To Whom It May Concern:

Jason M OBrien was admitted into the hospital on 07/03/2023. He arrived via ambulance due to his current medical
condition. Jason was seen in the ER for complaints of migraines, memory loss, fainting, and loss of appetite. Patient
was then admitted due to abnormal lab results and an abnormal Computed Tomography scan (CT SCAN) of brain.
Jason has had prior surgeries of his brain that stemmed from his TBI back in 2005. Due to his abnormal lab results
he was then sent to surgery where they then relieved pressure off of his brain. His surgery went well, the patient is
healing properly, and incision looks clean. Due to his ongoing swelling, and the TBI that patient OBrien sustained
during combat he will struggle with short term memory loss, dates, numbers, times. With time some memory might
be regained. The patient was given proper aftercare instructions and is to return for a follow up appointment on
08/01/2023. So please excuse Jason for the time he was admitted and, in my care, as well as 2 weeks post-surgery
