in 1997 by Dr. William S. Barnes, senior pastor of St. Luke’s United
Methodist Church, as a grassroots effort to solve a community issue

locations and patient capacity and as a nationally recognized nonprofit
health and human services agency.

specialty care providers, multiple faith congregations, community
organizations, foundations and corporations
-Patients eligible for services through Shepherd’s Hope must meet one of
the following criteria:
1. Have income at or below 200% of the poverty level.
2. Not be eligible for government assisted health care programs like
Medicare or Medicaid.
3. Demonstrate that they are currently uninsured.
-whereas the cost for a patient visit at a Shepherd’s Hope is the equivalent
of $120.

Orange Blossom Family Health- Health care center for the Homeless

- Charges for uninsured patients will be discounted based on a sliding fee
scale. Co-pays for insured patients will be determined by their individual
insurance carrier.
-Health Care Center for the Homeless, Inc. is a Federally Qualified Health
Center (FQHC) and
a Federal Tort Claims Act (FTCA) Deemed Facility, covered by
professional liability insurance through our enrollment in the Federal Tort
Claims Act program.
-Health Care Center for the Homeless (HCCH) is a 501(c)(3) organization
founded in 1993 by Dr. Rick Baxley, a volunteer physician who saw the
need to provide quality health care services to the homeless population in
Orlando, Florida.
-HCCH provides health services for anyone who is homeless and resides in
Orange, Osceola, and Seminole Counties. HCCH
also provides care for residents of Orange County who have a home but
are uninsured and whose income falls below 200% of the current Federal
Poverty Guidelines.
-In addition to primary medical care, programs available at our main
location include oral health services, behavioral health and substance
abuse counseling, vision services, an on-site pharmacy, a residential TB
shelter, mobile health services, and street outreach - our HOPE Team.
-The Health Care Center for the Homeless is fortunate to receive
government funding from the federal,
state, and local levels. We also receive private financial and in-kind
support to provide our many services
-As we care for uninsured or underinsured patients, we do not accept
private insurance. We currently accept
straight Medicaid, Medicare, Medipass, Wellcare, Staywell, Healthease, &
Prestige Healthcare.
- If this will be your first time coming to our Health Center, your initial visit
will be as a walk-in patient. If your
provider here determines they need to see you again within an allotted
time, the front desk will schedule your next appointment.
-The cost of our services is determined by a sliding fee scale based on total
household income, the number of persons living at your residence, and in
accordance with the current Federal Poverty Guidelines. The minimum
charge for a medical visit is $10.00; for a dental visit - $10.00; for a
behavioral health visit - $10.00; for pharmacy - $6.00 per prescription.
Please be aware these are minimum charges only and amounts will vary
based on the service or services provided. For patients unable to afford
the charge for services provided, assistance is available
-HCCH does accept walk-in patients in our medical clinic and they will be
seen in order based on the urgency of their current illness or health issue.
For our dental clinic, walk-ins are seen on Mondays at 8am and 1pm and
Wednesdays at 8am and are seen in the order they arrive.

began its operations in 1972 in Apopka, Florida.

-We accept most types of insurance plans, including Medicare, Medicaid,
HMOs, PPOs, KidCare, Medikids, Healthy Kids, Children’s Medical Services
(CMS), and other plans. Certified Application Counselors are available to
assist in finding and enrolling in insurance options through the
marketplace.
-Payment is expected on the day services are provided. Co-pays or the
nominal fees are collected at check-in. At check-out, additional fees may be
due based on the services you received.
-You are encouraged to apply for our sliding scale program. If eligible, the
program may reduce costs for the uninsured or be applied to insurance
deductibles and co-insurances. We serve all patients regardless of their
ability to pay.

you wish to apply to be a patient. A phone interview is required to
determine eligibility prior to submitting an application. No walk-ins’
applications are accepted. If you need immediate medical assistance,

-ALICE is Asset Limited, Income Constrained, and Employed individuals
who live and work in our communities. The ALICE population often earns
more than the Federal Poverty Level, but less than the cost of living. Thirty-
five percent of households in Orange County fit the ALICE profile with
14.2% of Orange County households living in poverty. Often, as multiple
expenses arise that take priority, the ALICE population neglects the need
for health care to use their funds and time on other necessities, often due
to intersecting social determinants of health. Racial and ethnic minorities
carry the heaviest burden of health disparities in the United States related
to health prevalence and premature mortality from chronic health
conditions.
-Our patients have been left out of the healthcare system either because of
race, immigration status, lack of wealth, or lack of experience to navigate a
complex health system. Grace Medical Home serves as a local health
facility working to achieve health equity. Our physical, mental and spiritual
health services for the low-income, uninsured in Orange County allow
people to attain their full potential of health and well-being. Grace is
dedicated to filling the health care gaps that affect our local communities by
providing whole-person health care to tackle health disparities. We provide
the safety net and deliver care to those who might otherwise go without.

Grace continually seeks to break down barriers to care to achieve health

Grace Medical Home is a non-profit medical facility serving the working,
low income and uninsured residents of Orange County. Grace Medical
Home provides comprehensive adult and pediatric care. Patients pay a
yearly registration fee as well as a flat fee at each office visit – fee is based
on family size and household income.

-Living in Orange County for the last two months
-Uninsured and not enrolled in any government-assisted healthcare
programs (such as Medicaid, Kidcare, Healthy Kids, Share of Cost,
Medically Needy, Medicare or VA Benefits)
-Currently employed, OR have been employed within the last six months,
OR a full-time student, OR a single parent of a child under age six
The monthly gross income of your family is equal to or less than 200% of
the Federal Poverty Line - see income guidelines in the back. Between the
ages of 0 and 64
*Proof of income and that you live in Orange County is required at
registration appointment. Children 18 and younger must be accompanied
by parent or legal guardian. Proof of legal guardianship is required.
