Chapter 1
THE PROBLEM AND ITS BACKGROUND
Introduction
Hyperlipidemia, a key contributor to cardiovascular diseases, is a global health concern.
While pharmacological treatments like statins are widely used to manage lipid levels, these is
increasing interest in natural, dietary-based interventions due to their fewer side effects and
additional health benefits (2024 healthline media LLC) . Allium sativum (garlic), Abelmoschus
esculent (okra) have been traditionally recognized for their lipid-lowering properties. Garlic is
rich in organosulfur compounds, such as allicin, which have been shown to reduce cholesterol
synthesis and improve overall lipid profile. Okra, on the other hand, contains high levels of
soluble fiber and antioxidants, aiding in the regulation of blood cholesterol and fat absorption.
This research aims to explore the potential synergetic effects of garlic, okra, and Stevia in a
convenient, functional candy format to offer an accessible, food-based intervention for
hyperlipidemia. By evaluating the anti-lipidemic efficacy of this blend, the study seeks to
provide evidence for an innovative, natural alternative to traditional lipid-lowering therapies,
promoting cardiovascular health through dietary means. According to the encyclopaedia Stevia,
a natural, non-caloric sweetener derived from the Stevia rebaudiana plant, has gained popularity
for its ability to sweeten without raising blood glucose or affecting lipid metabolism, making
metabolic disorder. Combining these three natural ingredients into candy offers novel and
palatable approach to lipid management, making it easier for individuals to incorporate into their
daily routines.

Theoretical Framework
This study integrates key concepts from herbal medicine and functional food
development to explore a novel intervention for hyperlipidemia. Garlic and okra hay their own
anti-lipidemic properties and it also combines with health benefits of the natural sweetener which
is the stevia. Support the hypothesis that a candy formulation could serve as the practical food
for the management of lipid. The synergistic effects of the combination of the ingredients offers
a promising natural alternative to traditional treatments for hyperlipidemia. Garlic and okra has
bioactive compounds with known health benefits. Garlic is rich in sulfur-containing compounds
like allicin which has been associated with lipid-lowering effects. Okra contains flavonoids,
polyphenols and soluble fibers which can help lower lipid levels by enhancing lipid metabolism
and reducing absorption of cholesterol. Stevia rebaudiana is a natural, zero-calorie sweetener,
which is beneficial for metabolic health and does not raise blood sugar level, making it an ideal
alternative to traditional sugars. The development of a functional food like candy infused with
these plant-based extracts is supported by the theory of nuctraceuticals, which are foods that
provide medicinal or health benefits beyond basic nutrition.
Statement of the Problem
This study concerns the increasing prevalence of hyperlipidemia, high cholesterol, and
hypertension, especially in individuals living in Purok Koti-koti, Brgy. Darubba, Quezon, Nueva
Vizcaya. Increased morbidity and death rates linked to cardiovascular illnesses result from the
fact that many people remain untreated or improperly managed despite advances in medical
understanding and treatment choices. The goal of this study is to identify the underlying causes
of the inadequate management and low level of awareness of these illnesses. Its specific goal is
2

to investigate how socioeconomic position, health literacy, and access to healthcare affect
people's capacity to identify and manage cardiovascular risk factors. Gaining an understanding of
these processes is important to creating focused treatments that can improve early diagnosis and
encourage efficient management techniques in this susceptible group.

Scope and Delimitation of the Study
This research focuses on the development and evaluation of a novel anti-lipidemic candy
formulation incorporating Allium sativum (garlic), Abelmoschus esculentus (okra), and Stevia
sugar as potential agents for lipid reduction. The study aims to investigate the optimal
formulation that maximizes the efficacy of these ingredients while ensuring appealing sensory
attributes, stability, and safety. Evaluation methods will include both in vitro anti-lipidemic
assays and comprehensive sensory analyses to assess taste, texture, and overall acceptability.

However, several limitations must be acknowledged. The findings from controlled laboratory
experiments may not be directly applicable to human subjects due to physiological differences.
The research will focus solely on the anti-lipidemic properties of the candy formulation,
excluding other potential health benefits. Additionally, the study will explore only one type of
candy formulation, which may limit the applicability of results to other food products. It will not
investigate the long-term health impacts or safety of regular consumption, nor will it examine
interactions with other medications or dietary supplements. Variability in the active compounds
of garlic, okra, and Stevia sugar due to factors like sourcing and processing may also influence
results. To ensure reliability, appropriate control groups will be incorporated into the study
design, and all procedures will adhere to ethical guidelines and regulations.
Significance of the Study
This study addresses a critical public health issue the rising prevalence of hyperlipidemia,
high cholesterol, and hypertension in specific demographic groups, particularly in Purok Koti-
koti, Darrubba, Quezon, and Nueva Vizcaya.
The result may be beneficial to the following individuals;
Residents of Purok Koti-koti, Brgy Darrubba, Quezon, Nueva Vizcaya. The study
specifically addresses the healthcare challenges faced by this community, particularly individuals
from lower socioeconomic backgrounds, those with limited health literacy, and those with
restricted access to healthcare. By pinpointing barriers to managing hyperlipidemia and high
cholesterol, the research will guide tailored interventions aimed at improving their health
outcomes. The local population stands to benefit from more accessible, targeted care that
addresses their unique needs.

REVIEW OF RELATED LITERATURE
This chapter is present the studies and literature that are related and relevant to the topic
presented in the study. This includes topics on the Efficacy of Garlic in hyperlipidemia, Potential
Dietary Medicine of Okra, the Stevia efficacy as natural sweetener and Hot-air dried Okra and
Garlic .
Efficacy of garlic in hyperlipiemia
According to (K'Alam, M'Nyeem, and A'Samad, 2018), garlic’s medicinal value is linked
to its lipid-lowering and anti-atherogenic properties. As a common ingredient in human diets,
garlic has shown promise in managing hyperlipidemia, as confirmed by various clinical trials.
Most research on the impact of oral garlic supplements on serum lipid levels has demonstrated
positive effects, particularly in cases of mild hyperlipidemia. However, these lipid-lowering
effects appear to be temporary, typically lasting only a few months. The review suggests that
more rigorously designed clinical trials are needed to better understand the scope and
consistency of garlic's hypolipidemic effects in clinical settings.
Stevia efficacy as natural sweetener
According to (R’Mathur 2024) The perennial herb Stevia rebaudiana is currently
produced all over the world as a very effective sugar substitute with no calories. The two main
steviol diterpene glycosides that give stevia its sweet flavor are stevioside and rebaudioside A.
These are widely distributed throughout the plant's leaves and provide human taste buds a taste
that is 150–450 times sweeter than sugar. In the culinary business, both processed sweet
glycosides and unrefined stevia preparations (having 50% glycosides) are widely used.

Numerous preclinical and clinical investigations indicate that because stevia herbal preparations
and its constituent parts are safe and have positive health effects, they may have pharmacological
and therapeutic applications. Stevia leaves contain a variety of compounds, such as fatty acids,
flavonoids, and glycosides, which all contribute to the plant's many health benefits. Stevia's
strong antibacterial, antioxidant, and immunomodulatory properties, as well as its capacity to
boost insulin production in diabetics, are derived from these constituents. To identify the
compounds in stevia that cause the benefits that have been observed and the molecular
mechanisms by which they function, more research is needed. Although the exact mechanisms
by which stevia, a natural sweetener, lowers food intake, it has also been demonstrated to lower
total cholesterol, triacylglycerols, and low-density lipoproteins.
Hot-air dried okra and garlic
According to (Kim k., Lee Y., Park S. 2023) Hot air dried garlic powder had higher
solubility compared to other drying methods. Hot air dried garlic powder demonstrated increased
vitamin B1 content, though results for inorganic contents were inconsistent. Analysis of physical
properties showed that Hot air dried garlic powder had high cohesion. These findings are
valuable for evaluating quality attributes in processed products made from garlic powder.
The common drying methods of garlic are hot-air drying, microwave drying, vacuum
drying and vacuum freeze drying. Different drying methods show different advantages and
drawbacks during the drying process. Hot-air drying, the most commonly used drying method in
the food industry, is more efficient and less operating costs (Ratti, 2001). This method also
maintains high storage stability of garlic slices. However, hot-air drying requires a long-time
processing, and high temperature leads to the deterioration in the quality of the final product. To
7

remediate these issues, garlic can be pretreated before drying to save energy and obtain high
quality product.
Okra: a natural solution for hyperlipidemia
According to [Amani & Salma, 2020] Okra extract effectively reduces cholesterol and
triglycerides without adverse effects. Okra shows promise as a natural dietary intervention for
managing hyperlipidemia. [Tang et al., 2019]
Okra promotes cholesterol degradation, reduces lipid synthesis, and binds to bile acids,
lowering cholesterol levels. [Wang et al., 2014]
Okra shows promise as a natural dietary intervention for managing hyperlipidemia.
Okra's potential for use in functional foods, such as supplements and candies, offers a convenient
way to incorporate its benefits into diets. [Xie et al., 2018]

RESEARCH METHODS AND PROCEDURES
Research Design
This study employs an experimental research design to evaluate the anti-lipidemic
properties of a candy formulation composed of Allium sativum (garlic), Abelmoschus esculentus
(okra), and Stevia rebaudiana (stevia). Experimental research is suitable for this investigation
because it involves manipulating variables (the candy formulation) and assessing its effects on
measurable outcomes such as lipid profile, blood sugar, and vital signs.
The research is designed to test the hypothesis that combining these three natural
ingredients into a functional candy can significantly improve lipid levels and overall
cardiovascular health. Participants will be divided into experimental and control groups, where:
The experimental group will receive the candy formulated with garlic, okra, and stevia.
The control group will not receive the candy or will receive a placebo candy without active
ingredients.
The study will assess the following parameters before and after the intervention:
Lipid profiles (e.g., cholesterol, triglycerides).
Random blood sugar levels.
Vital signs (e.g., blood pressure, heart rate).
Additionally, phytochemical analyses will be conducted to determine the bioactive
compounds present in garlic, okra, and stevia, further establishing their roles in lipid metabolism.

This experimental approach allows for a systematic evaluation of the formulation's efficacy in a
controlled setting, providing reliable data to support the use of natural, dietary-based
interventions for hyperlipidemia.
By incorporating structured experimental procedures, this design ensures the study’s
validity, reliability, and relevance in addressing hyperlipidemia through a novel, functional food-
based approach.
Research Setting
This study will be conducted in a rural setting with distinct socio-economic and
geographical characteristics that will influence both the research design and the interpretation of
the findings. The rural community under investigation presents unique challenges and
opportunities that must be carefully considered when analyzing healthcare access and delivery.
Respondents of the Study
Darubba, a barangay in Quezon, Nueva Vizcaya, offers a rich and promising environment
for research in fields such as agriculture, environmental studies, and socio-economic
development. This rural community is endowed with natural resources, a supportive and
experienced agricultural community, and distinct climatic conditions that create opportunities for
innovative studies and sustainable development initiatives. Among its sitios, Koti Koti emerges
as an ideal location for health-focused research, particularly concerning hyperlipidemia and
hypertension. These chronic conditions are increasingly prevalent in rural settings, where
lifestyle factors and limited healthcare access play significant roles.
10
ALDERSGATE COLLEGE INC.
School of Medical Sciences
This study specifically focuses on individuals aged 40-60 in Koti Koti, a demographic
that represents a critical population for understanding the intersection of rural living and the risk
of chronic health conditions. By examining their dietary habits, physical activity levels, and
access to healthcare, this research aims to design targeted, sustainable interventions that can
mitigate the impact of hyperlipidemia and hypertension without relying on medications.
Additionally, the study seeks to contribute to broader efforts in improving health outcomes and
preventing these conditions in rural communities across the Philippines.
Research Intrument
This study will primarily use structured interviews to gather in-depth qualitative data,
providing insights into the participants’ experiences, perceptions, and behaviors related to
healthcare access in a rural setting. Interviews are an effective method for exploring the complex
and personal factors that influence healthcare-seeking behavior, especially in communities where
healthcare resources are limited.
Interviews will be conducted in the preferred language of the participant (e.g., Ilocano,
Tagalog, or English) to ensure they are comfortable expressing their views clearly.
Each interview will last approximately 30 to 45 minutes and will be audio-recorded with
the participants' consent. The researcher will also take field notes during the interviews to
document nonverbal cues and contextual observations that may provide additional insight.
Data Gathering Procedures
In our experimental research process, the first step involves carefully selecting
respondents who meet the criteria necessary for the study. This selection process ensures that the
11
ALDERSGATE COLLEGE INC.
School of Medical Sciences
participants are relevant to the research topic, which in this case pertains to individuals who are
affected by hyperlipidemia and hypertension. After the respondents are chosen, the next step is to
conduct a one-on-one orientation session with each participant. During this session, we explain
in detail the nature of the study, including the purpose, procedures, and potential outcomes. This
is crucial for ensuring that the respondents fully understand what will be involved in the
research. As part of this process, we also provide each participant with an informed consent
form. The purpose of the consent form is to give participants the opportunity to review the
details of the study and decide whether they are willing to take part. The form explains their
rights, including their right to withdraw from the study at any time without any consequences.
Once the participants have had sufficient time to review the consent form and ask any questions,
they are asked to sign it, confirming their voluntary participation.
After the consent forms are signed, we proceed to the final step of the orientation, which
involves interviewing the participants. The interview questions focus on gathering information
about whether the participants are currently taking any medication for managing their
hyperlipidemia and hypertension. This step is crucial for understanding the context of their
health conditions and ensuring that the study’s findings are based on accurate and relevant data.
