HOW TO PURCHASE MOH INTERNATIONAL CLEARANCE FORMS
1. OPEN this link => [Link]
2. Create an ACCOUNT to get a USERNAME & PASSWORD to get access to the MOH PORTAL.
3. TO CREATE an ACCOUNT, click on APPLY NOW then provide your personal details
(NAME, NUMBER, EMAIL) to create an account.
4. After use your USERNAME (EMAIL) & PASSWORD from the SIGN UP to LOGIN.
5. After LOGGING IN, click on the *3 Parralel lines* above on your left side then
choose INTERNATIONAL CLEARANCE.
6. Then you select the type of network you want to use for payment via MOMO.
NB: To purchase you need to have ¢500 in that MOMO NETWORK you want to use.
7. After paying, a RECEIPT will be sent to your email for keeps to print out and
submit when submitting your CLEARANCE FORMS to MOH.
8. After paying and buying the forms successfully, click on INTERNATIONAL CLEARANCE
and choose APPLY NOW.
9. After clicking on APPLY NOW, fill the forms, save and print it out and submit it
to your CURRENT OR OLD FACILITY for endorsement but don't forget to write a LETTER
requesting for INTRODUCTORY LETTER from that FACILITY.
10. After signing it at your FACILITY plus taking your INTRODUCTORY LETTER, submit
it to your NURSING SCHOOL for ENDORSEMENT and return it finally back to MOH to be
endorsed and later forwarded to NURSING AND MIDWIFERY COUNCIL, GHANA (NMC) for the
actual verification.
