The administration of anticoagulant drugs to Mr. Andi aims to prevent blood clot formation, which could further obstruct the coronary arteries and exacerbate his chest pain. This treatment is essential in managing potential acute coronary syndrome episodes by reducing the risk of further cardiac events .

The communication between the nurse and the doctor reflects effective interdisciplinary collaboration through clear, structured communication. The nurse provides a detailed report on Mr. Andi's symptoms and medical history, queries the doctor’s input for further patient care, and summarizes the agreed plan of action, thus ensuring that both parties are aligned on the care approach .

Mr. Andi's agitation and rapid, shallow breathing are significant as they may indicate acute distress and increased respiratory effort, often associated with a cardiac event or severe anxiety due to cardiac insufficiency. This highlights the urgency and need for immediate medical evaluation and intervention .

The doctor recommends continuing the breathing exercise therapy, administering parenteral anticoagulant drugs to reduce chest pain, and ordering an EKG for further assessment of Mr. Andi’s cardiac condition .

Ordering an EKG for Mr. Andi is important because it can provide crucial information about the heart's electrical activity, helping to rule out or confirm the occurrence of a new cardiac event such as a myocardial infarction or arrhythmia. This diagnostic measure is essential given Mr. Andi's existing symptoms and recent medical history of a heart attack .

Mr. Andi's vital signs, including a high pulse rate of 124 beats per minute and low blood pressure of 100/58 mmHg, indicate a cardiovascular system under stress. These readings suggest potential hypotension and tachycardia, which can be signs of cardiac dysfunction or distress, contributing to the concern of a new cardiac event .

The nurse demonstrates critical thinking by considering Mr. Andi's recent medical history of a heart attack, recognizing the significance of his current symptoms such as chest pain and shortness of breath, and expressing concern for a potential new cardiac event . The nurse proactively seeks guidance from the doctor and suggests continuing breathing exercises while starting anticoagulant therapy to address chest pain.

The nurse's report effectively follows the SBAR protocol: Situation - introducing herself and stating the patient’s key symptoms; Background - providing Mr. Andi’s medical history and current vital stats; Assessment - expressing concerns about a potential new cardiac event; Recommendation - suggesting specific interventions like breathing exercises and anticoagulants. This structured communication ensures clarity, improves patient safety, and supports effective decision-making .

The nurse's actions reflect professional responsibility by carefully monitoring Mr. Andi’s condition, continuing prescribed treatments like breathing exercises, and seeking further guidance for appropriate interventions. Her focus on addressing his symptoms promptly and consulting the doctor for additional measures underscores a commitment to patient-centered care, ensuring Mr. Andi receives comprehensive and timely treatment .

Mr. Andi exhibits symptoms including shortness of breath, chest pain, a high pulse rate of 124 beats per minute, low blood pressure at 100/58 mmHg, restlessness, and rapid, shallow breathing . These symptoms are concerning for a potential new cardiac event given his recent history of a heart attack.
