Introduction
UNISE is a multi-sectoral nutrition score- card data management tool implemented using DHIS2 to capture, manage and monitor multi-sectoral
nutrition data at all administrative level. This platform was developed by Federal Ministry of Health (FMOH) mainly MCH directorate in
collaboration with UNICEF and currently it is integrated to the national District Health Information System (DHIS 2) system.

Soqota Declaration (SD) is the government’s special commitment declared to end stunting in children under two years old by 2030. To achieve
this goal, Nutrition data revolution is one of the key approaches that the declaration is envisioning to accomplish. As a result, the declaration
took the lead on the implementation of the UNISE platform for the national multi-sectoral nutrition data management. UNISE will be the main
data management tool for the multi-sectoral nutrition score card under implementation.

UNISE consists of the nutrition specific and nutrition sensitive indicators. The nutrition specific indicators are mainly reported by the health
posts, health centers, clinics and hospitals and these reports are organized by the health bureaus at all levels. Nutrition sensitive indicators
currently collected from six NNPII signatory sectors at all administrative levels starting from kebele. These sectors are Health, Agriculture,
Women and children Affair, Social and Labor Affair, Water Irrigation and Energy (WIE) and Education sector.

Objective of this document

UNISE is currently integrated in the FMoH DHIS2 system which is mainly designed and implemented based on the old HMIS structure of the
country. The document discusses the following main points which are useful to understand and use UNISE/DHIS2 properly:

• The Organization Unit naming and hierarchy of UNISE
 UNISE Indicator Definition
Indicator definition of the sectors in UNISE follows the exact data definition of the DHIS2 platform. A data value or a data captured in
UNISE/DHIS 2 needs at least three dimensions that answer three questions: the WHERE, the WHEN and the WHAT. In DHIS2 the WHERE
dimension is called the Organization Unit which shows the administrative location and hierarchy levels of the country. The WHEN dimension
shows the frequency of the data collection, and in DHIS2 is called Period. The WHAT dimension show the actual data captured from the source
and in DHIS2 is called Data Element

The following section describes the list of data elements and indicators and their descriptions defined by the current NNPII signatory sectors
implemented in UNSIE

Health Sector
The health sector has identified the following list of indicators at Kebele level and woreda level. These indicators are nutrition sensitive indicator
they are different from the nutrition specific that were collected suing the health’s HMIS system for the last 20 years.

UNISE/DHIS2 Organization Unit naming

UNISE is a multisectoral data management tool, all the NNPII signatory sectors will use this tool as their data management for their work and
improved decision making system. It is fully integrated with DHIS2 system, as the NNPII coordination body is led by Health sector as well as many
of the UNISE indicators mainly the nutrition specific are captured from the health sector.

UNISE/DHIS2 Organization Unit defines the organization structure of DHIS2, for example how geographical areas, administrative areas, health
facilities, and other are arranged with respect to each other.

Example in Ethiopia, administrative areas hierarchies are: Federal, Region, Zone, Woreda, Kebele. Health facilities such as health centers (HCs),
health posts (HPs), hospitals also can be defined in DHIS2 as organization units.
Since the FMoH DHIS2 is implemented and configured based on the old HMIS pattern, the organization unit naming is not followed the standard
naming of the geographic administrative area of the country however the levels, locations as well as spatial coordinators are the same as the
actual location of the places.

UNISE/DHIS2 Organization Unit naming Guide

In the national DHIS2 system implemented by the FMoH, the organization unit names have a health specific concatenation, which results in
minimizing the readability and usability of the system to other sectors. This guide helps to understand the naming convention of the DHIS2 for
the organization units. The following table shows the Organization unit hierarchy names and actual location

UNISE Data Entry and Data Output Guide – Nutrition Sensitive

The routine data entry App is where aggregated data is manually registered in the UNISE/DHIS2 database. Data is registered for an organization
unit, a period, and a set of data elements (data set) at a time. A data set often corresponds to a paper-based data collection tool.

In order to enter data using the routine data entry app, sectors need to select organization unit per the location of the data source.

To enter Kebele level data, you need to select the Health post org unit. Each health post of the country can be mapped to the Kebele
administrative level of the country. More than 98 % of the kebeles in the country mapped to the health post organization unit definition in the
UNISE/DHIS2,however there are some kebele that doesn’t have health post as well as there are kebeles that have more than one health posts.
For these kebeles that have two health posts in it, choose the name of the health post named by the kebele name and enter the data in it. For
the kebeles that doesn’t have health post organization unit, please recorded them and it can be added in to the UNISE/DHIS2 system. To enter
woreda data you need to select the organization unit of the woreda name followed by WrHo word. The same apply also to enter data at zonal
regional and federal level.

In the data output or reporting side of the system also you need to select the corresponding health post (kebele name) to report kebele name,
woreda followed by WrHo for woreda report and will continue in a similar fashion for the above levels too.

UNISE Data Entry and Data Output Guide – Nutrition Specific

The nutrition specific part of UNISE entered as the usual pattern of the HMIS. The nutrition specific data can be collected at health post, health
center, clinics, hospitals and any point that provides health service based on the FMoH service provide centers. These data can be aggregated by
kebele, woreda , zone , region and federal level to generate report
