1.0 PROCEDURE

NOTES:
(1) When a technical question, deviation, or concession is generated the originator must provide a definitive
statement in sufficient detail to enable those not directly involved with the onsite problem to understand the
query and its implications. Sketches and/or diagrams are to be included where appropriate.

2.0 RESPONSIBILITIES
2.1 CIR Initiator
 Ensuring that all queries are prepared in accordance with this procedure.
 Ensure CIR raised accurately and all boxes filled in.
 Proposing suitable solutions to queries
2.2 CIR Coordinator
 Allocation the correct sequential numbers.
 Issue registered CIR to Operation management for review and respond
 Ensure data base updated accordingly CIR Status
 Distribute CIR Status report weekly in according with distribution matrix
 Ensuring all affected documentation and drawings are revised and all approved CIR are
incorporated.
2.3 Operations Management
 Assess change, ranking CIR.
 Approval or reject for further action.
 Assign a CIR action person.
 Answering the CIR within the time limit required with a positive reply.
