Hi, good morning/afternoon, my name is _ and I’m calling from Affordable Healthcare, how are you

doing today?

Actually the reason I’m calling you is to let you know that, you may qualify for zero-dollar health
insurance plan that’s subsidized by the state, but before I tell you more,

1) May I know if you already have coverage like Medicare, Medicaid or any other government
insurance? (Answer has to be NO)

Okay then I will transfer you to my supervisor, they’ll give you all the information about this program,
but before I do,

2) may I know if you are between the age of 19-64? (Answer cannot be less than 19 and more than
64)
