
materials used (predominantly acrylic) are susceptible to damage from impact,
scratches, temperature extremes and chemicals including solvents and cleaning
products. In addition, the acrylic and other parts are susceptible to dimensional changes
due to the absorption of moisture.
Cleaning Instructions
The phantom can be wiped clean with a damp cloth containing a mild soap or detergent
solution.
1. Use a clean, soft cloth and apply only light pressure.
2. Dry the phantom by blotting with a clean cloth or chamois.
If attempts to clean the phantom using this technique are unsuccessful, please contact
Modus for approved alternatives.
Do NOT use:
• Wax-based or chemical-based cleaners,
• Window cleaning sprays,
• Kitchen scouring compounds, or
• Solvents such as acetone, gasoline, benzene, alcohol, carbon tetrachloride, or
lacquer thinner.
These products can scratch the surface and/or weaken the acrylic and other plastic
parts. Damage caused by inappropriate cleaning activities is not covered by the
warranty.
Scratches that arise during the normal use and handling of the phantom should not
inhibit its performance. If you are concerned about any issue regarding the appearance,
care, or performance of the phantom please contact Modus.
Operating Conditions
The QUASAR Programmable Respiratory Motion Phantom is designed and manufactured
to operate at normal office or clinic temperature and humidity. Rapid changes in
temperature and humidity may cause temporary changes in the dimensions of some
parts. For this reason we recommend that the phantom be stored in the office or clinic
where it is used. If conditions change, time is required to allow the phantom to become
stable in the new conditions. We recommend that the phantom be stored and used in
the following environmental conditions:
• Temperature: 15 degrees to 30 degrees Celsius
• Humidity: 30% to 80% relative humidity (non-condensing)
• Altitude: up to 2000 metres.
• Indoor use ONLY
• Installation Environment: Installation Category II
• Pollution Environment: Pollution Degree 2
Appearance
The QUASAR Programmable Respiratory Motion Phantom may be damaged by extended
exposure to direct sunlight, such exposure should be avoided. With age, exposure to
sunlight and exposure to higher energy photons, the acrylic in the phantom may acquire
a yellow tint. This is a normal and unavoidable characteristic of acrylic that does not
affect the performance of the phantom.

Description
The QUASAR Programmable Respiratory Motion Phantom is designed to enable
dosimetric and non-dosimetric measurements on a moving test object.
The Phantom features a cylindrical Insert moving within a Body Oval. Motion occurs in
the superior-inferior direction. Several different Inserts are available for different
applications. Insert motion is linked to a moving Chest Wall Platform designed to work
with third-party motion tracking systems.
The Programmable Respiratory Motion Phantom can run with or without a computer.
There are three Modes of Operation:
• Oscillation Mode – in which motion is under software control and motion profiles
are programmable
• Rotation Mode – in which motion is under software or local control and motion
profiles are sinusoidal
• Position Mode – in which the position of the Translation Stage is under software
or local control.
The Respiratory Motion Phantom Software application is used to send commands to the
phantom. In this application you can import, create, edit and save motion waveforms.
The QUASAR Programmable Respiratory Motion Phantom is suitable for QA of most
external beam installations including 3D conformal radiation therapy and intensity
modulated radiation therapy (IMRT) systems. It can also be used to test imaging
systems including x-ray CT, PET and SPECT.
Procedures for using the phantom, which are described in the sections that follow,
should be implemented as part of an appropriate QA program.
NOTE: The phantom and moving inserts are designed to test the function of
radiotherapy and imaging systems under typical operating conditions. They should not
be expected to expose every potential problem associated with the systems being
tested. Users should develop tests to address specific features and functions of the
equipment being tested.

Translation Stage
The Translation Stage is driven in a horizontal direction. Amplitude of motion is adjusted
by repositioning the thumbscrew. There is a scale on the top edge of the translation
stage to assist with setting the amplitude. The Translation Stage has a magnetic clasp to
attach moving inserts.
The Translation Stage amplitude must be set to 20 mm for programmable motion in
Oscillation Mode and for accurate positioning in Position Mode.
When an ion chamber holder is in place in the moving insert the center of the chamber
will align with the center of the body oval when the Translation Stage is at the center of
the stroke.
Cams
There are two cams inside the Programmable Drive Unit housing. The cams are used to
drive the vertical motion of the Chest Wall Platform. One cam has a sinusoidal profile
(circular) the other has the profile of a typical respiratory cycle (see figure 5). Both
profiles have a 10 mm peak to peak amplitude.
The Cam with a sinusoidal profile must be used for programmable motion in Oscillation
Mode.
Clamp
The Clamp is used to attach the Programmable Drive Unit to the Body Oval. The Clamp
consists of a cylindrical acrylic extension, which passes through the Body Oval, plus a
cross bar and thumbscrew to secure the Programmable Drive Unit to the Oval. The
extension is the same material as the Body Oval.
Cables
The QUASAR Programmable Respiratory Motion Phantom is provided with a standard 2
m long Ethernet Cable and Crossover Cable Adapter. The Crossover Cable Adapter can
be at either end of the Ethernet Cable.
The Ethernet Cable and Crossover Cable Adapter connect the Programmable Drive Unit
to a computer or computer network for software control.
Power Supply
Ratings
Power is supplied by an Elpac Power Systems 50 W switching power adapter model
FWC5024F: 110 - 240 V~, 50 - 60 Hz, 1.0A; Unit input: 24Vdc, 2.1A.
Mains supply voltage fluctuations must not exceed 10% of the nominal supply voltage.
Power Cable
The phantom is supplied with a North American standard cord set. This cord can be
replaced with a territory specific cord set with a matching IEC 320 connector. The power
cord is the disconnect device.

rotation run to a text file. The encoder values record the phantom’s actual
motion during then run and are stored as the stage position in mm.
• Settings opens the Phantom Control Settings window.
The Settings window, shown in Figure 17, contains three sections: Device Discovery,
Motor Properties, and Phantom Data Preferences.
Device Discovery
The Device Discovery table shows all the devices available for the software to connect
to, including the Simulation phantom. To refresh the list, click Refresh. To configure a
device (the Simulation phantom cannot be configured), click Configure… This will open
the Device Configuration window.
Device Configuration
The Device Configuration window can be used to modify a phantom`s network settings,
including the name of the device as it will appear in the Device Discovery table. To
change any setting, modify its value (to a valid new value) and click Update. Updating
the device will cause it to reboot and it will not appear in the Device Discovery table until
the reboot is complete (this may take up to 60 seconds). To load a new firmware file to
the phantom click Load Firmware and select a valid firmware file. For details on updating
the firmware, refer to Appendix 7.
Motor Properties
The Max. Acceleration and Max. Velocity parameters are used in testing (in the
Waveform Editor) to determine if the motor can accurately reproduce a waveform’s
oscillations. The true value of these parameters depends on the load placed on the
motor and can be approximated via the Acceleration Test. The acceleration test uses
rapid oscillation to determine the limits of the motor’s acceleration and velocity.
The Motor Range parameter can assume values between 15 mm and 20 mm (default
value is 20 mm) and is used for the motor position displays in the software. This
parameter should match the amplitude of the translation stage.
The Phantom Data Preferences determine if encoder data, retrieved from the motor
after executing oscillations or rotations, are saved. If Save To Encoder File is checked,
the data will be saved. If Prompt Before Saving is checked, the user will be prompted for
the save location after a run is completed, otherwise the data is saved to the Default
Waveform Location (see Drop-down Menus section). The file has a default name of
‘pRespOutput_<date>_<time>.csv’, where <date> and <time> denote when the motor
motion completed. The values in the file report the translation stage location in mm in
10 ms intervals.

Appendix 1: .QRM file format
This is the format that is used when saving files from the QUASAR Programmable Respiratory Motion
Application. The file is made up of a header followed by a column of respiratory motion displacement
data. The QRM format has a fixed time interval of 10 ms (0.010 s), therefore the time column is not
displayed.
The displacement values are scaled between 1 and -1. The displacement values are signed floating
point numbers.
There is a multiplier value in the header that is used to scale the data into cm. The default is 1.5 (the
maximum linear displacement of the Programmable Drive Unit in Oscillation Mode)

QA Thoughts: Gate 80% - 90%, such that the tumor is moving Sup with no Lat movement. This would
also occur during a nearly complete relaxed exhale. The tumor is expected to move only
about 1.0 cm, since:
(100 mm / 100% cycle) : (90 mm / 90% cycle) & (80 mm / 80% cycle)
δx = (90 mm) – (80 mm) = 1.0 cm
The amplitude of the Translation Stage can be adjusted by loosening the thumbscrew, moving the Stage with respect to the Programmable Drive Unit, and then retightening the thumbscrew. It can be set from 0 to 20 mm in Oscillation Mode and Position Mode, and from 0 to 40 mm peak to peak in Rotation Mode . During adjustment, it is important to use the amplitude scale on the Programmable Drive Unit and verify the amplitude with the motor running to ensure accuracy and proper functionality .
The QUASAR Programmable Respiratory Motion Phantom ensures accurate positioning in Position Mode by requiring the amplitude of the Translation Stage to be set to 20 mm. This setting aligns the center of the ion chamber with the center of the body oval when the Translation Stage is at the center of the stroke . The amplitude is adjustable using a thumbscrew, and accuracy is maintained by checking the amplitude with the motor running after adjustment .
The hardware warranty for the QUASAR Programmable Respiratory Motion Phantom covers defects in materials and workmanship for one year from delivery, providing free repair or replacement for covered defects . In contrast, the software warranty assures substantial performance adherence as described in Modus materials and covers one year from delivery, focusing on rectification or replacement of defective software . Both warranties exclude coverage for user modifications, misuse, and external damage sources, yet the remedies differ slightly in terms of return procedures and related actions .
The spectrum visualization tool in the Wave Editor application of the QUASAR Programmable Respiratory Motion Phantom plots the Fast Fourier Transform (FFT) of a waveform and a smoothed version of the FFT, displaying the frequency domain representation from 0 to 60 bpm . This tool benefits users by allowing them to visualize dominant respiratory frequencies and assess the impact of filters, which is crucial for applications that require precise replication of respiratory motions. It aids in debugging and refining waveforms for accurate simulations .
To transition from Software Control to Local Control, disconnect the phantom by clicking the 'Disconnect' button in the software application . Additionally, the phantom automatically reverts to Local Control if the Ethernet Cable is disconnected, or if the Software Application is closed or times out after approximately 15 seconds . These automatic triggers ensure fallback to a standard operational mode in cases of accidental disconnection or software issues, maintaining the phantom's usability and control integrity .
The QUASAR Programmable Respiratory Motion Phantom operates in three modes: Oscillation Mode, Rotation Mode, and Position Mode. Oscillation Mode is available only under Software Control and involves a programmable waveform with amplitude automatically scaled to 30 mm peak to peak or less . Rotation Mode can be controlled locally or through software, featuring a sinusoidal motion profile with adjustable speed and amplitude . Position Mode allows setting the position within a range of +/- 20 mm from center; control of this mode is also possible locally or via software .
Aligning the ion chamber holder with the body oval is crucial for ensuring accurate measurements during motion simulation. This alignment is achieved by setting the amplitude of the Translation Stage to 20 mm, positioning the chamber at the center when the stage is at mid-stroke . This precise setup is essential for replicating the correct physical conditions of respiration, which affects the accuracy of experimental data and, consequently, the reliability of radiation treatment simulations or other clinical applications .
Connecting the QUASAR Phantom to a computer or network enables Software Control, expanding the phantom's operational capabilities beyond local control. It allows for remote control using the QUASAR Programmable Respiratory Motion application, enabling detailed adjustments and the use of programmable waveforms . Network connections facilitate managing multiple phantoms simultaneously, enhancing experimental setup efficiency. However, disconnection from a network prompts an automatic fallback to local control, which means careful management of connectivity is needed to avoid unintended mode shifts .
The removable Chest Wall Platform is designed to replicate vertical chest wall motion, making it suitable for optical tracking devices such as the RPM device from Varian . It uses two cam profiles: a sinusoidal cam for applications needing consistent motion replication, and a cam replicating a typical respiratory cycle for more realistic simulations . The sinusoidal cam is used in Oscillation Mode for programmatically controlled motion, while the respiratory cycle cam helps mimic an actual breathing pattern, thus enhancing simulation accuracy for medical and research applications .
The Control Knob is essential for local operation of the QUASAR Phantom, allowing users to start and stop motion and adjust speed or position without needing a computer. In Rotation Mode, pressing the knob starts/stops motion and turning it changes speed . In Position Mode, holding the knob shifts the phantom into this mode from Rotation Mode, with further turning adjusting the Translation Stage's position relative to the center . The knob thus provides intuitive, hands-on control across modes, facilitating quick adjustments and operational independence from external devices .
