Teacher Performance Monitoring Form

The teacher worked to monitor learning and provide feedback to improve student performance through distance and limited face-to-face learning. This included creating intervention plans and exploring virtual and regulated in-person options. Assessment and grading strategies needed improvement. The teacher later implemented individual learning plans with home visits and tutorials.
