Instruments Used in

Ear Surgery
Myringotome
• It is a lancet-type angled instrument used for
myringotomy. Alternatively, a sickle knife used in
myringoplasty can also be used to perform
myringotomy.
Mollison's mastoid retractor

• Used in mastoidectomy to retract soft tissues


after incision and elevation of flaps. It is self-
retaining and hemostatic.
Jansen's self-retaining mastoid retractor

• Used in mastoidectomy, similar to Mollison's


retractor.
Lempert's endaural retractor
• Used for endaural approach to ear surgery. It has two
lateral blades which retract the flaps and a third central
blade with holes. The central blade retracts the temporalis
muscle. The central blade can be fixed to the body of the
retractor by its hole.
Lempert's endaural speculum.


It is like the Vienna model nasal speculum but
curved. It is used to spread open the meatus when
giving local injection or making an endaural incision.
Mastoid gouge

• Used to remove bone in mastoid surgery. Various


sizes are available. However, it is not used now. A
drill is preferred to gauges.
Lempert’s curette (scoop)

• Used for removal of bony septa and granulations


in mastoid surgery.
MacEwen’s curette and cell seeker

• Used in mastoid surgery to explore the air cells


with one end, and to curette the intervening septa
and granulations with the other.
Farabeuf’s Periosteal Elevator

• Used for elevation of periosteum from the mastoid


cortex in mastoidectomy.

THROAT INSTRUMENTS
Boyle-Davis mouth gag
• It has two components: Boyle blade and Davis gag
that are used simultaneously.
• Opening the mouth and retracting the tongue
anteriorly.
• Tongue blades of various sizes can be
interchanged according to the age of the
patient.
• Used for various operations on the oral cavity
(palate surgery), oropharynx (tonsillectomy,
surgery of soft palate, pharyngoplasty), and
nasopharynx (adenoidectomy, excision of
angiofibroma).
Doyen’s mouth gag & Jenning’s mouth
gag

• Used to keep the mouth open for intraoral


surgery, retraction of the tongue is not
required.
• Mostly used for tongue surgery.
• It is applied on one side of the mouth on
molar teeth.
Draffin’s bipod
• Each pod has four rings.
• They can be assembled to vary the height
at which the tongue blade of the Boyle
Davis mouth gag can be suspended.
• Lower end of each pod can be placed in
one of the several depressions in
Magauran’s plate.

Denis browne tonsil holding forceps
• Used to hold the tonsil and pull it
medially during the process of
dissection.
Waugh’s tonsil dissection forceps
• For incision in mucous membrane
and dissection of tonsil.
• Single tooth of the forceps is
used to incise the anterior pillar.
• There is a stud in one of the
arms that can be felt through a
vent in the opposite arm. This is
useful to note the pressure
exerted at the tip
Yankauer suction tube
• Used for suction in tonsillectomy and other oral or oropharyngeal operations.
Mollison Tonsil dissector and anterior
pillar retractor
• Blunt end is used to dissect the tonsil and,
• Other to retract the anterior pillar to inspect the fossa for any
bleeding point.
Birkett straight artery forceps
• Used to catch bleeding points in the tonsillar fossa after
tonsillectomy, has transverse serrations.
• It is replaced by Negus second artery forceps underneath
the first artery forceps before ligation.
Negus second curved artery forceps
• It has a curved tip and is used after the first artery
forceps for ligating blood vessels in a deep site.
Negus knot tier and ligature pusher
• It is used to push the ligature loop on the Negus
second artery forceps to ligate the bleeding point.
Eve’s tonsillar snare
• Used for tonsillectomy. After the tonsil has been dissected
till its lower pole, snare is passed round the tonsil to
engage the pedicle and then firmly closed.
• It crushes and cuts the pedicle thereby minimizing bleeding.
• Wire used in snare is 25 gauge thick.
St. Clair Thompson’s adenoid
curette with guard
• This instrument is used to curette
the adenoids by a blind technique.
• Curette is introduced behind the
soft palate with the blade facing
down. It is held like a dagger and
the adenoid is curetted from the
nasopharyngeal wall in the midline by
sweeping movement.
Tonsil and adenoid microdebrider
• Electronic device, helps in easy and quick
debridement of adenoid or tonsillar tissue.
• Has a sharp blade rotating high rpm, sloughs off
and removes tissue by suction action.
Larynx and trachea

Tracheal hook {blunt

Used in tracheostomy .
Blunt tracheal hook:- is
used to retract thyroid
isthmus upward and
downward to expose the
trachea.
Sharp tracheal hook :- is
applied to lower border of
cricoid cartilage to stabilize
the trachea and to prevent
its movements during
respiration when making
incision in the tracheal wall.
Tracheal
dilator
• Used to keep the cut tracheal
edges open after incision in the
trachea so that tracheostomy tube
can be easily inserted . A curved
artery forceps can be easily used in
place of a tracheal dilator.

o Used for direct
laryngoscopy.
o It has single or twin light
carrier which can be
connected to a cold light
source through a flexible
cable.
o Older models have a single
electric bulb which works on
batteries or a transformer.
o Size of laryngoscope used will
vary with the age of the
patient.

Allis clamp
Use:-
The Allis clamp is a surgical
instrument with sharp teeth,
Used to hold or grasp heavy
tissue. It is also used to grasp

breast or bowl tissue
Kocher Hemostatic Forceps

Use:-
Kocher Hemostatic Forceps are
used to clamp massive tissues
and avoid slippage when
surgeons try to grip arteries.
Mosquito forceps

Use:-
Mosquito forceps is use for
handling blood vessels
and controlling bleeding
during surgical procedure
Non toothed forceps

Use:-

used in tracheostomy to
hold delicate structures
and superficial tissues.

spaces of the operating sites.

deliver oxygen to the lungs if
you’re unable to breathe normally
after an injury or accident or
because of muscle weakness.
