Please take note of the following:
 For those who are enrolled in colloquium, thesis and dissertation writing the
first time (or nth time) and need to submit the Request for Titles and Adviser,
please follow the following steps:

1. Secure the downloadable form from the UEP-Graduate Studies Facebook


page.
2. Indicate three (3) titles, the Rationale, Objectives of the Study, brief
description of the Methodlogoy, and the Literature Matrix.
3. Submit (2) to the GS Office for evaluation/finalization.
4. After finalizing the title, request the approval/confirmation of the assigned
adviser.
5. If the student is with the DepEd, submit the Request for Titles and Adviser
to the DepEd Research in-charge for the issuance of certification if the
suggested title is within the DepEd Research Thrust.
If not connected with the DepEd, proceed to (6).
6. Secure Permit to Study from your employer/agency where you are
working.
7. Return/Submit the documents at the GS Office for the final approval of the
Dean
8. Work on your proposal
9. Submit your proposal manuscript to the GS Office : 5 copies for thesis and
colloquium proposal and Re-Entry Plan, 6 copies for Dissertation Protocol.
The proposal must be indorsed by the adviser.
10. Wait for your schedule for the pre-oral defense/examination.

2. Objectives of the Study
3. Brief description of the Methodology
4. Literature Matrix using the format below
