By filing this application, I agree to voluntarily deduct my contributions to my Pag-IBIG


Fund MP2 Savings and confirm that I understood the following terms:

● This deduction is on-top of the mandatory Pag-ibig contributions which is currently


deducted to my payroll based on my actual gross earnings.

● I hereby authorize LizardBear Tasking, Inc. to deduct the chosen amount below
from my salary (Kindly tick or check chosen deduction scheme)

o One Time Deduction amounting to Php________ which will be deducted in the


nearest applicable payroll (Minimum amount of Php500.00).

o Recurring deduction amounting to Php 1,000.00 which will be


deducted per payroll/cut off (Minimum amount of Php500.00)

o Recurring deduction amounting to _______ which will be deducted every


end of the month payroll (minimum amount of Php500.00)

● This deduction will start in the nearest applicable pay period where I submitted or
signed this authorization.

● For continuous deduction/payment, I hereby authorize LizardBear Tasking, Inc to


continuously deduct this amount until such time that I submitted another request to
cancel this voluntary application.
