Division, Karnataka state Worked as a Mechanic, hereby declare that, for the
period from 14-04-1992 to 19-12-2020, the contribution for Provident Fund
was made by deducting from my salary.

I am a member under the EPS, 1995 with effect from enrolled by employer
M/s. KSRTC Kanakapura DEPO, Ramanagara division, Karnataka and
hereby exercise option to contribute on actual salary exceeding statutory wage
ceiling and to allow benefits of actual salary in Employees' Pension Scheme
1995 from 16.11.1995 as Employees' Pension Scheme member.

I, hereby, undertake to pay to EPFO, the due differential amount of higher


Pension, as per Supreme Court's direction
