I, Paz S. Cuaresma, hereby authorize my daughter, Juvelene C.


Bondaug, to claim my pension from the office for Senior Citizens Affairs
on my behalf. I will be in Siquijor to attend the needs of my
grandchildren for a while.

I have enclosed my Senior Citizen Identification card as a proof that I


have authorized Juvelene C. Bondaug to do so.
