EVALUATE with RIPTEAR

The purpose of this step is to begin the evaluation process and lay the foundation for formulating a treatment
plan. Keep in mind, you may have already obtained some of this information in the previous steps.

R Risks of current use – gauge risk of overdose and withdrawal. Think more generally
about risk of trauma, including suicide.
note: may have obtained some of this information from Step 2 (The three C’s)

I Initiation – ask how old when started and with what substance; ask age when use
became regular and age they noticed it had become a problem; when was the
period of greatest use?
note: may have obtained some of this information from Step 1 (quantity/frequency)

P Patterns of use – understand how much, how often, and route of administration
note: may have obtained some of this information from Step 1 (quantity/frequency)

T Treatment attempts and outcomes – gather details regarding prior treatment for
substance use disorder and how patient did with treatment; what were the treatment
goals, were they met, and for how long?

E Effects of substance use – ask about medical & psychological effects, negative effects
note: may have obtained some of this information from Step 2 (The three C’s); these
points may be used to help enhance motivation

A Abstinence - describe periods of prior abstinence and duration; what helped to


achieve abstinence, what was going on in life then?

R Return to use prevention plan – discover how patient will achieve/maintain stability
note: may draw from prior treatment attempts and prior periods of abstinence
