1st Reflective Log:

Introduction:
This reflection recounts my first clinical experience at Liaquat University
Hospital Hyderabad. I am Sameer Khaskheli, a BS Nursing student at Al Biruni
College of Nursing Hyderabad. This experience has prompted me to re-
examine and analyze my journey towards becoming a BSN. I realized that
experiencing the working environment firsthand is crucial for future nurses
and healthcare providers. Real-life experience is vital in learning.

Personal Feelings:
Initially, I felt nervous, if not scared, but my curiosity about the clinical setting
soon took over. I wondered what it would be like to care for unfamiliar
patients. However, my nervousness dissipated quickly. I was touched and
inspired by my "buddy mate's" compassion towards the residents of Civil
Hospital. She showed equal affection to everyone, caring for them as if they
were special to her. I was amazed by her selflessness towards strangers.

Personal Learning:
During my first day in the Gastro Department, I gained valuable practical
experience that differed from my prior knowledge. Our orientation with the
nurse manager of the Second Floor proved insightful. She shared information
about working in the Gastro Department and the long-term care facility's
system. Learning that she was an RN inspired me to succeed in my chosen
career.

After orientation, our group was divided, and we were assigned to specific
wings. Each of us was paired with a resident attendant and instructed to
observe and shadow them during the first three hours of their shift. Given my
experience in long-term care facilities, I was familiar with some procedures and
routines performed by Personal Support Workers (PSWs). Consequently, I felt
comfortable assisting the resident attendant/PSW in providing nursing care to
clients.

Although familiar with the setting, I still felt anxious about providing optimal
care. This experience taught me that being too comfortable and excited can be
detrimental, especially for students.

Conclusion:
I aim to apply the knowledge gained on my first day and continue exploring
opportunities to build a strong foundation for my future nursing career.
2nd Reflective Log:
This reflection recounts my second-day clinical experience at Liaquat
University Hospital Hyderabad. I am Sameer Khaskheli, a BS Nursing student at
Al Biruni College of Nursing Hyderabad. This experience has prompted me to
re-examine and analyze my journey towards becoming a BSN. I realized that
experiencing the working environment firsthand is crucial for future nurses
and healthcare providers. Real-life experience is vital in learning.

Personal Feelings:

Initially, I felt somewhat nervous, but my curiosity about the clinical setting
soon took over. However, this day my rotation was changed. I was assigned to
the Urology Department by my clinical instructor. I felt good and satisfied
about the rotation change.

Personal Learning:

During my first day in the Urology Department, I gained more valuable


practical experience than my last. The staff in the urology department was very
cooperative and encouraged me to engage in clinical practice. After this, I
checked patients' vitals and recorded them in their files. Following completion
of my previous tasks, I was assigned another duty by the RN, who instructed
me to administer medication to patients.

Conclusion:

I aim to apply the knowledge gained on my second day and continue exploring
opportunities to build a strong foundation for my future nursing career.
3rd Reflective Log:
Introduction:

I took vital signs of several patients in the urology ward. This role-play focused
on vital signs. Now, I will reflect on my role-play of TPR (Temperature, Pulse,
and Respiration), which was part of an assignment completed by me and my
two friends.

Personal Feelings:

When I played the nurse role, I first asked about the patient's condition, such
as "How are you today?" I maintained eye contact while speaking with the
patient. However, I found taking vital signs more challenging than expected,
especially when communicating with the patient. Verbal and nonverbal
communication is crucial between patients and nurses. Unfortunately, I forgot
to introduce myself to the patient, realizing it was my first mistake in
communicating effectively.

Personal Learning:

Before taking the patient's vital signs, I properly washed my hands using an
alcohol-based hand rub, a simple yet effective intervention in controlling
infections. I prepared everything needed, such as the patient's recording file,
pen, and instruments for taking vital signs. I verified the patient's identity by
checking their name, date of birth, MRN number, and doctor's name on their
wristband. Initially, I thought taking vital signs was straightforward, but my
experience proved otherwise.

Literature Review:

Vital signs encompass measuring a patient's temperature, pulse, respiration,


and blood pressure. These essential body functions include temperature, blood
pressure, pulse rate, and respiratory rate. Before taking vital signs, I asked for
the patient's permission. I started by taking the patient's temperature, pulse
rate, respiratory rate, and blood pressure, recording them in the patient's file.
However, when taking blood pressure, I forgot to support the patient's hand
with a pillow or my hand. I resolved not to repeat this mistake in my
profession.
Finally:

After completing the vital signs, I thanked the patient for their participation.

Suggestions:

During the role-play, I observed health professionals being polite and friendly
to patients. I noticed patience and encouragement from staff when
communicating with patients. Health professionals motivated patients to
speak, showing genuine interest in their words. This interaction impressed me,
teaching me how to motivate others. I'm glad I participated in this hospital
visit, demonstrating many positive qualities.
4th Reflective Log:
Introduction:

I received my clinical objectives from our clinical instructor, which focus on


utilizing the nursing process to provide care to sick adults in the hospital. My
objective is to achieve this goal.

Personal Feelings:

Initially, I felt apprehensive about applying the nursing process in real-world


scenarios. However, as I progressed, I became more confident in assessing
patients' needs, developing care plans, and implementing interventions.

1. Enhanced critical thinking skills through patient assessments


2. Effective communication with patients, families, and healthcare teams
3. Developed individualized care plans addressing specific patient needs

To enhance my nursing practice:

By achieving my clinical objectives, I have developed a strong foundation in


utilizing the nursing process to provide high-quality care to adult patients.
5th Reflective Log:
Introduction

This reflection recounts my experience administering IVs to patients in the


urology ward. As a nursing student, I aimed to develop my skills in this crucial
aspect of patient care.

Personal Feelings

Initially, I felt apprehensive about inserting IVs, fearing potential complications.


However, with guidance from experienced nurses, my confidence grew.

Personal Learning
1. Proper hand hygiene and sterile technique
2. Patient identification and consent
3. Selection of appropriate IV sites and equipment
4. Monitoring for potential complications

Literature Review
I learned Aseptic technique from my RN and staff, And also patient education
and consent. IV site selection and management and also learned about passing
IV with less pain.

06 Reflective Log:
Introduction

This reflection recounts my experience with pain management for patients in


the urology ward. As a nursing student, I aimed to develop my skills in this
crucial aspect of patient care.

Personal Feelings

Initially, I felt empathetic towards patients experiencing pain and anxious


about effectively managing their discomfort. However, with guidance from
experienced nurses, my confidence grew.

Personal Learning
I was learned these strategies from my ward health care providers.
