本校獎學金：
 博士生
(1) 入學第 1 學年至第 4 學年學雜費全免。
(2) 第 1 學年給予生活助學金，每人每月新臺幣 20,000 元
(3) 第 2 學年起，指導教授每月給予之助學金約新台幣 20,000 元。
(4) 須檢附英文能力達 IELTS 6.0(含)以上或具備等同程度之英文能力證明文件。

 碩士生
(1) 入學第 1 學年學雜費全免
(2) 指導教授每月給予之助學金約可達新台幣 10,000 元。

 於本校就讀滿一學期者，每學期可申請爭取外國學生獎學金，金額為每學期新台幣 10,000 -
50,000 元。

有下列各款情形之一，屬中華民國國籍：
一、 出生時父或母為中華民國國民（民國 69 年 2 月 9 日(含)前出生
者，僅以父親為準）
外國學生申請資格流程 二、 出生於父或母死亡後，其父或母死亡時為中華民國國民。
三、 出生於中華民國領域內，父母均無可考，或均無國籍者。
四、 歸化者。

※By the law of ＜Nationality Act＞Article 2：
A person shall have the nationality of the Republic of China (ROC) under any of the conditions provided by the following Subparagraphs:
1. His/her father or mother was a national of the ROC when he/she was born.
2. He/she was born after the death of his/her father or mother, and his/her father or mother was a national of the ROC at the time of death.
3. He/she was born in the territory of the ROC, and his/her parents can’t be ascertained or both were stateless persons.
4. He/she has undergone the naturalization process.

1. Attended overseas youth training courses organized by the Overseas Compatriot Affairs Council or technique training classes
accredited by the Ministry of Education (MOE)
2. Attended a Mandarin Chinese language center at a university/college of which foreign student recruitment is approved by MOE,
and to which the total length of stay is less than 2 years;
3. Exchange students, whose length of total exchange is less than 2 years; or
4. An Internship in Taiwan which has been approved by MOE, to which the total length of stay is less than 2 years.
 An individual, who has both foreign and R.O.C. nationalities and has applied for an annulment of their R.O.C. nationality before
February 1, 2011, will then be qualified to apply for admission as an international student and will not be subject to the limitation of
residing overseas continuously.

The applicant has not studied in Taiwan under the status of overseas Chinese student and has not received the student
status from the University Entrance Committee for Overseas Chinese Students in the year of enrolment.

Applicants have never been expelled by any university or college in Taiwan under these circumstances (with drop-
out records due to failure in conduct grades, academic grades, or being indicted guilty by the criminal law)

 Application for Undergraduate programs: graduation from secondary schools in a foreign country
 Application for Masters programs: graduation from universities with a Bachelor degree
 Application for PhD programs: graduation with a Master degree

International students applying for schools in Taiwan shall be limited to one application only. Upon completion of the course of
study, at a school in Taiwan, to which an international student has applied, the student's admission to another school's academic
level shall be handled in a manner identical to the admission procedures for local students. An exception is that an application
for a master's degree or higher levels of graduate studies can be processed under the procedures of each individual school.
※ Except for postgraduate programs, most of the courses in CMU are instructed in Chinese in undergraduate programs. Applicants
are expected to have sufficient listening, speaking, reading, and writing ability in Chinese language to apply for undergraduate

 申請文件 Required Materials/Documents
(1) 具結書
Declaration Form
如附件一，請於線上申請系統下載列印，完成簽名後掃描上傳。
Please see Attachment 1, the file of Declaration Form can be downloaded from Online
Application System, and the scanned file shall be uploaded after signing.

(2) 最高學歷畢業證明文件及成績單
Graduation Certificates and Diplomas & Transcripts of Completed Courses and
Grades
中、英文以外之語言，應附已公證之中文或英文譯本。
應屆畢業生未取得畢業證書者，可提供當學期在學證明書，須加蓋學校戳章。成績單應附
上班級排名及學制年限或學制起訖日期。
如獲得本校錄取，必須於入學前繳交經我國駐外使領館、代表處或辦事處驗證之文件。
Applicants must submit the graduation certificate and transcript of the highest degree
obtained. (A Chinese or English translation which is notarized by local authority is
required if the documents are not in Chinese or English)
Applicants shall submit the proof of enrollment officially issued by the
school/university if he/she has not graduated yet at the time of application.
The transcript should include the information of the applicant′s performance in class
expressed in number or percentage, and the program′s duration or the starting date
and completion date.
The graduation certificate and transcript shall be authenticated by Taiwan Embassy or
Taiwan representative office in/near the country before enrollment if the applicants
are admitted.

(3) 財力證明
Official Financial Statement
須由金融機構提出足夠在臺就學之財力證明（效期需為6個月內或包含近6個月紀錄且必須
顯示金額及幣值），最低額度為美金5,000元。中、英文以外之語言，應附已公證之中文或
英文譯本。
如為台灣獎學金獲獎生，須提供台灣獎學金證明書。
It must be submitted to prove the applicants′ ability to provide the needed funds for
enrollment with the balance on it. The minimum balance should be equivalent to U.S.
Dollar 5,000 or above. It should be issued by the bank officially with the information
of current balance and currency. The bank statement should be issued maximum 6
months before the time of application, or the last date of the financial record must be

within recent 6 months. (A Chinese or English translation which is notarized by local
authority is required if the documents are not in Chinese or English).

 A copy of Taiwan Scholarship Certificate is required if the applicant is a Taiwan
Scholarship recipient.

(4) 中文或英文就學計畫書

Two Letters of Recommendations
需由兩位推薦者提供推薦信，請於申請系統提供至少兩位推薦人資料(包含姓名，職稱，電
話及email)。
當您完成所有申請步驟並點選【確認送出】後，系統將自動產生電子郵件，並通知您的推
薦人於推薦人系統完成推薦程序。
You must provide at least TWO letters of recommendation, please provide the
contact information of the recommenders when you apply from online application
system. After you complete all application process, the system will automatically
send emails to the recommenders, and inform them to complete the process of
submitting recommendation letters.

授課語言以中文為主，申請者須繳交兩年內同等CEFR A2基礎級(含)或CEFR B1進階級
(含)以上之中文能力證明。華語文能力測驗對照表請參考附錄二。
申請者如符合以下條件，可提供相關證明，不須再繳交華語語言測驗證明。
• 母語為中文：
若您的母語為中文，請於自傳中詳述中文背景，說明平時使用中文的情況、學習的歷程
以及中文聽說讀寫的能力。
• 前一學位為中文授課：
若您先前任一學位(含高中畢業證書)為中文授課，請提供由學校正式開立的學位中文授課
證明。

  Applying for undergraduate programs:
Most of the courses are given in Chinese. The applicant must have the Chinese
Proficiency Certificate of CEFR Level A2 (Waystage) or Level B1 (Threshold) or above
(or equivalent), the certificate should be within two years. For the comparison of the
TOCFL, HSK, and CEFR please refer to Appendix 2.
Applicants that meet any one of the following criteria are exempted from providing
certificates of Chinese proficiency:
• Chinese (Mandarin) is your mother tongue If your mother tongue is Chinese, please
fill your autobiography in Chinese outlining your day-to-day use of the language,
learning progress, and your listening, reading, speaking, and writing skills.
• Your previous degree was conducted in Chinese (Mandarin) If any of your previous
degrees (including senior high school′s diploma) was conducted in Chinese
(Mandarin), please submit an official certification provided by your previous
university/high school.
• You have previously majored in Chinese (Mandarin) If any of your previous
degrees (including senior high school′s diploma) was of a Chinese (Mandarin) major,
please submit your graduation certificate or a certification provided by your
previous university/high school.

 申請碩士班及博士班： 所有碩士班及博士班提供外國學生全英語課程，申請者須繳交兩
年內相當於CEFR B2高階級 (含)以上之英文能力證明(例如TOEFL iBT 72, TOEFL ITP 543,
IELTS 5.5)。語言能力測驗對照表請參考附錄二。
申請者如符合以下條件，可提供相關證明，不須再繳交英文測驗。
• 國籍為英語系國家：
若您的國籍為官方語言或通用語言為英語之國家，則無需出具任何文件，官方語言及通
用語言為英語之國家請參考附錄三。
• 在英語系國家取得前一學位：
若您先前任一學位在英語系國家取得，請提供畢業證書作為證明。
• 前一學位為全英語授課：

All graduate programs provide English courses for international students.
Applicants must have an English proficiency certificate of CEFR Level B2 or above
(or equivalent), such as TOEFL iBT 72, TOEFL ITP 543, IELTS 5.5, or other equivalent
tests. For CERF conversion please refer to Appendix 2.

Applicants who hold a foreign nationality as well as an ROC nationality should submit
documents (such as Certificate of Entry and Exit Dates issued by National Immigration
Agency) showing residence abroad for at least 6 consecutive years (Applicants for the
undergraduate programs in School of Medicine, School of Dentistry, School of Chinese
Medicine and School of Post-Baccalaureate Chinese Medicine should submit
documents showing residence abroad for at least 8 consecutive years), and the ROC
ID number and date of birth of parents.

申請時具外國國籍，且曾具中華民國國籍並曾在臺設有戶籍者，須繳交經內政部許可喪失
中華民國國籍滿8年之證明。

(13) 越南及泰國籍人士向我國駐外館處申請赴臺灣留學簽證時需提交基本外語能力（華語或英
語）證明，印尼人士除了外語能力證明以外另須提供財力證明，請向駐印尼、越南及泰國
當地之臺北經濟文化辦事處確認其申請規定。
According to the student resident visa regulation for students with the nationality of
Indonesia, Vietnam and Thailand, applicants should submit the certificate of Chinese
or English proficiency test of certain level. Students with the nationality of Indonesia
should submit financial proof additionally. Please confirm with the Taipei Economic
and Cultural Office in Indonesia, Vietnam and Thailand for application requirements.

備取作業 Waitlist Procedure

正取生須於學費繳交期限內完成繳費註冊，逾期視同放棄。其缺額若學系有列備取生，則由備
取生依備取排名順序遞補。遞補名單及截止日期由本校另行通知備取生及公告。
Admitted students are required to pay the tuition and fees and complete the enrollment
by the deadline. Failure to complete the payment by the date indicated on our website
will lead to the suspension of your registration. In this case, CMU will notify the wait-
listed students based on the wait-listed order. An official announcement of result will be
released on CMU website.

重要入學須知 Important Admissions Reminders

(1) 依教育部規定，外國學生經入學學校以操行、學業成績不及格或因犯刑事案件經判刑確定
致遭退學者，不得向本校申請入學。違反規定經查證屬實者，取消其入學資格。
According to the Ministry of Education regulations, if you have ever been expelled by any
university or college in Taiwan under these circumstances (with drop-out records due to
failure in conduct grades, academic grades, or being indicted guilty by the criminal law),
you are not allowed to apply to this university. Violation of this regulation will result in the
cancellation of admission regardless of the merits of the application.

(2) 外國學生所繳入學證明文件有偽造、假借、塗改等情事，應撤銷錄取資格；已註冊入學者，
撤銷其學籍，且不發給任何相關學業證明；如畢業後始發現者，應由學校撤銷其畢業資格
14 並註銷其學位證書。

 registration, and will not be given any certificates. If it is found after their graduation, their
diplomas will be revoked and the qualification of graduation will be deprived.

(3) 外國學生來臺就學後，其於就學期間許可在臺初設戶籍登記、戶籍遷入登記、歸化或回復
中華民國國籍者，喪失外國學生身分，應予退學。
If international students apply for the initial household registration in Taiwan or acquire
naturalization or restoration of Taiwan Nationality during their period of study, their status
of foreign students will be denied and their student status will be discontinued.

(4) 禁止使用雙重身份（外國學生及僑生）申請入學：申請人若皆符合外國學生及僑生資格，
申請前即須確定以其中一種身份申請，禁止申請人以外國學生及僑生雙重身份申請來台就
讀。
No double identities (overseas Chinese student and international student): Individuals
should decide with which status they wish to apply from the very beginning, as the rules
are strict and there are no exceptions. Those who study in Taiwan with double identities
(overseas Chinese and international student) will have their recognized student status
revoked.

(5) 入學學士班前畢業年級相當於國內高級中等學校二年級之國外或香港澳門地區同級同類學
校畢業生，其畢業應修學分應增加12學分。但已畢業離校二年以上者，不在此限。
Undergraduate students whose high school graduating level is equivalent to Taiwan′s
high school grade 5 have to complete additional 12 credits at CMU before graduation.
However, those who have graduated from high schools for at least 2 years are exempted.

(6) 本 校大 學部教 學以 中文為 主， 申請就 讀 本 校者必 須具 備中文 聽、 說 、讀 、寫 能力。

所有研究所提供外國學生英語課程。
Most of the undergraduate courses are given in Chinese. International students are
required to have adequate Chinese reading, writing, listening and speaking ability. All
graduate programs provide English courses for international students.

(7) 申請入學最多可申請三個系所，但僅能選擇一系所辦理報到及註冊入學。
An international applicant cannot apply for more than three departments or institutes at
the same time. If accepted by the university, the student may choose only one program to
enroll in.

(8) 研究生可跨系所找符合資格的指導教授。
All graduate students can find the qualified thesis advisors from any departments
or institutes of CMU.

(9) 外國學生依教育部「外國學生來臺就學辦法」申請來臺就學，以一次為限。於完成原申請
就學之學程後，如擬繼續在臺就學者，其入學方式應與國內一般學生相同。但下列情形，
不在此限：
一、於完成申請就學學校學程後，申請碩士班以上學程，逕依本校規定辦理。
二、外國學生申請來臺就讀學士班以下學程，在國內停留未滿一年，因故退學或喪失學籍，
得重新申請來臺就學，並以一次為限。

外國學生經入學學校以操行或學業成績不及格、違反法令或校規情節嚴重致遭退學或喪失
學籍者，不得再申請入學。
An international student applying to study at an educational institution in Taiwan is limited

 to only applying once. If a student wants to continue studying in Taiwan, their application
shall be handled in the same manner as the admission procedures for domestic students.
However, this requirement does not apply to an international student in either of the
following circumstances:
1. If an international student is applying for admission to a master′s degree or higher level
program after completing the course of study at the educational institution to which
they originally applied, the university to which the student is now applying shall handle
the application in accordance with its regulations;
2. If an international student applied to come to Taiwan to undertake a bachelor′s degree
or lower level program in Taiwan and after coming to Taiwan stayed for less than one
year for some reason then discontinued their studies or forfeited their student status,
that student may lodge another application to come to Taiwan to study, but only one
such re-application is permitted.
If an educational institution in Taiwan where an international student was studying
considers that their conduct or academic performance was unsatisfactory, or if the student
seriously violated any ordinances or the regulations of the educational institution and the
circumstances were serious and as a result, in accordance with the provisions of its
regulations governing student awards and penalties had to discontinue their studies or
forfeited their status as a registered student, the student is not permitted to re-apply to
study in Taiwan on the basis of the provisions of the previous paragraph.

(10) 外國學生註冊時，應檢附六個月以上醫療及傷害保險或全民健康保險等相關保險證明文件。
如為國外所核發，應經駐外館處驗證。
International students are required to provide documents of health care & injury
insurance, National Health Insurance, or any other type of insurance that is valid for at
least ６ months beyond the date of registration. The proof of insurance issued in a foreign
country shall be verified by Overseas Agencies.

(11) 入學本校就讀之外國學生到校時，已逾該學年第一學期三分之一課程者，當學年不得入學。
但碩士班、博士班研究生經所屬系（所）主管同意者，得於第二學期註冊入學。
Admitted applicants arriving later than the one-third mark of the first semester are not
permitted to enroll for the academic year. Graduate students are allowed to enroll for the
second semester with the consent of the Director of the department or the Dean of the
graduate school.

住宿費：本校宿舍僅開放給大學部學士班學生。學士班(學士後中醫學系以外)一年級外國新生可優先獲得
學校宿舍安排，北港宿舍為4人一間，住宿費約新台幣8,300元(每人/每學期)。水湳宿舍為2~4人一間，住
宿費約新台幣22,700~25,700元(每人/每學期)，相關收費標準及住宿規定請依學務處公告為準。
其他費用：生活費每個月平均大約新台幣6,000至10,000元，得視每人的生活方式而有所不同。書本費依
照所修習的課程與出版商的訂價而有不同。

Accommodation Fees ： CMU dormitory places are available to undergraduate students only
(School of Post-Baccalaureate Chinese Medicine is not included). New international
undergraduate students have first priority for on-campus accommodation. For Beigang dormitory,
the room is shared with 4 people and the rent is about NT$. 8,300 per person per semester. For
Shui-nan campus, the room is shared with 2~4 people and the rent is about NT$. 22,700~25,700
per person per semester. The regulations and fee rates for CMU dormitory is subject to the
announcement of Office of Student Affairs.
Living Expenses：The average living expenses range from NT$ 6,000 to NT$ 10,000 per month.
This figure may vary individually. Costs of textbooks are at one′s own expenses and can vary from
program to program.

(14) 本校大學部招收外國學生轉學生，經由每年所舉辦之「寒假轉學考」及「私立醫學校院聯
合招考轉學生」考試入學，但外國學生經入學學校以操行不及格或因刑事案件經判刑確定
致遭退學者，不得轉學進入本校就讀。碩博士班不招收外國學生轉學生。
International students transfer to undergraduate programs should apply to attend the
Transfer Exams of Spring Semester held by CMU and the Joint Transfer Exam held by the
Committee of private medical universities. However, international students whose student
status is discontinued by the University because of misconduct, poor academic results or
criminal offences, are not allowed to apply for transferring to CMU. Postgraduate students
are not allowed to transfer to CMU.

 學士班：僅開放秋季班招生；授課語言以中文為主。
 碩士班及博士班：開放秋季班及春季班招生；所有研究所提供外國學生英文課程。研究生可跨系所找符合
資格的指導教授。
 Undergraduate programs: only accept fall intake and most of the courses are given in Chinese.
 Master and Doctoral programs: accept both fall and spring intakes. All graduate programs provide English courses
for international students. All graduate students can find the qualified thesis advisors from any departments or
institutes of CMU.

All the documents I have provided (including diploma, passport, and other relevant documents
whether original or copy) are legal and valid documents. Should any of the documents be found
to be altered or to violate any university regulation, I will lose my CMU admission privilege and
no proof of attendance or credits completed will be issued by the University.
五、 本人所提供之最高學歷證明(申請大學部者提出高中畢業證書、申請碩士班者提出大學畢業證書、申請
博士班者提出碩士畢業證書)，在畢業學校所在地國家均為合法有效取得畢業資格，並所持有之證件相

The proof of the highest level of education attained that I have provided (for bachelor′s degree
applicants -your high school diploma; for master′s degree applicants – your bachelor′s degree
diploma; and for Ph.D. degree applicants, your master′s degree diploma) is valid and legally
recognized by the country in which I received such education. The diploma provided is equivalent
to that degree which is awarded by certified schools in Taiwan. If this is found to be fraudulent
(including those through alterations and forgery) and in violation of any regulation, my admission
will be canceled, my student status will be revoked, and no transcript or diploma will be issued.
六、 本人未曾遭中華民國各大專院校以操行、學業成績不及格或因犯刑事案件經判刑確定致遭退學。如違
反此規定並經查證屬實者，本人同意取消入學資格並註銷學籍。
I have never been expelled from any university or college in Taiwan under these circumstances
(with drop-out records due to failure in conduct grades, academic grades, or being indicted guilty
by the criminal law). Should I breach any of the regulations, I will be denied admission and my
student status will be revoked.
七、 本人若於就學期間在臺初設戶籍登記、戶籍遷入登記、歸化或回復中華民國國籍者，以致喪失外國學
生身分，本人接受校方退學之處分。
I will accept the discontinuation of my student status by the University if I apply for the initial
household registration in Taiwan or acquire naturalization or restoration of Taiwan Nationality
during their period of study, and thus lose my status of foreign student.
八、 本人了解並保證符合外國學生申請來臺就學，以一次為限之規定。於完成申請就學學校學程後，除申
請碩士班以上學程，如繼續在臺就學者，其入學方式應與我國內一般學生相同。
I hereby certify that I qualify for the regulation that international students applying for schools in
Taiwan shall be limited to one application only. Upon completion of the course of study, at a
school in Taiwan, to which an international student has applied, the student′s admission to
another school ′ s academic level shall be handled in a manner identical to the admission
procedures for local students. An exception is that an application for a master′s degree or higher
levels of graduate studies can be processed under the procedures of each individual school.

 In compliance with the regulations stipulated in Article 8 and Article 9 of Personal Information Protection Act, the
university (China Medical University) hereby issues the statements concerning its obligation in duly notifying you
when it collects, processes, and uses your personal information. (If you are under the age of 20, your legal
guardian or parent must review and understand and agree to the terms of this statement and its amendments.)
1. Name of Institution: Chine Medical University (“the University” or “CMU”).
2. Purpose of Personal Information Collection
The University, in compliance with the law, may need to collect, process, and use your personal information for
the purpose of executing affairs related to the following events or activities: Examination, civil service, civil service
protection & training (134), Information (Communication) Services (135), Information (Communication) and
database Management (136), Investigation, statistics and research analysis (157), Student data administration

5. Retention Period, Available Area, User, and Format of Collected Personal Information
(1) Time Period: Applicants’ information, including examination score, is to be used only for the stated
purpose, unless otherwise instructed by authorized government agencies.
(2) Available Area: All collected personal information are for use in Taiwan (including Penghu, Kinmen, Matsu,
and other offshore areas of Taiwan).
(3) User: Users of China Medical University, Ministry of Education, or other National Competent Authorities for
Business Objectives. Any amendments for the users will be announced on the website of Office of

(4) Format: Notifications for applicants on exam matters, test scores, admissions, registration, and verification;
contact information of applicant, parents, guardian or contact person; academic research or other required
information.
6. For applicants, missing personal information could present risk of not completing the registration, hindering
notification of emergency matter, or failing to receive test scores.
7. The applicant has the following rights towards his/her own personal information:
(1) request, browse, update, and correction;
(2) request for photo copies;
(3) request for stop of collecting, processing, using or removal of personal information.
Details of how to exercise such request can be obtained through Admission Guideline.
8. Each applicant is responsible for the accuracy of all information provided. Should correction or update be
needed, proof of authentication is required when submitting to the Office of Admissions for correction.
9. Upon request from authorized government agencies or law enforcement agencies, CMU will provide student
