PREMISES

Section 1. Responsibilities of the Supervised Field Experience Supervisor:

1. Provide a basic orientation on the nature and function of the officer.


2. Assign tasks and works to the trainee/s.
3. Serve as the contact person of the Supervised Field Experience Adviser in the workplace.
4. Evaluate the trainee/s according to his/her/their performance through a Performance
Appraisal Form.
5. Certify and validate all requirements that the trainee/s shall submit to the Supervised Field
Experience Adviser; and
6. Act as the disciplining authority in case a misbehavior, misconduct, and neglect of duty on
the part of the trainee/s

Section 2. Responsibilities of the Supervised Field Experience Adviser:

1. Provide guidance and supervision over the trainee/s for the duration of program.
2. Monitor the trainee/s performance through a Supervised Experience Supervisor.
3. Provide all the necessary guidelines in the Supervised Field Experience program; and
4. Serve as the contact person of the Supervised Field Experience Supervisor in case of
misbehavior, misconduct, and neglect of duty on the part of the trainee/s.
Section 3. Responsibilities of the Trainee/s:

1. Perform the functions, duties and responsibilities assigned to them by the supervised Field
Experience Supervisor for the furtherance of their understanding of the program’s objectives.

2. Report regularly to the place of work in accordance with the agreed time with the Supervised
Field Experience Supervisor; and

3. Comply with all the Supervised Field Experience guidelines as given.

Section 4. Other terms and conditions:

1. It is understood that the trainee/s will not use or disclose others, without the written
consent of the Laboratory, any information that they obtained during or in relation to their
training and which are all considered confidential.

2. The ME 98 Supervised Field Experience program shall start on January 2023 and end
in May 2023; and

3. There shall be _ trainees who will be assigned in the Supervised Field Experience,
namely:

Section 5. Conduct Unbecoming:

1 In case of misbehavior, misconduct, and neglect of duty on the part of the Supervised
Field Experience trainees/s, the Supervised Field Experience Supervisor shall immediately report
to the Supervised Field Experience Adviser the incidents and circumstances involving such
conduct and behavior with stern warning in the Supervised Field Experience trainee/s.
