A resolution of the Supreme Student Government is submitted in order for the STNHS
Student Leaders to have Student Leaders Quarterly Performance Evaluation.
WHEREAS, the Supreme Student Government officers of Sto. Tomas National High School
being one of the frontline will have a Quarterly Performance Evaluation.
WHEREAS, Student Leaders Quarterly Performance Evaluation is a way to teach the
student leaders develop and maintain a school environment designed to maximize learning
potential.
WHEREAS, the purpose Student Leaders Quarterly Performance Evaluation
Is to reinforce strengths and overcome areas of weakness.
WHEREAS, Student Leaders Quarterly Performance Evaluation primarily means to have
dependability or degree of supervision needed to carry out task to completion
WHEREAS, the conduct of Student Leaders Quarterly Performance Evaluation is helping
the student leaders to have the ability to adjust to change in environment, job assignment,
methods and personnel.
WHEREAS, the conduct of Student Leaders Quarterly Performance Evaluation is helping the
student leaders to have the ability to communicate with peers, to earn respect, and
confidence. To have job knowledge, innovative ability, good judgment, action oriented
WHEREAS, student leaders will have extent to which work is accurate, thorough, effective
and on-time.
NOW, THEREFORE, BE IT RESSOLVED THAT THE Supreme Student Government of Sto.
Tomas National High School does hereby suggest the approval of the Student Leaders
Performance Evaluation per Quarter.
BE IT FURTHERE RESOLVED THAT THE Supreme Student Government of Sto. Tomas
National High School does hereby pledge its full support for the implementation of Student

III. RATIONALE:
In reference to DO 49, s. 2011”Mandated Programs, projects and Activities of the
Supreme Student Government (SSG), the SSG is the foremost co-curricular student
organization authorized to operate and implement pertinent programs, projects and activities
in school nationwide. It lays the groundwork for good governance, volunteerism, unity and
cooperation by providing the students various venues where the can improve their leadership
knowledge, skills and attitudes.
It trains students to become better members of society in accordance with the ideals
and principles of participatory democracy and good citizenship. More importantly, it helps the
Department of Education (DEPED) achieve its trust as indicated in the Education for all EFA
Goals, Millennium Development Goals (MDGs) and the Basic Education Social Reform
Agenda (BESRA).
In order to synchronize its program, projects and activities (PPAs), one of the following
to be implemented by the SSG in the community is the Involvement in the Community:
Outreach Program . Thus, Insupport to the implementation, the Supreme Student
Government in cooperation with THUDONGMI Foundation as the project sponsor will conduct
Community Outreach at Dakias Elementary School, Tuel Tublay, Benguet.
IV. OBJECTIVES
GENERAL OBJECTIVES: Conduct Community Outreach to help the students develop a
sense of responsibility for their own actions.
SPECIFIC OBJECTIVES:
To teach the 53 Kindergarteen to Grade 6 pupils on Personal Hygiene like“how
to brush your teeth, how to comb your hair, how to wash your hand”.
To bring joy to Filipino children and to be a reminder that whatever situation they
may be in, there will always be someone that is ready to give love, hope and
care.
V. METHODOLOGY
This activity was carefully planned by the SSG adviser and officers upon knowing that
ma’am THUDONGMI Foundation will again sponsor the activity. The SSG officers planned for
the implementation on February 22, 2020 at Dakias Elementary School as suggested by the
sponsors.
Upon the approval of the school principal, the SSG adviser and officers and other
concerned individuals were informed for the activities to be undertaken with specific
scheduled date.

Student Government (SSG), the SSG is the foremost co-curricular student organization
authorized to operate and implement pertinent programs, projects and activities in school
nationwide. It lays the groundwork for good governance, volunteerism, unity and cooperation
by providing the students various venues where they can improve their leadership knowledge,
skills and attitudes
Thus, the SSG Officers together with their very supportive adviser Ma’am Josephine
Danishad their STNHS BOOK,TOY DRIVE, PERSONAL HYGIENE and GIFT GIVING at
Tanglag Elementary School, Tanglag Rosario La Union. Their mission was not to deliver
books and toys, not to have personal hygiene and gift giving but since Mrs. Jung Suk Choi
and the able principal of Sto. Tomas National High School donated paint. Because of these
two sponsors who have a generous heart, the SSG officers and adviser were happy to
include wall painting in their mission as requested by Tanglag Elementary School.
The SSG adviser ma’am Josephine Danis together with the TES Principal instructed the
officers on what to do for the painting. Before painting, the SSG officers cleaned first the wall.
Other officers mixed the paint to be use in painting. All officers were very excited to paint the
wall. Though it was too hot and tiring task, the officers were able to paint the walls found near
the school ground of Tanglag Elementary School from 9:00 to 11:00 in the morning.
“We hope to have the same activity to other schools again” said by some of the Supreme
Student Officers.

In our life, we’ve learned that true happiness comesfrom giving. Helping others along the way
makes us evaluate who we are. Being able to help is a privilege for the Sto. Tomas National
High School Supreme Student Government Officers and adviser for it teaches us how to
become concerned to those who are in need, it helps us to understand the spirit of
compassion and to share what we have to and to abolish the selfishness hiddendeep within
us.
As part of the project andactivities written in the STNHS SSG Action plan found in KRA 5
Partnership and Linkages: Community Services, theSSG Organization conducted STNHS
Book, Toy Drive, Personal Hygiene and Gift Giving at TanglagElementary School,Tanglag
Rosario La Union on December 11, 2019. There were 107 pupils as chosen beneficiaries of
the said event.
Simple program was organized by the SSG Adviser and officers. Grade 11 Representative
JayffersonSabido was the master of ceremony for the morning activities. The group brought
prizes and organized games for the pupils to enjoy. Games and other activities were actively
participated by the pupils. Every pupil who won in the games were given prizes. We also
conducted TOYGIVING to the 107 beneficiaries. The officers led by the SSG president Dan
Tonton guided the pupils to choosetoy they wanted. Toys and prizes were sponsored by
Jung Suk Choi who is from South Korea. We worked as one for the success of the activity.

Aside from the DepEd Vision and Mission, the Core Values are also essential. They serve as
guide in doing the teaching and learning process. One of it is the love for fellow men or Maka-
tao which is simply means respecting each other’s rights and opinions regardless of the
existing differences in between. Filipinos are naturally kind, gentle, generous, hospitable,
hardworking, obedient and polite which make a great impact to the world.
In connection to this, the SSG adviser Ma’am JospehineDanis with the SSG officers had the
Part III of the STNHS BOOK,TOY DRIVE, PERSONAL HYGIENE and GIFT GIVING
Outreach Program which is the Gift Giving and Personal Hygiene. Sto. Tomas National High
School 32 teaching and non-teaching personnel headed by the head teacher sir Edgar P.
Soriano and School Head sir Danilo P. Gayaosupported and helped them to conduct
Personal hygiene and Gift Giving on the same day December 11, 2019, 1:00 – 3:00 P.M. at
Tanglag Elementary School, tanglag Rosario La Union.
SSG Vice President Lester Lee Lorenzo served as the master of ceremony started the
program by calling Ms. Vernalyn Andres, SSG Secretary for the opening prayer, asking God’s
presence in the said event. It was followed by a welcome remarks from the Tanglag
Elementary School energetic principalma’am MA. LORELIE A. DOMINGO, Ph.D. She thank
everyone for visiting and chosing their school as beneficiaries of our outreach program. Right
after the opening and welcome remarks, an inspirational message was delivered by sir Danilo
P. Gayao.
Personal hygiene activity was prepared by the SSG officers, they distributed one toothpaste
and one toothbrush to each pupil and asked them to form a circle with the help of the STNHS
teachers and TES teachers. The officers taught them how to put toothpaste and how to brush
their teeth.
In addition, SSG Adviser and officers together with the faculty and staff of Sto. Tomas
National High School distributed the gift to each 107 beneficiaries of the said activity. Inside
the gift were crayon, notebook, pad paper, eraser, pencil, sharpener and pencil case.
Everyone wears a big smile in their faces as they received their gifts from the group. STNHS
were very happy and looking forward for another gift giving activity in the near future.
Having a generous heart STNHS family collected some money to be given as prizes to those
pupils who will win the game “GIVE ME” that was facilitated by Sir DaniloGayao and the SSG
Officers.
After the outreach program, the group realized that people can search for their happiness, just
enjoy what people doing and that gives happiness to each person in the activity. This activity
also thought the officers a lot of good things; contentment with what they have, care for the
people around them, and this makes them realized the importance of a friend in ups and
downs of life.
