Appendix A – Routine Lifting Plan

▪ LOADING OF VARIOUS LIFTED EQUIPMENT FROM JETTY TO VESSEL


1. Lifted ob ject is inspected: container checklist is used as a guide.
2. Application of tagline (s) to the designated spot on lifted object by riggers.
3. Operator is signalled to hoist down and hooking of the link of the 4 legged wire rope sling assembly by riggers.
4. Riggers move away and operator is signal led to lift by Banksman.
5. Operator moves load to its final position and set load down with the aid of the Banksman.
6. Tag line (s) and link are detached from lifted ob ject and hook.

▪ LOADING OF VARIOUS LIFTED EQUIPMENT FROM VESSEL TO JETTY


1. Lifted equipment is inspected by supervisor.
2. Application of tagline (s) to the designated spot on lifted equipment by riggers.
3. Operator is signalled to hoist down and hooking of the link of the 4 legged wire rope sling assembly by riggers.
4. Riggers move away and operator is signalled to lift.
5. Operator moves load to its fina l position and set load down with the aid of the Banksman.
6. Tagline (s) and link are detached from lifted object and hook.

Physical and environmental consideration

Ground Conditions Are the outriggers evenly /adequately extended? ✓

Is lay down / landing area(s) adequate in both size and load bearing capacity? ✓

Obstacles Has route to be travelled been selected and cleared of obstructions? ✓

Lighting Is the lighting condition adequate to perform the operations safely? ✓

Demarcation Have barriers been positioned to prevent access to unauthorised personnel? ✓


Are the weather conditions within limits to avoid their effects on the safety of the lifting operation?
Environment ✓
Wind speed shall not excede 10 m/s (20kts) or a lower value (PIC/Crane Op evaluation)
IF ALL CONTROLS ARE NOT IN PLACE, THE OPERATION SHALL NOT START!
