would register the client at reception, as this is your partner care for
educational purposes you would document the following. Client registered
for educational purposes as client for partner care Semester II.
At this point you will obtain a hard tissue form and begin the hard tissue.

October 20, 2023 - Confirmed appointment with client via text or
verbally on October 19, 2023. If this was not done do not write
it, Client arrived on time for the appointment, Completed the online
COVID and COVID form., no concerns noted. Updated medical and
dental history no changes to medical history and no dental concerns
noted by the client. ( dental concerns may be new sensitivities or
fracture). Document your sterilization data, Load, Cl IV and student
that did the wrapping. Inform client of what you are doing today
Obtained informed consent to proceed with screening completion and
or hard tissue, document what your are starting with.
Sign up for instructor to sign you in.
Completed screening and note the findings above. Reviewed by
instructor. Instructor will indicate that the client is accepted as a
DD1 client. At this time you would register your client and have the
client pay $30 at the dental reception area with Ms. Kamilla, as we are
doing client care you will note that there is no fee charged as the
client is for educational purposes, partner care for Clinic II Semester
II. you will explain the next process in care that you will be
completing hard tissue. Obtained informed consent to proceed with
the hard tissue exam. Hard tissue completed and reviewed by
instructor.
Review the COVID screening, review the medical history and then proceed
to complete your documentation. If you have not finished the screening or
had the screening reviewed by an instructor you will need to continue the
notes on the screening form.
5. If you have completed the screening and had it reviewed by the instructor
you may use a white progress sheet to continue with your notes.
6. Notes completed SIGN UP FOR A SIGN IN - you cannot go intra oral until
you have had a SIGN IN from an instructor.
7. At sign in from the instructor they will guide you to the next steps, which
may completing the screening and or moving on to hard tissue ( the whole
mouth)
8. Reminder to bring your notes in plastic sheets and in a binder, organized
and ready to use at chairside.
9. Review your hard tissue sequence: a) Missing teeth in all quadrants,
unerupted wisdom teeth? ask the client have you had your wisdom teeth
removed? if not documented in blue
they may be unerupted. If extracted they are missing and boxed
b). Abfraction,
Abrasion, Attrition, Erosion, rotations/tipping
c) open or loose
contacts
d) percussion or
mobilities
e) restorations,
crowns, bridges, implants, suspected caries in green
f) Occlusion
Complete the entire hard tissue form: when completed Sign up for an
instructor to review.
10. While waiting for the hard tissue review go on to complete the caries risk
diet analysis, do not worry about completing the analysis component do the
best you can with the information you have been given. Be detailed in the
diet, white rice or brown rice, flavoured water, soda water etc.
11. Continue with your home care form.
12. Then if there is time you will continue with the Extra Oral Exam, if
students are at this point a demo will be completed for each row by the
instructor.
THOSE ON DUTY PRE - MUST ARRIVE AT 8:00 TO ASSIST JAIMIE WITH THE
TRAY SET UPS AND OR WRAPPING INSTRUEMENTS, AT 1:00 PM FOR THE
AFTERNOON CLINIC.
THOSE ON POST DUTY - MUST BE IN THE DISPENSARY TO ASSIST JAIMIE
WITH THE STERILIZATION AT 11:30 AM AND FOR THE PM AT 4:30 PM
