LESSON 7 : PROGRAM IMPLEMENTATION

Teaching / Learning Objective:

After this lesson, the student will be able to:

1. define implementation;
2. discuss the importance of program implementation;
3. understand the components of implementation..

INTRODUCTION:

Program implementation refers to how well a proposed program or intervention is put into
practice and is fundamental to establishing the internal, external, construct, and statistical
conclusion validity of outcome evaluations.

The steps for implementing a program may seem straightforward: identify a need and provide the
service or product to a target population.

When programs are implemented poorly, it not only reduces the potential for helping, but it
wastes scarce public resources because poorly implemented programs are unlikely to be very
successful. In addition, when a program is implemented poorly, we don’t know whether or not it
works.

Implementation is important for all programs and increasing the quality of implementation
increases the chances that the program will yield its intended outcomes.

What is Implementation?

“Implementation refers to efforts designed to get evidence-based programs or practices of known

dimensions into use via effective change strategies”.

The gap between how a program is intended by its designers to be delivered and its actual
delivery in practice is referred to as “implementation variation”. Implementation may vary from
strict adherence to program protocols as designed to subtle or major changes in program
protocols. The challenge is to implement a program with sufficient quality to obtain the
outcomes found in original trials.

“When it comes to implementation, what is worth doing, is worth doing well.”

COMPONENTS OF IMPLEMENTATION

Two types of activities during the plan implementation phase

1. the preparatory and developmental activities

2. the actual delivery of health services

LESSON 7 : PROGRAM IMPLEMENTATION

It will then be necessary for the planners to prepare the necessary guidelines for the
preparatory and more detailed planning activities, and to see to it that the selected
developmental projects are planned and implemented.

Two parts of plan implementation

1. the drawing up of the plan of implementation

2. plan execution

Plan of Implementation

 This translates the health plan into operational terms.

 Its expected outputs are:
 legal authorization to implement
 approval of administrative instruments to institute changes (orders, memos,
SOP’s, etc.)
 defining targets and resource requirements by geographical areas;
 formulation and approval of project plans;
 monitoring and control system
 scheme for plan promotion

Plan execution

 Execution on the plan means the actual delivery of health services as provided in the
approved plan, the setting into motion of the formulated projects, the introduction of
minor changes, and the launching of actions designed to promote the approved plan.
 The processes of planning, organizing, directing, co-ordinating, controlling, financing
is put into action.

Monitoring and control systems are integral as they provide ongoing feedback during the execution phase, allowing for timely identification and correction of deviations from the plan. This ensures adherence to predefined objectives, optimizes resource use, and facilitates achieving the intended program outcomes by maintaining focus on strategic goals .

Challenges in the plan execution phase include dealing with unforeseen changes, resource misallocation, and non-compliance with planned protocols. These challenges can be addressed through flexible and adaptive management practices, ensuring effective coordination and communication among stakeholders, and maintaining robust monitoring systems to provide feedback and corrective measures .

This statement underscores the importance of high-quality implementation in health programs. It highlights that mediocre efforts in implementation can negate the benefits of evidence-based programs, leading to ineffective outcomes and wasted resources. Prioritizing quality in implementation ensures that programs achieve their full potential and intended health impacts .

Detailed planning activities are important because they provide a strategic roadmap for implementation, ensuring that all necessary guidelines, resource allocations, and coordination efforts are synchronized. This preparation minimizes risks of implementation variation and enhances the probability of achieving program success .

Legal authorization and the approval of administrative instruments are crucial in transitioning a health plan into an operational program. They provide the needed authority and framework for instituting changes, ensuring that program execution aligns with regulatory and organizational standards, which is essential for accountability and effectiveness .

Preparatory and developmental activities set the foundation for successful implementation by organizing the guidelines, selecting developmental projects, and planning details. These steps ensure that all necessary resources and strategies are in place for smooth execution, minimizing variations and maximizing the chances of achieving desired program outcomes .

'Implementation variation' refers to discrepancies between a program's intended delivery and actual execution. These variations can significantly impact outcomes, as they might alter the effectiveness of evidence-based practices, potentially leading to results that vary from those seen in initial trials .

The components of implementation, including preparatory activities and actual service delivery, are crucial for successful outcomes as they ensure that a program is effectively translated from plans to practical action. These components facilitate adherence to program protocols and adaptation to necessary changes, thereby increasing the likelihood of achieving intended outcomes .

Poor program implementation can reduce the effectiveness of helping target populations and lead to the waste of scarce resources. Ineffective implementation results in programs being unlikely to succeed, and it becomes unclear whether the programs work or not .

Defining targets and resource allocations specific to geographical areas ensures that the implementation is adequately tailored to the local needs, demographics, and logistical capacities. This targeted strategy helps optimize resource distribution and enhances program relevancy and efficacy in different community contexts .
