I, LEA A. GALLEGOS of CHEERS TRADESTAR INC., of legal age, Filipino,


and a resident of Purok 6, Villa Gonzaga, City of Santiago, Isabela after having been sworn to
in accordance with the law, depose and say:
1. That, I am the registered owner of a motor vehicle registered with the Land
Transportation Office (LTO) specifically described as follows:

MAKE : ISUZU
YEAR MODEL : 1988
ENGINE NUMBER : 4HF1186238

2. That, I caused and changed the Body Type of the aforesaid motor vehicle from
ALUMINUM VAN to DROPSIDE;

3. That, I intend to register the motor vehicle and hereby humbly request the Land
Transportation Office (LTO) to amend the motor vehicle registration and thereby
reflecting its new Body Type which is ALUMINUM VAN to DROPSIDE;
