ETHICS ASSIGNMENT – 2020

Using all available resources, provide concise answers / comments to the following ethical issues in emergency care.

1. It is not always appropriate to initiate CPR / resuscitation.

Describe some situations where you feel CPR / resuscitation is not appropriate in your clinical setting.

2. The HPCSA in their ethical booklet on “withholding and withdrawing care” (Available on their website) discourage treatment
(and therefore resuscitation) when the prognosis is futile.

Discuss the concept of futility and provide a few examples that you may encounter in your clinical setting.

3. You and your ACLS team have been attempting resuscitation on a 30-year old male patient who collapsed after a severe
asthma attack. After 10 minutes of intensive efforts the patient’s brother rushes in requesting that you withhold further
resuscitation and presents you with a document entitled “Living Will”.

Would you continue resuscitation of this patient? Are living wills valid in South Africa?

4. Your institution has requested that you draft a protocol / guideline on termination of resuscitation efforts.

Make notes on what considerations will need to be made prior to terminating an active resuscitation.

5. The issue of relatives being present during a resuscitation is controversial. What are your thoughts regarding presence of family
members during a resuscitation? What considerations need to be taken if family members are allowed to witness a
resuscitation?
