ool 4: Healthy Workplace Improvement Tracking Tool

How to Use [Link] 4 – HW Itrack

1. Please refer to Health Workplace Improvement Tracking Form (HW-Itrack) Provide the
information needed in Column 1 and Column 2 of HW-Itrack. Please refer to the
information entered in Column A and Column B of the HWIP form for consistency.

2. Provide the information needed in Column 3 of HW-Itrack. Please refer to the information
entered in Column D of the HWIP form for consistency.

3. Column 4 of HW-Itrack should be consistent with the information entered in Column E of


HWIP.

4. Column 5 of HW-Itrack should indicate the actual output or accomplishment during the
period of monitoring (Column 3).

5. Column 6 of HW-Itrack is answerable by Yes or No. Indicate (Yes) if the expected output
is achieved and (No) if the expected output was/were not achieved.

6. Discuss with the Healthy Workplace Team the positive and negative factors that affected
the achievement of the output. (SWOT analysis)

a. If the Expected Output was achieved, discuss with the team what factors
contributed to attaining the desired output. There may be issues or areas of
concern that may potentially provide problems in future activities and formulate
preventive actions to address them.

b. If the Expected Output was not achieved, discuss with the team what factors
contributed to not attaining the expected output. Identify corrective actions to
address, mitigate or avoid the negative factors that will affect the achievement of
the output

c. List all the factors and prioritize them if feasible. List them in Column 7 (positive
factors) and Column 8 (negative factors) accordingly.

7. Review, analyze, and prioritize with the Healthy Workplace Team the positive (Column 7)
and negative factors (Column 8) and collectively formulate strategic activities that will be
needed to maximize the positive factor/s and /or address, mitigate or avoid the negative
factor/s that hinders the achievement of the output. List the said strategic activities in
Column 9 of HW-Itrack form. These strategic activities shall be included in the revised
Healthy Workplace Improvement Plan.
8. Please refer to the sample below.
