Joint Mobility and Balance
1) Observe the motion of
the individual joints of the
shoulder:
• Sternoclavicular
• Acromioclavicular
• Scapulothoracic
• Glenohumeral

All of these need to move


in harmony to create
healthy shoulders.

Sternoclavicular Movement
The clavicle articulates with the
manubrium.

Saddle type joint with 3 planes of motion


and a fibrocartilaginous disc.

Clavicle rotates up and down with


elevation and depression of the scapulae.

Clavicle rotates anterior and posterior with


protraction and retraction of the scapulae.

Clavicle rotates on its axis in response to


shoulder movement.

Acromioclavicular Movement
Gliding synovial joint between the end of the clavicle
and the anteromedial surface of the acromion.
There is some motion in this joint as the shoulder moves
but the exact mechanics are not entirely clear.

Scapulothoracic Movement
The scapulothoracic joint is defined by the
movement of the scapula on the rib cage.

Not a traditional bony joint but a soft tissue


articulation created by the coordinated
action of the muscles of the shoulder.

The scapulothoracic joint moves in:


• Elevation/depression
• Protraction/retraction
• Upward/downward rotation

Scapulohumeral Rhythm
When the shoulder moves into
flexion or abduction above
approximately 60-80 degrees, the
scapula and humerus must
coordinate their movement to
minimize impingement of the
humeral head against the acromion.

The scapula and humerus move in a


1:2 ration. In other words, the
humerus moves 2x as fast as the
scapula. This is called the
scapulohumeral rhythm.

Examples of Depression: Pulling the shoulder blades down toward the hips

Examples of Adduction/Retraction: Pull the shoulder blades toward the spine to stretch the
chest

shoulder
Examples of Downward Rotation: Anytime the arm returns to the side

Glenohumeral Movement
A synovial ball and socket joint with a small
socket (glenoid fossa) and a big ball (humeral
head).
Has a large range of motion in all planes
including:

Flexion/extension
Abduction/adduction
Horizontal abduction and adduction
Medial/lateral rotation

Creating Glenohumeral Stability
The rotator cuff is responsible for
positioning the humeral head in the
glenoid fossa so the big muscles can
move the arm while maintaining
congruency of the glenohumeral
joint.

The rotator cuff should be trained


for endurance rather than strength
so low resistance and high reps is
the goal.

Glenohumeral Congruency

When the humerus rotates, the humeral


head should stay relatively centered in
the glenoid fossa.

This pattern may need to be trained in


clients with unstable or hypermobile
shoulders.

Lack of congruency in the glenohumeral


joint can lead to anterior shoulder pain,
biceps tendinitis and bursitis.

• Focus on keeping the head of the humerus
from rolling forward and back as it rotates.
• Focus on endurance rather than strength so
use light or no resistance and high
repetitions.
• Consider putting the arms in different
positions and working the rotation in
different positions - goal post arms, straight
elbow, arms in flexion or abduction.

Enhancing Mobility of the Shoulder
Use dynamic flexibility exercises to
maintain or improve range of motion
and train coordination and balance of
the shoulder muscles.

Use self myofascial release to remove


minor limitations.

For more severe limitations refer client


to a PT or manual therapist for
treatment.

Include exercises to mobilize the


thoracic spine as well as the shoulder.

Scapular control is essential for creating


appropriate force transmission through
the upper body to generate power and
to minimize the risk of injuries to the
shoulder girdle, neck and thorax.

Developing Scapular Control
Step 1: Begin with awareness of scapula
position through mobility and basic
scapular stability exercises.

Step 2: Progress to chest presses and


horizontal pulling exercises below 90
degrees of flexion/abduction that don’t
require upward rotation of the scapulae.

Step 3: Gradually work the arms above 90


degrees of shoulder flexion/abduction to
gain control of the scapula in upward
rotation.
