called “the Declarant”) (give the name of the company, partnership or the sole proprietorship)
That the Declarant has obtained a contract/order from Bestway Cement Limited (hereinafter called “the Company”)

That I do hereby solemnly declare on oath that I or any other person/organization on behalf of the Declarant has not
paid or given or promised to pay or give any bribe, kickback, favour, gift or any other gratification or inducement of
any kind or description to any official of the Company or to any family member or friend of any official of the
Company nor to any agent or middleman for procuring the above described contract/order as well as other existing
contracts/orders between the Declarant and the Company.
That I do hereby solemnly declare on oath that I or any other person/organization on behalf of the Declarant has not
paid or given or promised to pay or give any bribe, kickback, favour, gift or any other gratification or inducement of
any kind or description to any official of the Company or any family member or friend of any official of the
Company nor to any agent or middleman for the continuance of the above described contract/order as well as other
existing contracts/orders between the Declarant and the Company.
That I further declare that (strike out as appropriate):
I am not related to or have close association, either personal or business, with any Director or member of
the management/supervisory/other staff of the Company.
I am related to/associated with Mr/Mrs/Ms.________________________________________________

nature of relationship or association) and I have already notified this fact, in writing, to the Board of
Directors of the Company.
That I further declare that if any official or any family member or friend of any official of the Company or any agent
or middleman demands any bribe, kickback, favour, gift, gratification or inducement for the award, continuance or
the renewal of the above described contract/order as well as other existing contracts/orders between the Declarant
and the Company then I shall immediately inform the Board of Directors of the Company in writing.
That I do hereby declare that if it is found by the Company that my above statements are untrue, then the Company
shall have the right to cancel the above described contract/order as well as other existing contracts/orders between
the Declarant and the Company with the further right of taking any other action as prescribed under law or deemed
fit by the Company including, but not limited to forfeiture of any moneys payable to the Declarant.
That I do hereby solemnly declare on oath that I am duly authorized to make this Declaration on behalf of the
Declarant and that this Declaration is legally binding on the Declarant (in case of a company or partnership, please
attach herewith a certified copy of the Board Resolution).
That I further declare that if at any time in future there is any change in circumstances which necessitates a revision
in any of my declarations above, I shall immediately notify the Company in writing.
That I acknowledge and unconditionally accept that this is a continuing Declaration and it shall also cover all
existing and future contracts/orders between the Declarant and the Company.
That I acknowledge and unconditionally accept that this Declaration is binding on the Declarant, its legal heirs,
lawful assignees and successors.
That I do hereby declare on oath that the above statements are true to the best of my knowledge and belief.
