INTERVIEW QUESTIONNAIRE FOR 2/E

Q1. Where did you graduate?

I graduated from: (choose appropriate to you from below answers)

- Viet nam Maritime University , the abriviation is VIMARU.

Q2. Where were you born?

I was born in Thai Binh Province of Viet nam. It is about 70 km from Haiphong and
100 km from Hanoi Capital.

Q3. How long had you been working on board?

I had been working onboard the ship for about 10 months, from July 2015 to April
2016.

Q4 . Which company have you work for?

I have been working for T&T Shipping Company since 2012. But before that I
worked for some other companies: VINASHIP, VINALINES, ….

Q5. Which brand of main engine did you operate? Which type of main engine
did you operate?
I worked with the engines of various brand such as: MAN B&W, MISHUBISHI,
HANSHIN, AKASAKA …

Q6. How about out put power of main engine had you operated?

I operated main engine with output power from 3,000 to nearly 9,000 wats.

Q7. If some cylinders of main engine were suddenly increased its exhaust
gas temperature, can you tell me reasons of this problem and how to
solve this problem?
The reasons of this problem may be:

- Fail of fuel injection valve,

- Leaking of exhaust valve,

- Not enough intake air ( failed of intake valve, air scavenge air box),

- Incorrective of fuel injection valve timing or damage of fuel injection pump.

Q8. What should you do when occurring a fire in the scavenge air box?
When occurring a fire in the scavenge air box, I should do following items:

- Reducing speed to slow and asking bridge for permision to stop main engine.

- Stopping the fuel oil supply,

- Putting the scavenge air box fire extinguishing into function.

Q.9. What can continuous surging of turbocharger be caused of?

Continuous surging of turbocharger can be caused by:

- Mereased back pressure after turbocharger fould or damaged turbin nozzle ring
and damaged bearing.

Q10. What can a few spots of turbocharger be caused by?

A few spots of turbocharger can be caused by:

- Crash stopping of main engine

- Rapid increasing of main engine speed.

Q11. Why the engine speed must be reduced when one or more cylindes out
of operation ?
With one or more cylinder out of operation the main engine speed must be
reduced because:

- In such cases the air supply is no longer optimal

- The thermal load will be higher

- Governor hunting will occur

- Torsional vibrations as other mechanical vibrations may occur.

Q12. When the ship under manouvering if exhaust valve of one cylinder
could not close. What should you do?
It should do the following:

- Close fuel valve before fuel pump of that cylinder

- If possible, should hang up fuel pump of that cylinder by specail tools.

Q13. When the ship is under navigation and you are on watch, if occuring
blackout, what should you do?
When such case occurs, I should do following:

- Inform to the bridge and chief engineer,

- Keep standby generator engine and supply electric power to main switch boad.

- Start main engine.

- Find out reasons of blackout and solve the problem.

Q14. Can you tell me the function of cylinder oil for diezel engine.

The function of cylinder oil for diezel engine are:

- Lubricating surface of cylinder liner and piston ring,

- Cleaning surface of cylinder liner sealing the combustion chamber.

Q15. When can you discharge bilge water via 15 ppm equipment and how is
about discharing flow via this equipment?
According Regulations in MARPOL we can discharge bilge water via 15 ppm
equipment when:

- The ship should be en route,

- The ship is not in special areas as maditeranan sea, red sea, etc…

- Follow regulation of some country in which required a distance between the


ship and nearest coastal.

- Content of discharged oilly water mixture is less than 15 ppm.

- Discharing flow via 15 ppm equipment is as bilge water discharing flow table
that was approved by the register.

Q16. If our oily water separator (OWS) or overboard discharge monitoring


equipment breaks down. What should you do?
We should do as following :

- Immediately stop the flow of effluent over board.

- Fromptly record the failure of the equipment inthe record book.

- Retain all bilge water on board until complete repairing the equipment or

- Discharge bilge water to approved reception facility if needed.

Q17. To avoid PSC’s difficencies, as management office, what should you do


before your ship entering the port?
To avoid difficiencies and get good PSC records I should direct our engine crew to
do and do my self following items:

- Check condition of all safety equipment, fire fighting equipment, life saving
equipment, as emergency generator, emergency fire pump,life board, fire dampers, 15
ppm equipment, waste incinerator, batteries, entrance doors, emergency escape trunk,
co2 fire fighting system.

- Repair them if trouble occur.

- Clean and arrange the engine room ( bilge wells, floors, machineries, store, work
shop, aceommadition area, air vent pipe coaming).

- Check correct all log book and oil record book.

Q18. What do you base on to carry out procedures on board the ship?

Carrying out procedures onboard the ship, crew shoul refer to:

- SMS manual (Safety Management System Manual)

- Equipment Instruction Manual

- Other Onboard Guides

Q19. What do you base on to make maintenace plan for machineries?

Maintenace plan for machineries to be done onboard based on the following


items:

- Running hours of machineries as the instruction of manufacturer,

- Actual condition of machineries,

- Requirement of registers manufacturer and company.

Q20. How about your family?

Thank you for your attention to my family. I live in Haiphong city with:My Parent,
Wife, 02 children, son and daughter. My son is 8 years old and daughter is only 2 years
old.

- We live in my parent’s house. I still have not my private house.

- With my salaly when work for your vessels I hope after some contracts I could
save enough money for buying a new house.

Q21. How about content of sulfur in fuel oil using for specail area?

- For M.D.O content of sulfur is 0.01 percent,

- For speceail area (coast), for H.F.O content of sulfur is 0.1 percent, for
european emission cotrol area (ECA)

Q22. If there is ballast water in the fuel oil tank. What should you do?

It is to do:

- Transfer fuel oil to other empty tank for setting,

- Separate water by purifier,

- Send the sample of that fuel oil to the supplier for analysis if need.

Q23. Have you ever worked on board the ship with foreign chief engineer?

- Yes, I have worked on board the ship with Japanese chief engineer, Taiwanese
chief engineer.

(- No, I have never worked with foreign C/E).

Q24. Have you ever been to Europe?

- Yes, I have. That europe countries are Belgium, Spain, France, Italy …

Q25. Please tell me about main engine have you ever operated?

I already worked with some kinds of M/E such as:

- Two strocks engine MAN B&W: …

Q26. Are you married ?

- Yes, I am married. I have got married since 2005, almost 10 years already.

Q27. How many children do you have?

- I have one son, one daughter. My son is 8 years old and daughter is only 2.

Q28. Do you want to become chief engineer?

- Yes, I do . But I want to work a second engineer for some years more to get
more knowledge and skills before become a C/E.

Q29. How long does it take you from your home to office?

- It takes about 15 minutes by car or motocycle. That distance is about 5


kilometers, quitely so far.

Q30. What are your duties on board the ship?

As a 2nd engineer, I have to perform my duties which clearly described in


Company’s SMS manual for my ship. These are in brief as below:

- Responsible to the Chief Engineer for day-to-day operation of the engine room
department.
- Efficient operation and maintenance of main engine and related machinery,
manoeuvring system, cargo turbines, pumps and associated control equipment, crane
and cargo handling equipment and other …
- Be in charge of scheduled maintenance and repair equipment under his
control.
- Ensure the technical state and normal working condition of main engine,
propeller shaft system, emergency steering, fresh water distiller, steering gear,
generator, safety and fire fighting equipment in E/R, air compressor bottles for starting
engine, deck machinery such as mooring winch, crane and anchor, cargo handling
gears, oily water separator, sewage treatment plant, ballast water treatment plant;
automatic devices, testing, measuring or checking devices or
- Make maintenance plan of engine department; assign watches and
maintenance duties, check the working days and arrange the time off for crew of
engine department.
- Engage in watch keeping duties from 04:00 to 08:00 and from 16:00 to 20:00.
- Report “Voyage Stand-by” under his control to Chief Engineer at least 3 hours
before ship departure.
- Receive, maintain, distribute, adjust and calculate the amount of lubricant oil.

- Monitor and record all specifications, repaired or maintained items; manage all
types of records, technical document and engine log books;
- Familiarise with the contents of all relevant Company’s manuals and ensure
that all personnel in engine department are also familiar.
- The Second Engineer shall actively understudy the Chief Engineer in all
aspects of his duties and responsibilities and assist him as required;
- Carry out other duties according to the Chief Engineer’s orders.- obey all the
orders of the chief engineer, make working plan for every day, month, inport.
Command engine crew to perform the jobs of the engine depatment.
Q31. How many ships did you work as rank 2/E?

- I worked as 2/E for two ships.


- The ships are recorded as in my Crew Bio Data (CV) sent to you. They are MV

Q32. Have you ever worked on board the ship with multi-nationality crew ?

- Yes, I have
- I already worked with crew of other nationalities such as Chinese, Japanese,
Ukrainian, Philipino …
Q33. Can you weld?

- Yes, I can do it not so well but it can be fit whenever ship has no fitter.
Q34. Please briefly describe some Conventions, Regulations & Code applied
for shipping industry . Do you know ?

- SOLAS: is abbreviation of “Safety Of Life At Sea” , is the International


Convention on Safety Of Life At Sea. Its regulations set out the safety
standards of ship’s construction, equipment, safety appliances and safety
manning.
- MARPOL: is abbreviation of “Marine Pollution”, is the International Convention
on Prevention of Pollution From Ships. It contains antipollution requirements
for ships such as antipollution equipment and tools, requirements on archiving,
transferring and discharging oil, oily water mixture as well as garbage. The
latest update requirements are also concerning air pollution.
- COLREG 72: is abbreviation of “International Regulations for Prevention of
Collision At Sea”. These rules set out the regulations for ships on lights, signals
and actions to conduct at sea to avoid collision. It is also called as “ The Rules
Of The Roads”.
- SAR 79: “International Convention on Maritime Search and Rescue (SAR). The
Convention was aimed at developing an international SAR plan, so that, no
matter where an accident occurs, the rescue of persons in distress at sea will
be co-ordinated by a SAR organization and, when necessary, by co-operation

between neighbouring SAR organizations. Although the Convention is setting
out obligation of ships to go to the assistance of vessels in distress.
- MLC 2006: “Maritime Labor Convention 2006”. Being established in 2006, the
Convention have been going into force since 20 August, 2013 and containing
provisions regarding 5 main articles as 1) Minimum requirements for seafarers
to work on a ship; 2) Conditions of employment; 3) Accommodation,
recreational facilities, food and catering; 4) Health protection, medical care,
welfare and social security protection; 5) Compliance and enforcement.

- Yes sir. But I’ve just read about that type of engine in instruction book
[Link] kind of overhaul work that have you ever done with ME ?
- I’ve already done overhauling exhaust valve, cylinder cover, piston, fuel
injection pump, fuel injection valve, puncture valve…..
[Link] many runing hours do you overhaul piston ? And How do you
overhaul it ?

- First, I discuss with CE for taking permission from Port Control. Then, I check
and make sure that we have enough spare parts and special tools. Then, we
organize a tool box meeting to make sure that everyone understand about
their duty and confirm for safety. I make a risk assessment to make sure that
we don’t miss any issue that could lead to danger for man and machinery….
Q38. Who will be respondsible for dismaltement M/E stuffing box?

- I will, sir

Q39. Have you ever measured M/E crankshaft deflection ? How many
positions must you check?
- Yes, I have. There are 5 position, sir

[Link] you ever checked main bearing clearance? How do you check?

Which special tool do you use to check?

- Yes, I have.
- I use dial gauge for checking main bearing clearance
- If there is too large a difference in the crankshaft deflection readings, check
the individual bearings
Calibrate the main bearing measuring tool : insert the measuring tool in the
calibration block , set the dial gauge to 0 (zero)
Remove the [Link] pipe from the main bearing cap
Insert the measuring tool and check the clearance between the upper shell and
the journal . The difference between the actual clearance measurement and
the measurement recorded in the adjustment sheet or the clearance noted for

a new bearing installed later must not exceed 0.1 mm . An appriciable increase
in clearance may be an indication of loose staybolts or wear of the lower shell
After measuring , dismount the bearing cap and the upper shell . For further
inspection of the bearing. Inspect the bearing and the journal.

[Link] you know propeller shaft grounding system? What is it used for?

- Yes, I do.

- When the propeller shaft revolves it is more or less electrically insulated from
the rest of the hull. This can result in spark erosion in the shaft bearings and
gear. Propeller shaft grounding provides a low electrical resistance to the hull
preventing erosion damage.

- Grounding the shaft will also include the propeller to the ship's cathodic
protection system.

- It is essential that the shaft grounding device represents a very low ohmic
resistance.

Q42. Do you know M/E revolution per minute detector device? Where is it?

- Yes, I do. It is installed on fly wheel cover

- Oil mist high level, Cooling FW pressure low, Cooling FW high temperature,
Piston cooling oil high temperature, exhaust temperature high, exhaust
temperature high deviation….

[Link] much high exhaust temperature deviation that M/E slow down?

- It’s more than 50oC

[Link] do you know that may be cylinder liner have crack when ME
running?

- Cooling fresh water pressure hunting

[Link] do you test ME overspeed ?

- Touch ” over speed shut down test” button on the simulation test menu screen
of the C/R display
- The conditions for the test are indicated at “preparation” on the screen. Each
indication lights up when the corresponding condition is met.”over speed
mode”button becomes effective when all are satisfied.

- Touch “over speed mode” button to turn the speed control dial to the fuel
supply position, which makes”output rpm” button effective
- Touch”output rpm” button to allow automatic rpm increase up to overspeed
level
- An overspeed alarm “overspeed” is issued to indicate “overspeed shutdown” on
the screen
- Once the above has been completed , perform the following recovery operation

Operate the speed control dial to stop position


Press “ CAUSE RESET” button on C/R operating pannel to confirm the cause
The cause indicator”overspeed” goes out from the display.

Q47. During watch keeping , you found the fire in the engine room. At that
time , what will you do ?

 If the small fire , I will carry fire extinguisher for extinguish


 If the big fire , I will push fire alarm system immediately
Q48. Before entering enclosed space . what should you do ?

 Prepare tools.

 Check all items in the checklist, such as test content of oxygen, test
explosion point, exploded-proof lamp, good ventilation.

 Communication means should be available between operators inside and


watchman outside and so on. Operators should wear protective clothing,
a helmet, working boots, lifeline. Also a mark plate “Dangerous Operation
in Progress” should be established at the entrance.

Q49. When will an engineer in charge of watch call the Chief Engineer?

 When engine damage or malfunction occurs which may be such as to


endanger the safe operation of the ship; when any malfunction occurs
which it is believed, may cause damage or break down of propulsion
machinery, auxiliary machinery or monitoring and governing systems; and
in any emergency or if in any doubt as to what decision or measure to be
taken.

Q50. What check have to be made before any hot work is permitted ?

 I must make sure that : ther is no gas leakage on the cargo deck or pump
room area , the near by places shall be free of inflammable materials or
gases , the area where the hot work is done is not piled with solvents of
any kind , including diesel oil , kerosene , paint cleaners and thinners .
Gas testing is also important to make sure there is no flammable or toxic

gas is present at the work site is and the oxygen content is 21 % by
volume . Ventilation condition should also be checked before hot work.

Q51. How to operate waste oil incinertor ? what kind of garbage can not be
trown into waste oil incinerator ?

 Firstly, check the oil level in the oi tank , open residue cook and discharge
oily water, heating the bilge oil in the tank to 70-80 degree centigrade

 start the incinerator . In the beginning , combustion bilge oil with light oil
and after the water content in bilge oil is comparativey low , stop
supplying light oil and use bilge oil only.

 Attention : the temperature in combustion can not be higher than the


stipulated figures of the incinerator ( usually not more than 500 degree
centigrade ).

 The following garbage can not be thrown into the wate oil incinerator :

- Any containers which have contained gases under pressure or aerosol


cans into the wate oil incinerator

- The materials that may produce harmful gases and ashes.

- The materials that may produce high temperature and prolonged


incineration of such materials that may cause damage to the incinerator.

Q52. Garbage Management and Log?

According to revised MARPOL Annex V shipboard generated garbage is to be


grouped into the following categories:

1. Plastics – Garbage that consists of or includes plastic in any form, including


synthetic ropes, synthetic fishing nets, plastic garbage bags and incinerator
ashes from plastic products. Garbage under this category is prohibited to be
discharged at sea.
2. Food wastes – Spoiled or unspoiled food substances. Food wastes may be
discharged at sea under specific circumstances/requirements (refer to
the simplified overview of the discharge provisions of the revised MARPOL Annex
V developed by IMO).
3. Domestic Wastes – Garbage generated mainly in the accommodation spaces
on board the ship (e.g. drinking bottles, papers, cardboard etc). Garbage under
this category is prohibited to be discharged at sea.
4. Cooking Oil – Edible oil or animal fat used for the preparation or cooking of
food. Garbage under this category is prohibited to be discharged at sea.
5. Incinerator ashes – Ash and clinkers resulting from shipboard incinerators
used for the incineration of garbage. Garbage under this category is prohibited to
be discharged at sea.
6. Operational wastes – Solid wastes (including slurries) that are collected on
board during normal maintenance or operations of a ship, or used for cargo

stowage and handling. Operational wastes also includes cleaning agents and
additives contained in cargo hold and external wash water that may be harmful
to the aquatic environment. Operational wastes does not include grey water,
bilge water, or other similar discharges essential to the operation of a ship
(boiler/economizer blow down, gas turbine wash water, machinery wastewater
etc). Garbage under this category is prohibited to be discharged at sea.
7. Cargo residues – Remnants of any cargo which remain on the deck or in holds
following loading or unloading. This category does not include cargo dust
remaining on the deck after sweeping or dust on the external surfaces of the
ship. Such garbage may be discharged at sea under specific
circumstances/requirements (refer to the simplified overview of the discharge
provisions of the revised MARPOL Annex V developed by IMO). It is essential
to remember that besides other requirements (e.g. distance from shore) cargo
residues in order to be discharged at sea they should not be harmful to the
marine environment. Cargo residues which are considered harmful to the marine
environment are classified according to the criteria of the United Nations
Globally Harmonized System for Classification and Libeling of Chemicals (UN
GHS) meeting parameters such as: acute aquatic toxicity category 1, chronic
aquatic toxicity category, carcinogenicity, mutagen city, reproductive toxicity etc
8. Animal Carcasses – Bodies of any animals that are carried on board as cargo
and that die or are euthanized during the voyage. Discharge of such wastes
permitted at sea under specific circumstances/requirements (refer to
the simplified overview of the discharge provisions of the revised MARPOL Annex
V developed by IMO).
9. Fishing Gear – Physical device that may be placed on or in the water or on the
sea-bed with the intended purpose of capturing marine or fresh water
organisms. Garbage under this category is prohibited to be discharged at sea.

Finally, following a ships’ use of port reception facilities the ships’ crews and agents
should encourage waste reception facilities service providers to use the IMO standard
format for the waste delivery receipt.

Garbage Disposal Log should be filled up correctly in strict compliance with


regulations.

Q53. How many colors of garbage are there on board the ship ?

Red , black, blue ,yellow, brown

Q54. How do you segregate garbage on board the ship ?

Red – plastics

Black – oily rags /incinerator ash

Blue – food waste

Yellow – cargo residues, paper products, rags, glass, cans , tins , crockery,
floating dunnage , linning and packing materials

Brown – batteries and aerosols

Q55. Please tell me how to overhaul ME high pressure pump with VIT type

Q56. Why are the exhaust gas temperature of all cylinders very high ?

Increased scavenge air temperature owing to inadequate air cooler function

Fouled air and gas passages

Inadequate fuel oil cleaning or altered combustion characteristics of fuel

Wrong position of camshaft

Q57. Please, tell me what problems that you had to deal with ME ? And
how did you do in such situation?

Q58. While you work on board , because you are 2nd engineer so you have to
help 3rd engineer to overhaul diesel generator engine’s turbo charger.
Please, tell me how to work ? And tell me which measurements do you have
to make and how much is limited of that measurement ?

I should take the following clearances with turbocharger in MET type:


1. Radial clearance: (0.37-0.67 mm)

 Set the dial gauge at the top end of compressor impeller and set dial
gauge to zero.
 Push the turbine shaft upwards at compressor side by hand.
 Record the reading on dial gauge.
 The total movement from upper position to lower position means radial
clearance.
2. Axial clearance: (0.06-0.12 mm)

 Push turbine shaft toward turbine side by hand and set dial gauge to zero.
 Push turbine shaft toward compressor side by hand.
 Record the reading on dial gauge.
3. Radial tip clearance at gas outlet part:

 Measure clearance between housing and impeller blade at four positions


“top, bottom, port, starboard”
 Take calculation “top+bottom”, “port+starboard” and compare to
permissible range of clearance (0.75 x 2=1.50 mm)
4. Width of sealing ring groove

I should take the following clearances with turbocharger in VTR type:


1. Measure the “K” value at the blower side

 The “K” value should be measure between the shaft end and the
flange of bearing cover after removing the bearing covers.
 Compare the measured “K” value with stamped one on plate.
2. Measure the “M” value

 Push the rotor to the turbine side and measure “K2” at the blower side.
The back of the blower impeller should be touched with the gland
strips of the partition wall.
 Calculate M= K2 – K
 Compare the measured “M” value with allowance of “M”
3. Measure the “L” value

 Push the rotor to the blower side and measure “K1”. The front of the
blower impeller should be touched with the blower casing insert.
 Calculate L=K- K1
Compare the measured “M” value with allowance of “ L “

Q59. How do you treat the ME Lub Oil ? How much temperature do you set
when operation LO purifier?

Q60. How many months do you take LO for analysis?

Every 6 months
Q61. Do you have experience with crane system on board the ship ? How do
you do before arrival the port?

Yes, I have

Before arrival the port , I often check the LO level , check the brake , check the
LO cooler in normal condition. If the deck crane system is operated in the cold weather
, I must warm up the system

Q62. If the LO temperature of crane is high alarm . How do you do?

Find the trouble and clean the LO cooler immediately

Q63. How do you require spare part and store?

Q64. Do you have experience to face with AMSA and US Coast Guard? Does
your ship have any defficency?

Q66. How do you test ME fuel oil leaking alarm?

Q67. How do you change HFO to DO? Which temperature will you change and
which temperature so that DO already coming in engine?

Q69. Your ship bunker in Singapore and when finish pumping you found that
your ship have abour 20MT shortage of FO. How do you do?
