DAY 1

FEB 14, 2024

On my first day I wake up early as 7am I ride a bus to go on my Ojt at exact 8am In the
morning I arrived in city hall on my first day at the office of councilor Tanny Joe Calalay
they oriented me on what I will do in my on the job training after that I started My works
as the encoder of data in database on my On the job training I am encoding the anti rabies
for the month of Feb after I finish Encoding that anti rabies I . Data I can go home after I
encode all the dat exact 5pm I leave the office and go home.

DAY 2
FEB 15, 2024
On my second day I wake up as early as 7:30 am and I ride a city bus to go on my Ojt 7:58 I
arrived at the Quezon City Hall and I rushed in to go to legislative bldg to go to The office after
that anti rabies forms are waiting for me after I enter the office I started Open the PC and start
my work as encoder of the anti rabies data on the computer I edit the phase 1 of anti rabies data
of brgy Ramon Magsaysay after of hours Of editing the data on the database I turn off the PC
and ready my bag and belongings And wait for 5pm to leave the office at exact 5pm I quickly say
goodbye to my supervisor that I will go home and after that I ride a bus to go home.

DAY 3
FEB 16, 2024
On my third day I woke up again 7:30 to wash up and get ready to go on my Ojt at exact
8am I arrived in the Quezon City Hall and enter the office after that 9:30 am the Anti
rabies forms of brgy bungad arrived and after it arrived I start to open the PC and I
started to create a database and I encode the phase 1 of anti rabies of brgy bungad After I
create the database I enter data of anti rabiesof brgy bungad I close the PC and get ready
again my belongings to go home I say goodbye at my supervisor at 5pm and after that I
leave the office and ride a city bus to go home.
 DAY 4
FEB 19, 2024
On my fourth day I woke up again 7:30 to wash up and get ready to go on my Ojt at exact
8am I arrived at the Quezon City hall and entered the office after that I open the PC and
open the database because I need to create a report for some old data that I input to the
data base after that I shutdown the PC and leave the office at exact 5pm and go home.

DAY 5
FEB 20, 2024
On my fifth day I woke up again at 7:30 am to wash up and get ready to go to the office At
exact 8am I arrived at the office after that I always go to my working station to open the
PC and go ready for work after I prepare I open the PC and go to the data base to delete
some old file to give way to the data that I will input for tomorrow after I do my task for
that day I shutdown the PC and rest a little bit and get ready to go home at exact 5pm I
leave the office and go home.

DAY 6
FEB 21, 2024
On my sixth day I woke up again 7:30 and wash up and get ready to go to the office At
exact 8am again I arrived at the office I sit to my working station and open the PC That I
always used and go to data base to update the database inputs like dogs name owners
address because it’s always need to update time to time and if it’s update regularly it needs
to accurate so I always check it after of my task because my only task on the office is about
databased after that I will shutdown the PC and get ready to go home I rest a little bit and
get home after 5pm again I leave the office and go home .

DAY 7
FEB 22, 2024
On my seventh day I woke up again 7:30am I wash up and get ready to go to my Ojt at
exact 8am again I arrived at the office after I arrived I rest a little bit and after I rest I open
immediately the PC to do my task for that day because that day my task is I need to insert
new data of anti rabies to the data base and after I enter that I rest and get ready again to
go home I say goodbye to my supervisor because he was their that day that I will go home
at exact 5pm again I leave the office and go home.

DAY 8
FEB 23, 2024
On my eight day I woke up 7am early to cook my lunch and wash up and get ready to go to
the office I ride city bus so at exact 8am I arrived at the office and after that I rest for five
minutes after my rest I sit to my working station and start to open the PC I need to create
records this time using excel so I used excel after I create some back up data of the anti
rabies phase 1 of brgy Ramon Magsaysay district 1 after that I rest and rest and after of
rest because my task is already completed I Ready my belongings and at exact 5pm I leave
the office and go home.

DAY 9
FEB 26, 2024
On my ninth day I woke up 7:30 I wash up and ride a bus to go to the city hall 8am in
the morning I arrived at the city hall and I rushed in to the office so that I can start my
duties after I rush to go inside I immediately go to my working station and open the PC and
start to open the excel because I need to create and insert data of brgy damayan after of
hours creating and inserting data using excell I finally finish and after I finish I quickly
turn off the PC that I used and wait til 5pm I rest and get ready to leave at exact 5pm I say
goodbye to every staff there and leave the office and ride a bus from city hall to sm to go
home .

FEB 27, 2024
On my tenth day I woke up 6:30 I wash up cook my lunch and leave the house to go to the
office I arrived at the city hall 8am because it’s so traffic that day and after I arrived I
entered the office and sit to my working area to start my task for today because my task
that day is I need to update the all old records in the data base of the anti rabies so first I
open the pc and quickly go to the data base and after that I quickly check the data I need to
input to the data base so that I can update old records after I put all necessary data I turn
off the PC that I always used and I leave the office and go home.

DAY 11
FEB 28, 2024
On my eleventh day I woke up 7am I cook my lunch and quickly go to bathroom to wash
up and get ready to go to the office at exact 8am I arrived at the office I rest a little bit after
the rest I get I turn on the PC I always used and get to work I open the data base and I
delete the very old data of solicitations of brgy Ramon Magsaysay because it’s all of that
has been released already after do my task I quickly turn off the PC and get ready to go
home at exact 5pm I say goodbye to every one and I leave the office and ride a bus to go
home.

DAY 12
FEB 29, 2024
On my twelfth day I woke up again 7am cook my lunch for today and wash up and after
that I ride city bus and go to the city hall at exact 8am I arrived at the office and I just sit
again and start my task for that day I open the excel and create a budget report that will
be submitted by us to the committee and after working on that task I rest and after that I
turn off the PC and say good bye to everyone in the office and leave the office and go
home .

DAY 13
MARCH 01, 2024
On my thirteen day I woke up as sharp as 7am cook my lunch for that day 735 I wash up
ride a bus and go to the office at exact 8am again I arrived at the Quezon City hall and I
rushed in to go to the office so that I can start my Ojt their after I enter the office I greet
everyone a good morning and I started my work I just open the PC and I add some new
info of data in the database after finishing that task I rest and wait at exact 5pm I say
goodbye to every staff and leave the office and go home.

DAY 14
MARCH 04, 2024
On my fourteen day I woke up 7am cook my lunch very fast and go wash up ride a bus and
arrived at the city hall at exact 8am I enter the office and I greet everyone a good
morning after that I quick go to my working station and I open the PC to go to do my task
for that day which is I upload or insert some new data again of anti rabies in to the data
base after of hours working on that I rest a little bit and after I got the rest is I prepare to
leave the office at exact 5pm again I say goodbye to every staff and leave the office and go
home .

DAY 15
MARCH 05, 2024
I woke up at exact 7 am I cook my lunch for that day and go to the office at exact 8am I
arrived at the office and after arriving at the office I greet everyone a good morning after I
do that I start working by opening the PC I always used after I open the PC I just open the
database and I update the anti rabies of brgy bahay toro distric 1 Qc after of my task I turn
off the PC and greet everyone goodbye and after I greet Everyone I leave the office and go
home .

FEB 02, 2024
I woke up early because I need to attend some orientation on the main campus at exact
8:15am I arrived and listen to the orientation after the event I go home and get ready to my
Ojt for the coming days
